About this file
This file contains a U.S. Air Force Transient Contractor HAZMAT Usage Worksheet and a detailed Hazardous Materials (HAZMAT) Tracking Process flowchart for a construction contract at Patrick Space Force Base (PSFB). The documents outline a comprehensive process for contractors to propose, get approval for, and track hazardous materials usage during a construction project, involving multiple stakeholders including the Contracting Officer, Installation Hazardous Materials Program (IHMP) Manager, and the Hazardous Material Management Process team.
The tracking process requires contractors to submit a proposed list of hazardous materials to the Contracting Officer, who then forwards it to the IHMP Manager for review. The contractor must prepare Chemical/Hazardous Material Request/Authorization Forms with current Safety Data Sheets (SDS) for each material. The IHMP Manager will input data into the Enterprise Environmental Safety and Occupational Health Management Information System (EESOH-MIS) and obtain authorization. Throughout the contract, the contractor must provide quarterly purchase reports of trackable hazardous materials, with the IHMP Manager tracking and inputting usage data. At contract completion, a final report is submitted to close out the contract in the EESOH-MIS system.
