About this file

This is a Time Compliant Technical Order (TCTO) issued by the U.S. Coast Guard Shore Infrastructure Logistics Center (SILC) for electrical equipment arc flash safety at USCG shore facilities. The order was authorized on May 13, 2019, and remains valid through December 30, 2019. The technical order applies to electrical equipment rated at or above 480 volts nominal or 1200 amperes at USCG shore installations and addresses organizational-level maintenance actions to ensure worker safety when accessing high-voltage and high-amperage electrical equipment.

The TCTO mandates two primary actions: (1) field units must affix general arc flash warning labels to all applicable electrical equipment within 60 days of release, with labels meeting ANSI Z535.4 standards and rated for outdoor use with minimum 48-month durability; and (2) the Mission Support Product Line must update preventative maintenance job plans for all equipment rated over 480V nominal or 1200A within seven months. The order specifies that general arc flash warning labels are preferred over detailed labels due to evolving analysis methods and the need for continuous maintenance validation. All ENG-13 Facility Engineers and Sectors with Material Maintenance Warrants must complete Red Vector arc flash training course RV-10747 within three months and provide on-the-job training to electricians. Field units must report completion through the Shore Asset Management (SAM) system using the Work Order Tracking module, with detailed instructions provided in Appendices A and B. Equipment inventory for identification is available through a referenced Excel file organized by geographic unit, containing approximately 3,715 pieces of equipment at 480V nominal and above, plus 293 pieces at 1200A and above.

The SILC MSPL shall track the completion of updates to SAM PM job plans for shore electrical equipment and report completion in the annual MSPL Asset Management Report.

2 TCTO DETAILS

This TCTO addresses the requirement to place arc flash warning labels on electrical equipment in accordance with the Occupational Safety and Health Administration (OSHA) and industry code-consensus standards. This TCTO also directs personnel to include instructions on how to de-energize equipment and install temporary grounding to assure worker safety in PM job plans for higher voltage and higher amperage equipment, and to perform all electrical preventive maintenance with insulated tools for safety. Field actions regarding workforce training on arc flash safety are also included in this section.

The SILC MSPL and SILC-ESD-FMB will review and update SAM job plans to verify that appropriate guidance is included regarding equipment-specific maintenance actions in accordance with manufacturer recommendations and industry standard practices. The following sections describe each of these actions.

2.1 Field Actions – Arc Flash Warning Labels

2.1.1 Introduction/Background

The National Electric Code Handbook, also known as the National Fire Protection Association (NFPA) National Electric Code 70 Article 110.16 (Arc Flash Hazard Warning), states that:

“Electrical equipment that is in other than dwelling units, and is likely to require examination, adjusting, servicing, or maintenance while energized, shall be field or factory marked to warn qualified persons of potential electric arc flash hazards. The marking shall be located so as to be clearly visible to qualified persons before examination, adjustment, servicing, or maintenance of the equipment.”

Proper warning labels raise the level of awareness of electrical arc flash hazards and decrease the number of accidents that result when electricians do not wear the proper type of protective clothing while working on energized equipment. The following sections describe a general arc flash warning label (required for USCG electrical equipment) and a detailed arc flash warning label (not required for USCG electrical equipment).

2.1.2 General Arc Flash Warning Labels

The requirement of 110.16(A) mandates a general arc flash hazard warning label, nothing more.

In alignment with Department of Defense and industry standard practices, a general arc flash warning label, as shown in Figure 2-1, meets all marking requirements for USCG arc flash warning on shore electrical equipment.

Figure 2-1: Typical General Arc Flash Warning Label

General arc flash labels are usually preferred over detailed labels at USCG shore units for several reasons, including the following:

a. Arc flash analysis methods are still evolving, and changes to the arc flash equations are expected. If the equations change, the arc flash calculation results will likely change, requiring detailed labels to be relabeled.

b. Confirmation of periodic maintenance and testing is a crucial input to an arc flash evaluation. With the commissioning process for new equipment properly completed, routine and periodic maintenance and testing is still required to continue the validation of the detailed arc flash warning label. A detailed arc flash warning label that was produced for a properly maintained system will be invalid in the future if routine and periodic maintenance and testing are not continually performed.

c. A detailed label can include unstated engineering assumptions regarding the energized work practice and working distance.

d. A detailed arc flash warning label will not recognize or acknowledge system configuration changes that affect the analysis results. For example, changing a circuit breaker trip setting can change the arc flash results from Category 0 to Category 4 or higher.

Considering the reasons stated above, a detailed label would have to be periodically updated, which is considered a significant resource burden. The responsible electrical personnel in charge of the energized work maintenance/action must review any available arc flash study to determine the energized work requirements. Additional technical bases in support of a general label are provided in the following electrical technical paper: Tri Services Working Group TP-15 Arc Flash Calculations and Detailed Arc Flash Warning Labels.

2.1.3 Detailed Arc Flash Labels

While not prohibited by USCG policy, it is not anticipated that many shore units will employ detailed arc flash warning labels for the reasons discussed in Section 2.1.2.

Large facilities with significant electrical infrastructure and appropriate resources may consider using detailed arc flash labels at their facility. Detailed labels are only authorized if the unit has an arc flash risk assessment program that includes documented periodic maintenance and testing. Detailed labels for new installations must also have been commissioned in accordance with the International Electrical Testing Association (NETA) Acceptance Test Specifications.

Item Number: J5514 (Danger Arc Flash and Hazard Label, 5” X 3.5”)

Arc flash labels shall meet the following minimum quality requirements:

a. Adhesive-backed polyester label designed to comply with the American National Standards Institute (ANSI) Z535.4 2002 & 2007 Product Safety Label Standards. Labels shall be screen-printed with UV-resistant inks and clear weatherable over laminated with clear polyester to ensure a high degree of chemical, abrasion, and heat resistance.

b. Rated for outdoor application and guaranteed by the manufacturer to retain original performance and properties for a minimum of 48 months from date of placement.

c. Comply with the requirements for arc flash labels as stated in NFPA 70E.

2.1.5 Placement of Warning Labels

The MSPL has reviewed the guidance within ANSI Z535.4-2011 (Product Safety Signs and Labels) for application to electrical equipment and developed the following guidance for field personnel when placing warning labels.

Warning labels shall be affixed to equipment in the center of the access panel so they are clearly visible to qualified persons before examination, adjustment, servicing, or maintenance of the equipment. Clean the equipment surface with either soap and water or another appropriate cleaning solution. After the surface is clean and dry, affix the label to the equipment using a hand level to assure the warning label is affixed horizontally and vertically true. Apply hand pressure to the label to ensure proper adhesion.

Remove any non-standard arc flash labels that existed prior to placement of the new arc flash warning label. Figure 2-3 demonstrates typical warning label placement.

Figure 2-3: Typical Warning Label Placement

2.1.6 Reporting Completion of Tasking

Field units shall record completion of the requirements identified below by entering data in SAM following the instructions in Appendix A.

2.1.7 Workforce Training

ENG-13 Facility Engineers and Sectors with Material Maintenance (MAT) Warrants shall acquire Red Vector user licenses from the Office of CG-43 and complete Arc Flash training course number RV-10747 within three months of the release of this TCTO. Upon completion of training, Facility Engineers shall provide On-The-Job training to all electricians at Units within their Regional FE geographic areas of responsibilities. On-The-Job training will include viewing of the Air Force Institute of Technology (AFIT) Arc Flash Video posted on YouTube:

https://www.youtube.com/watch?feature=youtu.be&v=eoTz_-MiHrA&app=desktop. The video is accessible on CG standard workstations, but contact the MSPL POC if bandwidth poses an issue. FEs will report via email to SILC-ESD-FMB when completed. POC is Mr. Jeff Crewe, Jeffrey.T.Crewe@uscg.mil, with subject line “Arch Flash Training Completion.”

Aligning personnel training with field practices regarding arc flash and shock hazard warning is necessary for compliance with the intent of OSHA Subpart S. Accordingly, SILC-ESD-FMB, MSPL, the HSWL Service Center Office of Shore Safety, and Commandant CG-113 (Office of Safety and Environmental Health) are developing a Coast Guard required training course for military and wage grade electricians assigned to shore units. This future training course will be administered and tracked using the Training Management Tool (TMT) on CG-Portal. Topics will include defining what is considered live work, how to avoid performing live work, how to assess the level of arc flash risk, and how to properly select personal protective equipment if performing live work is necessary. Additionally, SILC MSPL is working with the force rating master chief at Electrician's Mate (EM) “A” School to develop course material for shore-side electrical safety including both arc flash and shock hazards. This updated training solution shall be completed and published in TMT within twelve months of the published date of this TCTO.

Contractors accomplish some PM/corrective maintenance on USCG shore electrical equipment, and contract language must provide awareness of safety issues that exist due to past PM practices. A common example is when a fault protection circuit has not been tested or evaluated for many years. In this scenario, contract language must inform workers that the fault protection circuits cannot be relied upon to perform as initially designed. MSPL is developing and publishing specific template contract language on the SILC CG Portal.

2.2 Product Line Actions – Equipment-Specific Preventative

Maintenance Job Plans

2.2.1 Introduction/Background

Existing job plans for electrical equipment are based on RSMeansTM PM standards and, in many cases, do not direct electrical equipment maintainers to de-energize equipment, install temporary grounds, and perform lock out/tag out steps prior to making contact with equipment.

This is inconsistent with industry standard practices and places workers at risk of injury.

MSPL and SILC-ESD-FMB will review job plans to ensure appropriate guidance is included regarding equipment-specific maintenance actions, in accordance with manufacturer recommendations and industry standard practices. While RSMeans standard job plans have an acceptable initial level of detail for populating a PM program, more specificity is needed regarding electrical equipment. Improving worker safety and achieving better alignment with both industry standards and manufacturer recommendations are the main drivers for this more robust approach.

The prevalent code consensus standard for the PM of electrical equipment is the NETA Maintenance Testing Specifications for Electric Power Distribution Equipment and Systems.

MSPL will research NETA and determine which PM tasks are required for each type of equipment on a one-, two-, and three-year cycle. Some PM steps are typically only performed when equipment is first placed into service, with other actions only being performed every two to three years or longer.

2.2.2 Applicable Equipment

The following is an overview of equipment operating at 480V nominal (includes equipment operating at 440V-480V) and above:

The following is a top-level summary of all equipment operating at 1200A and above:

2.2.3 Creation of Updated Job Plans and Review/Approval

The SILC MSPL will review existing job plans and verify that appropriate safety measures are included in each job plan for electrical equipment. The Health Safety & Work-Life (HSWL) Office of Shore Safety will be consulted to confirm that safety precautions align with standard practices and SMART visit guidance.

MSPL will utilize and reference within Job Plans the guidance found in reference (f), Unified Facilities Criteria (UFC 3-560-01, latest) Operation and Maintenance: Electrical Safety. This approach adopts DOD policy for the levels of Arc Flash protection and boundaries, in alignment with OSHA and code consensus standards. Refer to https://www.wbdg.org/ffc/dod/unified-facilities-criteria-ufc for the most current UFC.

MSPL will also analyze a new PM job plan for thermographic imaging of certain equipment items based on size/capability, mission criticality, and unit staffing. Initially, annual

A.2 How to Complete SAM Work Orders for this TCTO

As work progresses on the work order, you can report the actual labor hours, materials, and tools used. Enter or update scheduling fields on the Work Order and Actuals tabs. Scheduling fields include Technician, Start Date, Hours to Complete, Materials Used, and Cost.

Under the Log field, list each electrical equipment item that has been completed (an arc flash warning label has been attached in accordance with this technical order). The work order may only be marked as completed when ALL applicable electrical equipment for the unit has been affixed with an arc flash warning label in accordance with the requirements of this technical order.

Track the actual labor hours that were spent to complete the work by going to the Actuals tab in Work Order Tracking and selecting the Labor sub-tab.

Actuals tab

a. You can specify and change actual hours for labor only on an approved work order, copy data from an existing plan and modify the data to reflect actual events, and specify labor and crafts until the work order is closed.

b. Report actual labor hours in the following ways:

1. Report hours for multiple types of labor at the same time.

2. Copy labor from the work plan.

3. Report labor individually.

4. Select planned labor.

A-4

Labor sub-tab

c. Use the Materials sub-tab under the Actuals tab to track the actual quantities, locations, and costs for items used on an approved work order. You can copy data from an existing plan, modify the data to reflect actual events, or insert new data. You can insert new rows until you close the record, but you can only edit information before the record is saved. That is, once the record has been saved, you can add additional information, but you cannot change information already entered.

Materials sub-tab

d. To add individual materials:

1. Go to the Work Order Tracking module and click on the Actuals tab.

2. Go to the Materials tab at the bottom of the screen.

3. Click New Row.

4. Change Line Type from Item to Material.

5. The description field will become editable; enter a Description of the materials used.

6. Enter a Quantity and/or Unit Cost.

7. Select New Row to enter additional materials.

e. You can also report actual material use in the following ways:

1. Use Select Materials to select multiple items at once.

2. Use Select Reserved Items to select items reserved in the inventory, based on the work plan.

3. Use Select Equipment Spare Parts to select spare parts for the equipment.

When you issue an item to a work order and it is not currently a spare part of the equipment on the work order, the system can automatically add it to the spare parts list of the equipment. The Add as Spare Part check box must be selected for the record in the Item Master application for the spare part list.

f. To track the actual quantities, hours, and rates for tools used on an approved work order, you use the Tools sub-tab under the Actuals tab in the Work Order Tracking

A-5 application. You can copy data from an existing plan and delete, add, or change the data to reflect actual events. Work orders must be approved before you can report actuals. You can insert new rows until you close the record, but you can only edit information before the record is saved.

Select tools

g. You can report actual tool use in the following ways:

1. Select Tools from the database.

2. Select Planned Tools to copy tool data from the work plan.

3. Select Issued Tools that were issued to the work order.

4. Use the New Row button to report individual tool use.

h. To view total costs for the work order, go to the Select Action dropdown list and select View > Costs.

A.3 Closing a Work Order for this TCTO

Perform any quality assurance (QA) checks after marking the work order complete and before marking it closed. If QA is not performed, the work order may be marked “closed” immediately after it is marked “completed.” Closing out a work order automatically archives the work order; at that point, edits are no longer possible.

a. Select the work order.

b. Click the Close icon or the Change Status icon and select Closed in the New Status dropdown, then click OK. The work order becomes a history work order and cannot be edited or changed.

c. Click Save.
