About this file

This is a Subcontractor/Teaming Partner Consent Form for Department of the Air Force Solicitation FA4626-26-R-0009, concerning the Repair Dorm 742 project at Malmstrom Air Force Base, Montana. The form is a template that requires editing by the prime contractor before distribution to major subcontractors and teaming partners identified in the proposal response.

The form authorizes the Government to disclose past and present performance information regarding subcontractors and teaming partners to the prime contractor during the source selection process. Since prime contractors are considered private parties, Federal regulations prohibit disclosure of subcontractor/teaming partner performance data without written consent. Completed consent forms must be submitted as part of the Past Performance Volume I of the proposal. The form requires signature and title of an individual with legal authority to bind the company, along with the company name and address. The form is addressed to Ms. Colleen Goad and Ms. Jillian Urick and explains that the Government emphasizes past performance in achieving best value in source selections.

SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE

RELEASE OF PAST AND PRESENT PERFORMANCE INFORMATION TO

THE PRIME CONTRACTOR

Past performance information, concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent.

Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the Government's

Evaluation Team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Past Performance Volume I. The following consent form requires editing by the prime contractor prior to forwarding to their respective subcontractors (see bolded text).

Attachment 11 FA4626-26-R-0009
