About this file
This is an Inspection Worksheet template for quality assurance evaluations of Laundry and Dry-Cleaning (L&DC) services at contractor locations under the Fort Meade L&DC contract (W91QV126QA033). The worksheet documents observations and discrepancies identified during inspections conducted by the Contracting Officer's Representative (COR) at the contractor's facility, requiring signatures from both the QAE (Quality Assurance Evaluator) and the contractor's representative upon completion.
The worksheet establishes three specific performance standards with corresponding acceptable discrepancy thresholds: use of bio-based laundry and dry-cleaning products (2 valid discrepancies per inspection period), cleanliness and serviceability of all linens per manufacturer care labels (10 valid discrepancies per inspection period), and proper cleaning of specialty items such as OCIE sleep systems per manufacturer instructions (5 valid discrepancies per inspection period). Inspections are conducted using random sampling methods, and any discrepancies exceeding the established thresholds must be documented and addressed by the COR with the contractor on-site. The worksheet captures essential administrative information including unit/organization, contract number, task order number, inspection date and time, building number, PRS number, and PWS paragraph references. While contractor signature is not mandatory, it becomes required if discrepancies are identified during the inspection period.

Instructions
| INSTRUCTIONS/ GUIDELINES |
| Observations made should be able to be defined and documented on this worksheet. |
Discrepancies over allowed threshold example: 6 sheets were damp = 6 sheets were damp within the sample size, informed the vendor of the observation/deficiency. 5 valid discrepancies per inspection period are allowed, anything above 5 is documented on the inspection worksheet.
Contractor must sign the inspection worksheet. This does not hold the contractor at fault for the findings of this inspection
Contractor is not required to sign, but if discrepancies are found then the COR must address this with the contractor onsite.
