1. Activities Summary Major contract activities that have occurred during the reporting period:
• Deficiency Reports (DRs) o Five new DRs were generated this reporting period.
o Aero Simulation, Inc. (ASI) has 19 DRs prepared for Joint Final
Inspection (JFI).
o ASI continued efforts on various other DRs.
o Additional information on specific DRs can be found in Appendix A.
• Engineering Change Proposal (ECP) o Four new ECPs were approved this reporting period.
o ASI has 18 ECPs prepared for JFI.
o ASI continued efforts on various other ECPs.
o Additional information on specific ECPs can be found in Appendix
B.
o Highlights and status of major ECPs are summarized below:
016-16, MH-60T Engine High Side Failure with loss of Np and Torque indications. ASI received notice that there has recently been a rash of interesting Engine High Side Failures in the fleet and one of the recommended actions from a mishap report was to simulate the High Side Failure in the simulator a little differently. The United States Coast Guard (USCG) provided data and an ECP was generated and approved. ASI quickly implemented the changes for evaluation by USCG Subject Matter Experts (SMEs). The changes were found to provide the required effects, and are in place on the Operational Flight Trainer (OFT) and Cockpit Procedure Trainer (CPT) as an engineering level change.
Formal Contractor Final Inspection (CFI) and JFI will still be conducted.
012-15, MH-65Dx Automatic Flight Control System (AFCS), Flight Director (FD) & Control Loader System (CLS) Integration and 001-16, MH-65D1 AFCS/FD System Problem Report (SPR) Clearance. The software efforts under these ECPs have been completed and accepted by the USCG. ASI received the final documentation from the Control Loading Original Equipment Manufacturer (OEM).
ASI is awaiting final documentation updates from the AFCS/FD OEM. ASI was informed there is additional documentation, however the AFCS/FD OEM may have data rights issues to resolve prior to delivery.
007-15, MH-60T CPT Telestra Upgrade. Site engineers have implemented the host side changes associated with the “test to speech” Automatic Terminal Information Service (ATIS) functionality on the OFT, in a development environment. ASI is in the process of documenting the hardware changes made by this upgrade. ASI began and completed the integration and CFI phases on this effort on the CPT. Site engineers will conduct JFI on the CPT and OFT with the USCG when CFI is completed on the OFT.
003-15, HC-144B OFT Forward Looking Infrared (FLIR) Universal Hand Control Unit (UHCU). The UHCU is installed in the OFT and is functional. Additional (minor) hardware is required to permanently install the cabling for the UHCU.
ASI has received all required hardware and will attempt to install the items next reporting period. Additionally, ASI is in the process of documenting the hardware changes made by this upgrade.
026-14, Update the MH-60T CPT and OFT Embedded Global Positioning (EGI), Inertial Navigation System (INS) Model. Based upon research and SME input, ASI conducted tuning of malfunction effects related to Receiver Autonomous Integrity Monitoring (RAIM) and Actual Navigation Performance (ANP) calculation and validities. As part of CFI phase, ASI noted issues with performing Stored Heading alignments. ASI engineers are currently working on addressing these issues.
014-14, HC-144A CPT Vibrations. Changes have been incorporated into the CPT Block Configuration Upgrade (BCU) software and hardware. ASI is in the process of documenting the hardware changes made by this upgrade.
ASI will attempt to schedule formal testing of this ECP once the documentation is complete.
013-14, HC-144B OFT BCU and 012-14, HC-144B RFTD
BCU.
• The Integrated Processing Computer (IPC) has been installed and in use since early November 2016 with no operational issues noted.
• Currently, three DRs remain open. The three open DRs are written up against the Enhanced Ground Proximity Warning System (EGPWS), and the same issues are present on the aircraft. These items were written up as DRs for tracking purposes and cannot be addressed until the issues are corrected on the aircraft. As of the end of this reporting period, these same discrepancies are still present on the aircraft.
• A list of open HC-144B OFT BCU DRs is included in Appendix A for tracking purposes.
• Engineering Change Notice (ECN) o No new ECNs were generated this reporting period.
o Additional information on specific ECNs can be found in Appendix
C.
• The MH-65D2 motion system has been dropping offline occasionally, due to a PLC communication timeout ever since it was installed. The motion system OEM suggested a complete motion computer replacement. The replacement computer was received and installed in December. The motion system operated without issue. However, in the last reporting period, the motion system experienced an uncommanded movement issue. This uncommanded movement was a serious safety issue. ASI troubleshot the problem and determined it to be a bad card in the new motion PC. ASI removed the same type of card from the old motion PC and installed it in the new PC. The new motion PC was placed back in the system. The motion system is fully operational and no PLC comm timeouts have been experienced. The bad card has been sent to the OEM for warrantee repair. ASI is working with the OEM to determine the course of action to take with the failed original motion computer.
• ASI is monitoring the MH-65D1 closely due to recent issues with the Input/Output (IO) system. The issue is that communication appears to be lost between the Host computer and IO system. Further research found that the driver installed on the system to communicate with the IO system has been superseded many times. ASI updated the IO system drivers on the host computer. No further issues with the IO system have occurred since the driver was updated.
• ASI continues to monitor the MH-65D1 and MH-65D2 due to what is described by instructors as an “aerodynamic lockup”. This problem requires a host reset to clear. Presently, ASI is unsure if this is related to, or the same as an issue where the airspeed seems to bleed off for no apparent reason. The scope of monitoring these devices has increased to include an AFCS/FD/Aero issue that manifests itself as bumps felt, primarily during IAS climb outs, but it has also been noted during straight and level flight on HDG/ALT mode. ASI will continue to collect data and pass along to the AFCS/FD/MH-65E OFT OEMs for evaluation.
• ASI engineers and technicians continued to support the USCG with efforts of other contractors.
o Support and assistance was provided for continuing modification efforts on the Aircrew Weapons Trainer (AWT) device.
o Support and assistance was provided for continuing modification efforts on the MH-65D1 device. This included utilization of CLIN 4006 Premium Time Support for support of weekend efforts.
• ASI has continued to purge the obsolete records, parts, and equipment from the Flight Training System (FTS) facility. The obsolete parts will be removed from the annual inventory utilization report.

#1 Generator Failure and #2 Generator Failure
Note: this ECP applies to the MH-60T OFT & the MH-60T CPT.
Current functionality: During main generator failure, #1 or #2 GEN caution is displayed.
Desired functionality: Add a second #1 Generator Failure (SLOW SWITCH) and include the following symptoms: #1 GEN Caution, SAS 1 and AUTOPILOT disengaged, AFCS Degraded, SAS 2 Fail Advisory, Stabilator Caution. Also include momentary loss of #1 MFD, DCS2, V1, ICU, and ADC1. The momentary loss should appear as these items losing power for 2-3 seconds, but then coming back on line as the #2 generator picks up the #1 AC Primary and AC Essential busses.
Add a second #2 Generator Failure (SLOW SWITCH) and include the following symptoms: #2 GEN Caution. Also include momentary loss of #2 CONV, and #2 and #5 MFDs. Again, this should be a 2-3 second power loss, with the equipment coming back on line as the #1 Generator picks up the #2 AC Primary Bus and #2 DC Primary Bus.

MH-60T CPT Image Generator and Host/IOS Computer Systems Replacement
Desired functionality: To have the same "gaming" areas and IOS visual/environmental cues control on both the CPT and
OFT.
Reason for the change: To allow the use of identical training scenarios on both the CPT and OFT. To be able to utilize the CPT as a fully functional backup device, with respect to training scenarios, for the OFT. This would also allow us to run virtually the same simulation code on the CPT that the OFT utilizes.
