About this file
This document is a Recorded Program Eligibility Questionnaire from the Department of Early Learning and Care (DELC) in Oregon. It outlines the requirements for a school-age recorded program to become registered with DELC's Child Care Licensing Division. The questionnaire covers topics such as whether the program is a group athletic or social activity, focuses on a single enrichment activity, is operated by a school district or government agency, serves participants 14 years and older, or provides care while a parent is on-site. If the program does not meet any of these criteria, it must complete the questionnaire and submit it to DELC for review. Once received, DELC will contact the program to provide information on the registration process or confirm the program is not required to register.
The document provides instructions for completing and submitting the questionnaire, including contact information for DELC's Customer Service. It also defines a "parent cooperative" program, which operates for no more than 4 hours per day, is overseen by a board, and has care provided by rotating parent members. Programs are asked to provide details on their name, contact information, location, ages served, hours of operation, and a brief description of the program.
