About this file
This document is an extensive Asbestos Inspection Report prepared by Gobbell Hays Partners, Inc. for the Colorado Department of Corrections regarding the Fort Lyon Correctional Facility located in Fort Lyon, Colorado. The inspection, conducted on July 24, 2006, comprehensively assessed asbestos-containing materials (ACMs) across multiple buildings on the campus, which originally consisted of structures built between the late 1800s and 1980s, previously owned by the Navy and Veterans Administration before becoming a state correctional facility in 2001. The report details asbestos content in various building materials including floor tiles, sheet vinyl, thermal system pipe insulation, wall plasters, roofing materials, and construction adhesives, with asbestos concentrations ranging from trace amounts to 90% chrysotile in some materials.
The inspection revealed significant asbestos contamination across the facility, with most buildings containing moderate to substantial amounts of ACMs. Notably, the report identifies contaminated crawlspace soils, steam tunnels, and even exterior ground areas between buildings. The findings recommend immediate management and potential abatement of asbestos-containing materials, with specific guidance for each building and material type. The report emphasizes the need for careful handling, proper personal protective equipment, and adherence to EPA and Colorado Department of Public Health and Environment regulations when managing or removing these materials. Several partial abatement projects were already completed between 2006 and 2009, addressing thermal system insulation, contaminated soils, and specific building areas.

1.0 INTRODUCTION
On July 24, 2006, Gobbell Hays Partners, Inc. (GHP) initiated a facility wide asbestos building inspection of the Fort Lyon Correctional Facility in Fort Lyon, Colorado. The purpose for the inspection was to test/quantify suspect asbestos containing materials
(ACMs) that exist, assess the condition of confirmed ACM materials, and make recommendations regarding the management of all confirmed ACMs in buildings throughout the facility.
GHP understands that the facility has been used as a State Correctional Facility since
2001. Prior to this, the facility was owned/operated by the Navy and more recently by the Veterans Administration as a hospital. Records indicate that original buildings on this campus were constructed in the late 1800s and early 1900s, and buildings were added to the campus through the 1980s.
Records indicate that a minimal amount of asbestos abatement was performed on some housing units and in Buildings 5, 6, 8, and 401 in 1989. Some housing unit sites that had been demolished in the past were also cleaned up during that time frame. At some point, the Kit Carson Chapel was moved from the plot of land east of Building 5 to the main entry to the FLCF complex. According to records and interviews with FLCF staff, the majority of the original buildings inside the fenced/gated portion of the campus, (non-residential buildings) underwent varying levels of asbestos abatement to accommodate renovation and/or retrofit for the transition from a VA hospital to a DOC secured facility.
Prior to initiating the facility wide asbestos assessment at FLCF, GHP reviewed documents from an asbestos inspection conducted in 1990 by Occusafe, Inc., when the facility was the Veterans Administration Hospital. GHP found the asbestos inspection report/lab data incomplete for several buildings and determined it was necessary to acquire/analyze an additional five hundred and ninety-five (595) bulk samples of suspect
ACMs to comply with current EPA/CDPHE/OSHA regulations.
The Occusafe, Inc. inspection report did not mention ominous maintenance practices used by the former owners which were communicated to GHP in interviews with former
VA employees such as “hammer milling” of used ACM pipe insulation and re-using those ACMs as patch insulation materials and attic insulation which resulted in severe contamination of the Dairy Barn. The Occusafe, Inc. Inspection Report made no mention of many large barrack type buildings that had been demolished prior to DOC acquiring the property, which resulted in asbestos contamination of a significant amount of soils on the campus. GHP also acquired/analyzed four hundred and eighty-nine (489) samples of soils to determine asbestos content and/or extent of asbestos in soils contamination.
Bulk samples were collected by GHP of all accessible known or assumed suspect ACMs in buildings and soils per the EPA/OSHA and CDPHE asbestos inspection regulations.
Bulk samples were given unique alpha-numeric identification numbers, consisting of three parts and labeled according to EPA regulations. The first letter “B” designates the sample as a bulk asbestos sample. The first set of numbers identifies the GHP project
Asbestos Inspection Report Page 2 number for the project. The second group of numbers represents the sequential sample acquired for a project. Project number 06505.01 is for initial site assessment, and
06505.03 for the asbestos in soils asbestos assessment. GHP Project Number 06505.02 reflects asbestos abatement projects completed concurrent with the site assessment work at FLCF in the fall of 2006. A description of each bulk sample and the sample location has been included. Sample locations and laboratory results are presented in Appendix A.
Bulk asbestos samples were randomly collected from homogenous areas of suspect ACM by Wade E. Anderson, Shawn R. Lopez, Jose Montoya, and Ernesto Rodriguez, all
EPA/State of Colorado certified asbestos inspectors. All bulk samples were submitted to
Reservoirs Environmental Services Inc. (RESI), a third party independent laboratory.
RESI is accredited through the National Institute of Standards and Technology (NIST) and participates in the NIST National Voluntary Lab Accreditation Program (NVLAP) as required by EPA. Bulk samples were analyzed by Polarized Light Microscopy (PLM) in general compliance with guidelines established by the US EPA (40 CFR Part 763, Subpart F, Appendix A). Asbestos concentrations were visually estimated and/or point counted and reported in percent for each layer of the sample.
In summary, most buildings on the FLCF campus contain moderate to significant amounts of ACM building materials. During the course of completing the building asbestos inspections, GHP discovered that asbestos contaminated soils also exist at the
FLCF. Steam tunnels that supply high pressure steam to all of the buildings on campus and contain damaged ACM thermal system insulation (TSI) were thought to have been the source of trace amounts of chrysotile asbestos in soil samples acquired at the site.
However, utilizing aerial site photos dating back to the 1930s, GHP determined that demolition of former structures at the site was the main contributor to the asbestos in the soil contamination. See GHP’s Asbestos in Soils inspection report/assessment identified as GHP Project Number 06505.03.
This report includes the description and location of all ACM building materials tested by
GHP and laboratory analysis results of all acquired bulk samples. This report also references identified ACM as determined by the 1990 Occusafe inspection report.
Drawings have been developed to assist DOC personnel in identifying and locating known or assumed ACMs at the FLCF campus. Note: FLCF was operational at the time of the asbestos inspection; therefore, inspections/bulk sampling was non-intrusive.
Suspect ACM that is obscured or hidden, i.e., inside wall cavities/pipe chases or under non-ACM floor coverings may have gone undetected during this inspection. Appropriate precautions should always be taken when entering wall cavities/pipe chases and/or removing non-ACM floor coverings.

3.0 REGULATORY REVIEW
EPA and the Colorado Department of Public Health and Environment (CDPHE) regulate all building materials that contain greater than 1% asbestos as determined through polarized light microscopy (PLM) analysis. OSHA regulates all building materials that contain smaller amounts of asbestos (even trace concentrations by PLM).
EPA and CDPHE Regulation No. 8, Part B, requires that suspect ACM in buildings built prior to 1988 be assumed to be asbestos or an inspection be conducted by an EPA accredited/CDPHE licensed asbestos building inspector. CDPHE, Regulation No. 8, Part
B, requires an asbestos inspection following the AHERA protocol be conducted prior to demolition or remodeling activities that could disturb ACM.
The EPA and CDPHE require all friable and non-friable ACMs be removed prior to renovation activities that may disturb the ACM, and all friable ACM must be removed prior to demolition.
In some circumstances, “point count” analysis is required for bulk samples previously analyzed by PLM. Point counting is a more detailed means of analysis than standard
PLM. Federal and State agencies define ACM as materials containing greater than 1% asbestos. The NESHAP regulation requires that if standard PLM analysis determines that a sample contains less than 10% asbestos, the material must be considered asbestos-containing or be point counted. Even if the sample is less than 1% by standard PLM, the material either has to be assumed to be ACM or point counted. If the point counting analysis is different than the PLM analysis, the point counting result takes precedence. If standard PLM analysis determines that a material has no asbestos or that the material contains greater than 10% asbestos, point counting is not necessary.
The requirements of CDPHE Hazardous Materials and Waste Management Division
Section 5.5 apply to the owner or operator of any property with asbestos-contaminated soil at which soil-disturbing activities are occurring or planned for any area containing asbestos-contaminated soil, but excluding asbestos waste disposal areas that have a
Certificate of Designation. The requirements of this Section 5.5 are triggered when the owner or operator has reason to know of asbestos-contaminated soil at a site, (such as through confirmation by analysis of observed material that is suspected of containing asbestos), or has reason to believe that visible asbestos may be encountered. An owner or operator who has no reason to know of asbestos-contaminated soil at a site, and has no reason to believe that visible asbestos will be encountered, does not have a duty under these regulations to sample or otherwise investigate for asbestos-contaminated soil prior to commencing excavation, or other soil disturbing activities, at the site.

4.0 ACM BY LOCATION
BUILDINGS
Building 3
Approximately 2,888 sq. ft. of various colored/patterned ACM sheet vinyl floor coverings exist throughout various locations of the building, (see drawings for locations), (12-22% Chrysotile). This material was in good condition at the time of this inspection exhibiting only minor marks and wearing.
Approximately 5,768 sq. ft. of various colored/patterned, 9” x 9” and 12” x 12” ACM floor tiles and associated adhesives exist throughout various locations of the building, (see drawings for locations), (2-25% Chrysotile). This material was in good condition at the time of this inspection exhibiting only minor breakage and wearing.
Floor tiles and sheet vinyl are normally considered non-friable if the material is in good condition and repair could be performed during routine operations and maintenance activities assuring that the material is not cut, abraded, or in any other way damaged causing release of asbestos fibers or the material must be properly abated prior to renovation/demolition activities.
Approximately 2,200 l.f. of ACM Thermal Systems Pipe Insulation (TSI) exists throughout the building, (see drawings for locations), (10-70% Chrysotile). This material was in good to poor condition at the time of this inspection. GHP inspected/assessed only those materials that were accessible. Access panels were not available for performing a thorough survey of pipe insulation inside wall cavities and/or pipe chases.
Pipe insulation inside walls/pipe chases should be assumed to be asbestos containing until determined otherwise. GHP determined that fiberglass insulation is obscuring remnant TSI on pipes. All fiberglass insulation should be assumed to be covering ACM material. Personal protective equipment should be worn by those entering wall cavities/pipe chases. Isolate the areas surrounding pipe chases when performing routine maintenance in these areas. Damaged material should be repaired immediately. The TSI in the crawlspace and mechanical room was abated in September 2006, GHP Project #
06505.02.
Approximately 9,650 sq. ft. of ACM wall/ceiling plaster exists throughout the facility, (see drawings for locations), (Trace-2 % Chrysotile). The material was in good condition at the time of this inspection. This material should be managed following the facilities
Operations & Maintenance Program. The material should be properly abated by a State of Colorado licensed General Abatement Contractor prior to renovation/demolition or if the material becomes damaged.
Approximately 360 sq. ft. of ACM wall tile exists at various locations throughout the building basement, (see drawings for locations), (4% Chrysotile). The material was in good condition at the time of this inspection. This material should be managed following
Asbestos Inspection Report Page 6 the facilities Operations & Maintenance Program. The material should be properly abated by a State of Colorado licensed General Abatement Contractor prior to renovation/demolition or if the material becomes damaged.
Approximately 1,200 l.f. of ACM baseboard adhesives exist throughout the building, (see drawings for locations), (2% Chrysotile). This material was in good condition at the time of this inspection. This material should be managed following the facilities Operation &
Maintenance Program. The material should be properly abated by a State of Colorado licensed General Abatement Contractor prior to renovation or if the material becomes damaged.
Approximately 900 sq. ft. of gypsum board with asbestos containing joint compounds exists in the auditorium, (see drawings for locations), (9% Chrysotile). The material was in good condition at the time of this inspection. This material should be managed following the facilities Operations & Maintenance Program. The material should be properly abated by a State of Colorado licensed General Abatement Contractor prior to
Approximately 3,000 sq. ft. of ACM tar impregnated roofing materials and flashing materials exist on the roof, (see drawings for locations), (15% Chrysotile). This material was in good condition at the time of this inspection exhibiting only minor weathering damage. Non-friable tar impregnated roofing materials are not regulated by the EPA or
State of Colorado provided that the materials are well maintained and remain non-friable during removal, replacement, or repair. This material can be repaired or replaced by properly trained personnel (OSHA training) provided that the material is not cut, sanded, grinded, or abraded using mechanical means.
