Past Performance Questionnaire: Instructions This questionnaire follows the format of Contractor Performance Assessment Reporting System (CPARS) guidelines for ‘Non-Systems.’ The data fields included on the attached form have the same character limitations that are set in CPARS.

The CPAR(s) of record for the contract citations identified on the Contract Data Matrix (CDM) finalized during the period of performance defined as ‘recent’ will be used to assess the quality of the offeror’s performance. In the event the contract citation does not have a CPAR of record, questionnaires will be issued by the GOVERNMENT to the points of contact (References) indentified in columns “M” and “N” of the CDM.

PPQ Form Part 1 is to be completed by the offeror; the Government will not complete any section of Part 1. It is encumbent upon the offeror to ensure data provided in Part 1 is complete and accurate. Incomplete / inaccurate data will be a reflection of the offeror’s quality of products and services.

PPQ Form Part 2 will be completed by the evaluator(s) (i.e., references identified in columns “M” and “N”).

The notional process that may be used by the government for processing of CPAR / Past Performance Questionnaire data is shown in the following figure.

The PPQ form is built in Micrsoft Word; after enabling content, please ensure you are able to open, use, save and print the data before completing.
