About this file
This is Addendum 4 to RFP-KTHFS-2026-002, issued by Klamath Tribal Health & Family Services (KTHFS) for the Beatty Health Center Project, a new 4,064-square-foot healthcare facility in Beatty, Oregon. The addendum modifies the original bidding documents and addresses clarifications regarding glazing specifications (Section 08 80 00) and air duct accessories (Section 23 33 00). Key updates include removal of security film requirements and digital ceramic printing from the glazing specification, clarification that control dampers should use Prolon VVT controls as standard off-the-shelf equipment, and confirmation that data outlet rough-in only is required with no CAT6 cabling or termination work included. The final date for RFP questions was Thursday, October 1, 2026, at 12:00 p.m., with a submission deadline of Wednesday, October 7, 2026. The addendum includes detailed specifications for glazing materials, insulating glass units, laminated glass, fire-rated glazing, accessories, installation methods, and duct accessories including backdraft dampers, volume dampers, fire dampers, control dampers, access doors, and turning vanes, with performance requirements and manufacturer qualifications outlined throughout.
KTHFS is responsible for supplying all furnishings, furniture, moveable equipment, exercise equipment, restroom accessories, kitchen appliances, medical and dental equipment, artwork, and health assessment equipment. The contractor must coordinate construction operations with KTHFS Facilities/Security Director and Health Project Manager, maintain site access for others, prevent utility disruptions, and comply with GANA and IGMA standards for glazing installation and SMACNA standards for HVAC duct construction. Preinstallation meetings are required one week before work begins, and mock-ups of window corners and printed glass designs must be approved before proceeding. Insulating glass units and laminated glass require five-year manufacturer warranties covering seal failures, misting, delamination, and coating defects. Fire dampers must comply with UL 555 standards and NFPA 90A requirements with 165-degree Fahrenheit temperature release devices. Control dampers specified as Prolon PL-AV Series must withstand 5 inches water gauge closed-position pressure and 2,000 feet-per-minute open-position velocity with maximum 3.7 cubic-feet-per-minute-per-square-foot leakage at 1-inch water gauge.

ADDENDUM NO. 004
TO ALL PLAN HOLDERS:
This Addendum forms a part of the Contract Documents and modifies the original bidding documents released for bid preparation. Please notify everyone concerned as your sub-contractors and suppliers of the issuance and contents of this addendum prior to date of bid opening.
THE FINAL DATE RFP QUESTIONS WILL BE ACCEPTED IS THURSDAY, OCTOBER 1, 2026, AT 12:00 P.M.
QUESTIONS AND CLARIFICATIONS:
Q1: Spec 23 33 00 Air Duct Accessories, 2.08, D references spec section 23 09 00 Instrumentation and
Controls for HVAC as regards actuators for control dampers (BD-1. ZD-1, ZD-2, ZD-3). I am not finding that spec section in the book. Can this be provided?
A1: All controls are packaged on this project so spec section 23 09 00 Instrumentation and Controls for HVAC is irrelevant, and the reference has been removed.
Q2: I noticed the altitude is up to 5000’, was this altitude taken into consideration when sizing the generators? Or are you looking for us to upsize and derate our gens based on that elevation and kW provided in the spec?
A2: No need to upsize and derate generators.
Q3: The one-line has the 2000A ATS as NEMA 3 but the spec indicates NEMA 1. Could you please confirm which is needed for the application?
A3: NEMA 3R is required.
Q4: 100KAIC is specifically called out for the 2000A ATS. Is this accurate or is 65kAIC acceptable for the
2000A ATS?
A4: No, ATS must be 100kAIC.
Q5: At both projects there is an RTU which uses a zoning system, but no control protocol or system is identified in the specifications. Please have the design team identify the controls to be used for the systems that incorporate zone dampers.
A5: We would recommend Prolon for any VVT controls, but we don’t have a spec or sequence for this as it is standard off the shelf equipment. This would be by the mechanical subcontractor with whichever equipment they submit.
Q6: The subcontractors for Data are saying they don’t see any specs for Data outlets and the legend calls for rough-in only. Could you clarify if we need to include Cat 6 installation and termination in our proposal?
A6: We are showing rough-in only, no CAT6 cabling or terminations. The cabling and terminations are done by KTHFS or KTHFS vendor for their systems.
Q7: Calling out for security film. The entrance makes sense but the “Admin office areas”, there isn’t anything labeled like that on the floor plan. Can they clearly identify where this goes?
A7: Security film is not included in the project. Please disregard. This line item has been removed from the Glazing spec.
RFP-KTHFS-2026-002 – ADDENDUM #4 – Beatty Health Center Project Page 5
Q8: It says in the ‘gymnasium windows’. Do they mean fitness room? And then this would be all (total of four) exterior windows?
A8: No digital ceramic printing or graphics are included in the project. Please disregard. This line item has been removed from the Glazing spec
