About this file
This Addendum #2 document is issued by Klamath Tribal Health & Family Services (KTHFS) and modifies the original bidding documents for RFP-KTHFS-2026-002, the Beatty Health Center Project, a new 4,064-square-foot health center facility in Beatty, Oregon. The addendum clarifies specifications and updates contract documents through the second revision, containing modifications to Section 01 10 00 (Summary) and Section 10 28 00 (Toilet, Bath and Custodial Accessories). The final date for RFP questions is Thursday, October 1, 2026, at 12:00 p.m., with a submission deadline of Wednesday, October 7, 2026. Key clarifications include confirmation that only two wheel stops are required per civil plans. The addendum provides updated architectural drawings, specifically E101 (Level 1 Lighting Plan Sheet) adding an exterior exit sign and wall pack at the exterior door, and E201 (Level 1 Power Plan Sheet) adding a receptacle at the exterior door.
The Toilet, Bath, and Custodial Accessories specification section details contractor-provided and contractor-installed items including grab bars, mirrors, towel bars, shower curtain bars, baby changers, hooks, recessed wastebaskets, and waste receptacles, with basis-of-design products from Bobrick and other manufacturers including ASI (American Specialties, Inc.) and Bradley Corporation. KTHFS will provide soap dispensers, towel dispensers, toilet paper dispensers, toilet seat cover dispensers, and feminine hygiene dispensers for the contractor to install with backing and support framing. Installation requirements mandate compliance with 2010 ADA Standards for Accessible Design, ASTM material specifications for stainless steel tubing and sheet, galvanized steel, tempered glass, and electrodeposited coatings. All accessories must be shop-assembled, free of defects, and delivered complete with anchors, fittings, and fasteners; the contractor must provide three keys for each master key lockable accessory to the owner and ensure proper coordination with internal wall reinforcement and toilet partition anchoring.

POWER GENERAL NOTES:
A. UNLESS OTHERWISE NOTED, 120V, 20A/1P BREAKERS SHALL UTILIZE #12
AWG CONDUCTORS. EXCEPTION: WHERE BRANCH CIRCUIT IS IN EXCESS
OF 125 LINEAR FEET, CONDUCTORS SHALL BE #10 AWG. EQUIPMENT
GROUNDING CONDUCTOR SHALL MATCH PHASE CONDUCTOR FOR
20A/1P CIRCUITS.
B. BRANCH CIRCUIT CONDUCTORS SIZED TO PREVENT VOLTAGE DROP
MAY BE REDUCED IN SIZE AS LOAD SUPPLIED BY REMAINING CIRCUIT
DECREASES. VOLTAGE DROP SHALL NOT EXCEED THREE PERCENT AT
THE FARTHEST BRANCH CIRCUITS.
C. MULTIWIRE BRANCH CIRCUITS SHALL BE INSTALLED PER ARTICLE 210.4
OF THE NEC. EACH MULTIWIRE CIRCUIT SHALL BE PROVIDED WITH A
MEANS THAT WILL SIMULTANEOUSLY DISCONNECT ALL UNGROUNDED
CONDUCTORS AT THE POINT WHERE THE BRANCH CIRCUIT ORIGINATES.
I.E. BREAKER HANDLE-TIE.
D. MAINTAIN A MINIMUM SEPARATION OF 12" BETWEEN CENTERLINE OF ALL
RECEPTACLES SET INTO OPPOSITE SIDES OF ALL SOUND ISOLATION
WALLS. WHERE ABOVE NOTED CONDITION OCCURS AND REQUIRES A
CONDUIT CONNECTION BETWEEN BOXES, UTILIZE FLEXIBLE TYPE
CONDUIT.
E. DEVICES INDICATED ON THIS DRAWINGS ARE DIAGRAMMATICAL ONLY.
CONTRACTOR SHALL REFER TO ARCHITECTURAL ELEVATIONS FOR
LOCATIONS OF OUTLETS, DEVICES, ETC. PRIOR TO ROUGH-IN.
