Addendum 1 Summary – RFP 26/27-04 Random Moment Sampling
This is Addendum 1 to Request for Proposal 26/27-04 issued by the Florida Department of Revenue for Random Moment Sampling services. Addendum 1, dated September 10, 2026, modifies the original RFP based on vendor questions received during Round 1 and must be acknowledged and submitted with all proposal responses. The addendum makes four substantive changes to the original specifications: reduces total cost proposal points from 250 to 75 in Section 3.16.C and clarifies total proposal scoring in Section 3.17; deletes language requiring Contractor Pollution Liability coverage with limits of $5,000,000 each occurrence and aggregate from Section 2.12; provides clarification regarding submission requirements for Attachment I (Proposer Reference Form), Attachment J (Customer Reference Form), and the required number of references; and deletes the final sentence of Section 2.4, subsection 5 regarding notification of system outages. The addendum includes replacement pages for Sections 16, 19, 32, 33, 43, 45, and 55 of the original RFP document.
Key contract requirements reiterated in the addendum include technical support obligations requiring 24-hour resolution of high-priority system issues and 72-hour resolution of non-high-priority issues, 99 percent system availability during regular business hours (7:00 a.m. to 7:00 p.m. Eastern Time), confidentiality and data safeguarding requirements, and system performance monitoring. The contractor must maintain commercial general liability coverage of $1,000,000 per occurrence and $5,000,000 aggregate, comprehensive automobile liability coverage of $1,000,000 combined single limit, and workers' compensation insurance. The maximum technical proposal points possible is 175, with a revised maximum cost proposal of 75 points, resulting in a maximum total proposal score of 250 points. Failure to achieve successful cutover on schedule incurs financial penalties of $1,000 per day of delay, with the Department retaining the right to unilaterally terminate the contract for poor performance or failure to deliver a fully functioning product within 30 days after scheduled cutover.
