About this file
Summary
This is Addendum #3 to RFP-KTHFS-2026-002 issued by Klamath Tribal Health & Family Services for the Beatty Health Center Project in Beatty, Oregon. The addendum modifies the original bidding documents for construction of a new 4,064-square-foot single-story health center facility comprising a community room, workout room, medical office, dental office, behavioral health office, lab room, equipment sterilization room, restrooms, showers, and breakroom. The final date for RFP questions is Thursday, October 1, 2026, at 12:00 p.m., with a submission deadline of Wednesday, October 7, 2026. The addendum includes revised mechanical schedules specifying Daikin equipment instead of Trane, updates to roof power plans and electrical riser diagrams to coordinate with the new mechanical specifications, and clarifications regarding appliances, door hardware specifications, glazing types, electrical cable usage, and window film scope exclusions. Key mechanical equipment includes three rooftop units (RTU-1, RTU-2, RTU-3) with capacity ranges from 800 to 1,600 CFM, motorized damper controls, and an exhaust fan with demand-controlled ventilation for various occupancy spaces.
The addendum specifies that KTHFS will supply all furnishings, furniture, moveable equipment, exercise equipment, restroom accessories, kitchen appliances, medical and dental equipment, and artwork, with the contractor responsible for installation. Contractor-furnished and installed items include only a GE 1/2 HP continuous-feed waste disposer (Model GFC525N). KTHFS-furnished and contractor-installed appliances include a GE Profile Series refrigerator, microwave oven, and dishwasher, each with specific warranty requirements ranging from five to ten years. The specifications address HVAC system requirements including compliance with ASHRAE 90.1-2022 energy standards, thermostatic zone controls with minimum 5-degree temperature deadband, optimum start controls with automatic setback to 55°F heating and 85°F cooling, and complete testing and balancing services. Electrical specifications clarify that both MC and Romex cables are permitted for branch circuits, with Romex restricted to concealed wall installation only and prohibited above suspended ceilings. All exposed ductwork must be galvanized spiral construction, and the project requires furnished operating and maintenance manuals and owner training.

QUESTIONS AND CLARIFICATIONS:
Q1: Project Certification – Is this project itself required to be registered and issued an AWI QCP Certificate of Compliance, or is the requirement solely for the millwork manufacturer / installer to hold current AWI QCP Certification?
A1: It is not required for the project to be registered.
Q2: Inspection Scope – Please confirm if AWI QCP installation inspection services are required to be contracted for this specific scope or its self-certification / compliance documentation by an AWI QCP certified shop is sufficient.
A2: No inspection is required.
Q3: Door 100A has hardware group 14 per door schedule. Please review and advise if this needs changed to group 01.
A3: Door 100A will have hardware group 01.
Q4: Door 100B has hardware group 14 per door schedule. Please review and advise if this needs changed to group 02.
A4: Door 100B will have hardware group 02.
Q5: Door 102A has hardware group 14 per door schedule. Please review and advise if group is correct or needs changed for panic type hardware.
A5: Door 102A will have hardware group 16.
Q6: Following doors have glass in them and don’t have a glazing type – 103A, 104A, 113, 116, 118, 119,
120. Please confirm T1 tempered glass.
A6: Correct, all doors with glazing will have T1 tempered glass.

Please confirm door type.
A7: Door 122A is intended to have a half glass type door.
Q8: Hardware group #08, calls for a Privacy Lockset with Occupancy Indicator but has an entrance/office lockset spec’d. Please review and advise privacy function required.
A8: Hardware group #8 is intended to be a Privacy Lockset for the restrooms/shower room.
Q9: Can you please clarify if MC Cable and Romex cables are permitted for branch circuits? Section 26 05 19 2.01K seems to allow MC, but Section 2.05 says only with written approval. Romex is not mentioned in those sections, but 26 05 29 3.01E seems to allow the use of Romex.
A9: Both MC and Romex cables are good to use. Romex cables can only be used if they are concealed within the walls; Romex cables cannot be put above any dropped or suspended ceilings.
RFP-KTHFS-2026-002 – ADDENDUM #3 – Beatty Health Center Project Page 5
