About this file

This is a Past Performance Questionnaire used for source selection of the MacDill AFB Flooring Services program (FA481425R0002) being procured by the 6th Contracting Squadron of the Air Force's 6th Air Refueling Wing at MacDill AFB, Florida. Respondents to this questionnaire are previous customers or agencies that have direct experience with the offeror's flooring services performance and are required to provide their written response within 10 calendar days of receipt.

The questionnaire collects information across five main sections: contract identification (contractor name, CAGE code, contract number, services description, period of performance, dollar value, role as prime or subcontractor, and flooring project details including square footage); customer or agency identification; respondent identification; performance information using a six-tier rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable); and general comments regarding the contractor's ability to execute contracted services. Performance evaluation covers two primary categories—Contract Administration/Management (seven criteria addressing responsiveness, pricing, compliance with facility rules, and invoicing accuracy) and Project Execution (eight criteria assessing project timeline, quality of flooring installation, labor adequacy, furniture handling, site maintenance, debris removal, and occupant interactions). The questionnaire includes a specific question regarding any partial or complete terminations for default or convenience, and concludes with a final assessment question asking whether the respondent would award the contract to the contractor today if given a choice. Completed questionnaires must be emailed to thomas.grove.1@us.af.mil and 6cons.pkm.flight@us.af.mil.
