About this file
This is a Past Performance Reference Questionnaire attachment to solicitation FA4626-26-R-0009 for the repair and renovation of Dormitory 742 at Great Falls Air Force Base, issued by the Department of the Air Force Global Strike Command. Offerors must submit between three and five recent and relevant past performance references with proposals due by 18 May 2026 at 2:00 p.m. Mountain Daylight Time. "Recent" references must be projects completed within the five years preceding April 2021, and "relevant" references must be similar in scope, size, and complexity to the solicitation requirements with contract values exceeding $5,000,000.
The questionnaire requires offerors to document specific project details including contract numbers, performance dates, dollar values, customer information with points of contact, competitive award status, contractor role (prime, subcontractor, or key personnel), descriptions of work performed, and identification of whether projects involved renovation or repair of dormitories, hotels, or large office facilities with partial demolition and reconstruction. Offerors must also demonstrate their proposed Project Manager's direct involvement and lead role on comparable projects. The evaluation criteria emphasize experience with coordination and integration of multiple building systems (HVAC, electrical, fire protection, plumbing, architectural, civil, mechanical, and structural), as well as management of project schedules, subcontractors, and risk. The questionnaire includes 24 performance rating questions using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory) to be completed by the customer reference, addressing contractor management capabilities, personnel qualifications, quality control, schedule adherence, problem resolution, subcontractor payment practices, and overall performance assessment.

FA4626-26-R-0009
When completed, this document is Source Selection Information IAW RFO 2.101 and 3.104. Past Performance Reference
Past Performance Reference Sheet In accordance with (IAW) paragraph 4 of Section L of this solicitation, please provide between three (3) and five (5) recent and relevant past performance references. As a reminder, “recent” is defined as “completed projects within the last five (5) years from April 2021”, and “relevant” is defined as the degree to which past performance projects are similar in scope, size, and complexity to the requirements of this solicitation and valued at more than $5,000,000.00.” Include rationale supporting your assertion of relevance. In assessing relevancy, the government will consider, at a minimum:
· Experience with renovation or repair of dormitory, hotel, or lager office-type facilities including partial demolition and reconstruction.
· Coordination and integration of multiple building systems (e.g., HVAC, electrical, fire protection, plumbing, architectural, civil, mechanical, and structural).
· Management of project schedule, subcontractors, and overall project risk.
· Relevancy will also consider the extent to which the referenced projects demonstrate the experience of the proposed Project Manager in performing similar work, particularly where the individual served in a lead role on projects of comparable size, scope, and complexity. Greater consideration may be given to references that the proposed Project Manager was directly responsible for overall project execution and outcomes on projects of similar scope, size, and complexity. Offerors shall include rationale supporting their assertion of relevancy.
One of the considerations in proposal evaluation is the verification of the offerors past performance on contracts or other work efforts, which reflect the offeror's ability to perform on the proposed effort.
Past performance references must be provided within Volume I – Past Performance of your company’s proposal, which is to be submitted and received no later than 18 May 2026 at 2:00 p.m. Mountain Daylight Time (MDT).
