Appendix E QASP/PRS
QUALITY ASSURANCE SURVEILLANCE PLAN/
PERFORMANCE REQUIREMENTS SUMMARY
The Contract Requirements listed in this combined Quality Assurance Surveillance Plan (QASP) and Performance Requirements Summary (PRS) summarize specific firm fixed price tasks that are to be performed under this contract.
QASP
A) Completion of Work. Work must be completed, or be within contractual timelines to be completed, to receive payment. Said another way, late work is not eligible to receive payment.
B) Retainage. The Government shall retain payment for late work monthly. Late work data shall be pulled from the Government CMMS system and any work items still in progress past their target completion date will be put onto a list that will be delivered with the payment of the monthly invoice. The Contractor can request payment for completed work when acceptable items from the list are complete in a subsequent monthly invoice.
Retainage Amounts (per work item) Work Order $ 2,000.00 Performance Directive $ 250.00
PM $ 250.00
a) Late PMs. A PM is only relevant for limited amount of time. As such the government will permanently retain money for PMs not completed within the below timeline.
Maximum Lateness of PM Annual (or more) 3 months Semiannual 1 month Quarterly 1 month Monthly 1 week Weekly 1 day
i) There is a limited allowance for less than 100% completion. If within the Contractor’s allowances, some incomplete PMs can be marked “skipped” with no retainage. For example, if the PRS requires a 90% completion rate, the contractor can mark a small percentage of the work as “skipped” with no retainage.
C) Key and Designated Positions. The Government reserves the right to deduct the CLIN Schedule value or fair market compensation package amount, as determined by the government, for time the Contractor does not provide fully qualified personnel defined in sections 7.2 and 7.4. For example, if an EMCS technician is not employed for six weeks, and the government may find the full compensation package for the technician is $8k/ month, they can deduct $12k from the corresponding monthly invoice payments (e.g. $8k one month and $4k the next month). Timing of the new employee starts as soon as the government accepts the qualification package of the employee (assuming the employee is hired and ready to work), or after 72 hours with no response accepting or denying the employee. The 72 hours with no response is not an acceptance.
PRS Description and Details The Performance Requirements associated with each Contract Requirement are as shown in the PRS and include:
A) Work Requirements. A series of subtasks associated with each particular Contract Requirement is listed in the PRS table. Quality of subtasks not on the document will be handled with other means, including, but not limited to, email, KO letter, and Contract Discrepancy Report (CDR).
B) Weight. The value of each Work Requirement is specified as a percentage of the Contract Requirement with which it is associated in column 3 of the PRS. The percentages are based on the costs incurred by the Contractor in carrying out a particular Work Requirement, the visibility of the service, and the detriment to the Government if the Work Requirement is not satisfied. The Weight multiplied by the total order period value for Lump Sum FFP, or the associated delivery order value for each IDIQ will be the primary basis for deducting for partially performed, unsatisfactorily performed, and non-performed work. However, the total monthly deduct is capped at 10% of the order period value for the Lump Sum FFP requirements. The total PRS weight percentages do not total 100% because not all CLIN items will be managed through the formal PRS process.
C) Performance Measure and Performance Standards. Specific measures and standards associated with each Work Requirement are listed in columns 5 and 6.
D) Maximum Allowable Defect Rate (MADR). The MADR for each Work Requirement is identified in column 7 of the PRS. The MADR is the defect rate for a population of services which, when exceeded, indicates that the Contractor's quality control is unsatisfactory. The MADR as a number of defects per period of time. Incidents reflect a one to one relationship with defects (i.e. they are the same).
E) Acceptable Quality Level (AQL). The AQL for a work requirement is identified in column 8 of the PRS. The AQL is the expected standard of performance based on the ratio of acceptable observed work performed to total work performed. When the observed Quality level is lower than the AQL it indicates that the Contractor’s quality control is unsatisfactory. The AQL is expressed as a percentage of the total population per period of time. Incidents reflect a one to one relationship with defects (i.e. they are the same).
In the event that a PM from any contract section is not completed within the timeline scheduled, it may be counted as a quality defect in addition to a timeliness defect. For example, a monthly preventive maintenance task that is completed three days late is not a quality issue but is a timeliness issue. However, a weekly PM task completed a week late is a quality of work issue since no work was completed and the nature of PM work does not allow a weekly task to be completed twice in close proximity of time with added benefit. See the QASP table for maximum lateness of PMs.
Quality work orders and PMs are defined in section 5.1.4 and 5.2.7 respectively and apply throughout the contract to all CLINS.
F) LOT. Column 9 outlines the prescribed population source eligible for surveillance. The Government is not limited to the population source listed; others may be included as required.
The government shall not deduct based on a population sample smaller than listed in Table 1.
Random samples are allowed Random Sampling/Inspections – Random sampling leverages statistical principals to manage the scope of a surveillance activity. This is done so that the amount of surveillance manageable and if properly applied, represents the conditions of the entire performance area with a high degree of confidence. To perform a random sample the performance area has to be broken down into discrete elements like work orders, an area, or specific sections of performance. Then by using a random number generator in MS Excel, each discrete element is assigned a number. The highest or lowest group based on the appropriate sample size then becomes the performance elements that receive surveillance.
1. To determine a statistically significant sample size Table 1 should be consulted (this is also auto-calculated in the PRS table Sample column once the correct lot number is inputted). Random number generation is typically accomplished using a spreadsheet function.
Table 1
Lot Size Sample Size 2-8 2
9-15 3 16-25 5 26-50 8 51-90 13
91-150 20 151-280 32 281-500 50
501-1200 80 1201-3200 125
3201-10,000 200 10,001-35,000 315
35,001-150,000 500 150,001-500,000 800 500,001 and over 1250
G) Method of Surveillance. Column 10 outlines the evaluation tools and techniques utilized to assess
Contractor performance of Work Requirements to established measures and standards. The Government is not limited to the tools and techniques listed; others may be implemented as required and 100% inspection is acceptable. The government may request to be present to witness any specific work, or type of work (e.g. priority 2 work, carpentry work, work on the Base Commander’s house, work order # 1233000, etc.) at no extra cost to the government. In such case, the COR would call and/or email the help desk and/or shop lead to make the request, and the Contractor will be required to make one contact with the method requested by the COR.
The government shall not delay the trade technician’s work (exluding the notification made to the
COR).
H) The following notes apply to the entire PRS.
1. Timeliness for all work. The MADR reflects units of work per period of time (daily, monthly, quarterly, semi-annual, etc.) and allotted response/completion time. Example: If a response time of 4 hours is required, a MADR of 1 incident would allow for one late response during the performance period. For report submissions, each day the report is late is counted as an additional incident. Example: If the report is due the 15th of the month and it is submitted on the 19th, four incidents will be noted. The AQL reflects the ratios of acceptable units of work to the total population of work evaluated. Example: If a response time of 5 days is required, an AQL of 90% would allow for 10% of the evaluated population to be late. In work orders, where a timely response and completion are required, both will be counted as part of the population and an opportunity to be on time.
2. Quality or documentation for all work. The MADR reflects work units that do not meet specified requirements and standards. Example: If the MADR allows 2 incidents, when the 3rd incident occurs deductions will be made. The AQL reflects the percentage of acceptable work required. Example: If the AQL allows a 2% deviation to the standard, when a deviation of greater than 2% occurs deductions will be made.
3. IDIQ. The PRS does not apply to IDIQ work. However, payment shall not be made until the work is complete. Partial payment may be made for partial work.
I) Method of deduction.
1. Deductions for defects in either safety or security, defects that result in notice of violation, audit findings by EPA or ADEC, major audit findings in other contract sections, fines, damage/flooding to facilities, or failure to comply with Government regulations shall be calculated as specified in the Performance Requirement Summary (PRS) examples. These shall be full deductions.
2. All other deductions shall be calculated as specified in the PRS examples and adjusted as shown below.
i) PRS calculation times 25% for the first month a deduction is invoked for a specific work requirement.
ii) PRS calculation times 50% for the second month and subsequent months a deduction is invoked for the same work requirement.
J) Surveillance Period
1. The Contractor shall choose the period of surveillance, in order to offer prompt payment.
To allow time for the Contractor to enter data into the government CMMS, the COR to assemble reports, and conduct due diligence on possible deductions, the government requires 10 calendar days. As such, the surveillance period can start the 21st of the previous month, and run through to the 20th of the payment month. For example, the government may deduct from the April invoice for poor work found between March 21st and April 20th. Subsequently, work evaluated between April 21st and May 20th will apply to the May bill. Or, the Contractor can choose the surveillance period to remain the true month and allow 10 extra days beyond the prompt payment act.
2. Some work will only be able to be evaluated after it is marked complete, even though some of the work was actually done outside the surveillance period. For example, work evaluated for accurate CMMS entries will be evaluated when marked complete.
3. Timeliness shall be measured in months before the current month, if the work order is late. For example, if a work order became due January 10th, and is still not complete in April, this work order shall apply to timeliness calculations in January, February, March, and April.
Note that increasing deduction adjustment is based on a specific work requirement such as Regulated Waste Management and not the three performance measures for this work requirement.
The adjusted deduction shall not be reset each option year. This means, the recurrence penalty listed in I.2 of this document are not reset each option year and apply to the whole of the contract.

Reports Direct Observation
Example 1 Assume the Contractor submits a regulated waste report one (1) day late. Upon review by the Government, three (3) errors are noted in the report. The MADR for Deliverables (Reports) is 2 incidents per reporting period. Since four (4) incidents exceed the MADR, the Contractor will be assessed deductions as follows:
Total Contract Billing for One Year is $11,000,000 (excluding IDIQ).
Total Contract Billing for One Month is $11,000,000/12 Months is $917,000 per month.
Maximum total deduct is $917,000 X 0.20 = $183,400/month Value for General Requirements is $917,000 x 5% (overall PWS section weight) = $45,850.00 Total Deduction for this requirement is as follows:
Price for Requirement x Weight x Percent Deduct for Defect Rate = Total Deduction For This Requirement $45,850 x 45% x 65% = $13,411.12 The proposed deduction modification would be applied to Example 1.
If a first month occurrence, $13,411.12 x 25% = $3,352.78 If a second month occurrence of 4 incidents, $13,411.12 x 50% = $6,705.56 If a third month occurrence of 4 incidents, $13,411.12 x 50% = $6,705.56 The $13,411.12 deduct is within the 10% cap and may be included as a full deduct.
Example 2
Assume the EPA issues a notice of violation for Air Management. The performance standard AQL is 100%. Therefore, the AQL was exceeded. This was the first time of this violation. The Contractor will be assessed deductions as follows:
Total Contract Billing for One Year is $11,000,000 (excluding IDIQ).
Total Contract Billing for One Month is $11,000,000/12 Months is $917,000 per month.
Maximum total deduct is $917,000 X 0.20 = $183,400/month Value for Environmental Management & Stewardship Permit Compliance is $917,000 x 5% (weight) = $45,850.00 Total Deduction for this requirement is as follows:
Price for Requirement x Weight x Percent Deduct for Defect Rate= Total Deduction For This Requirement $45,850.00 x 50% x 100% = $22,925.00 The $22,925.00 deduct is within the 10% cap and will be included as a full deduct.
Example 3
Assume the Contractor completed 200 priority 3,4, or 5 work orders in the month of April, and 50 are in progress and late at the end of the surveillance period. Of the work completed in the surveillance period, 15 work orders were late. This is the Contractor’s second month in the contract that Pri 3,4,5 work has been late.
Total Contract Billing for One Year is $11,000,000 (excluding IDIQ).
Total Contract Billing for One Month is $11,000,000/12 Months is $917,000 per month.
Maximum total deduct is $917,000 X 0.20 = $183,400/month Value for Level II Service work is $917,000 x 10% (weight) = $91,700.00 Total Deduction for this requirement is as follows:
Price for Requirement x Weight x Percent Deduct for Defect Rate = Total Deduction For This Requirement $917,000 x 10% x 100% = $9,170 Since this is the third occurrence, multiply by 50% $9,170*0.50=$4,585 The 4,585 deduct is within the 10% cap and will be included as a full deduct.
Example 4
The examples in 1, 2 and 3 above occurred in the same month and example 1, and 2 were a first occurrences and example 3 was a third occurrence.
The deducts are $13,411.12 + $22,925.00 + $4,585 = $40,921.12 The $40,921.12 deduct is within the 10% cap and will be included as a full deduct.
