1. Activities Summary Major contract activities that have occurred during the reporting period:

• Deficiency Reports (DRs) o Three new DRs were generated this reporting period.

o Aero Simulation, Inc. (ASI) has nine DRs prepared for Joint Final

Inspection (JFI).

o ASI continued efforts on various other DRs.

o Additional information on specific DRs can be found in Appendix A.

• Engineering Change Proposal (ECP) o No new ECPs were approved this reporting period.

o ASI has 12 ECPs prepared for JFI.

o ASI continued efforts on various other ECPs.

o Additional information on specific ECPs can be found in Appendix

B.

o Highlights and status of major ECPs are summarized below:

018-16, Coast Guard Aircrew Weapons Trainer (CG-AWT) Cabin lightning. ASI has received the LED light panels and is prototyping the installation to enable an evaluation of the lighting and its effects on the display system during trainer usage.

016-16, MH-60T Engine High Side Failure with loss of Np and Torque indications. ASI has completed Contractor Final Inspection (CFI) and is prepared to conduct JFI.

012-15, MH-65Dx Automatic Flight Control System (AFCS), Flight Director (FD) & Control Loader System (CLS) Integration and 001-16, MH-65D1 AFCS/FD System Problem Report (SPR) Clearance. ASI is making final Trainer Test Procedures and Results Report (TTPRR) updates and will mark this as ready for JFI next reporting period.

007-15, MH-60T Cockpit Procedure Trainer (CPT) Telestra Upgrade. Site engineers have implemented the host side changes associated with the “test to speech” Automatic Terminal Information Service (ATIS) functionality on the Operational Flight Trainer (OFT), in a development environment. ASI is in the process of documenting the hardware changes made by this upgrade. ASI began and completed the integration and CFI phases on this effort on the CPT. Currently ASI is in the integration and test phase on the OFT. Site engineers will conduct JFI on the CPT and OFT with the USCG when CFI is completed on the OFT.

003-15, HC-144B OFT Forward Looking Infrared (FLIR) Universal Hand Control Unit (UHCU). The UHCU is installed in the OFT and is functional. ASI is in the process of documenting the hardware changes made by this upgrade.

Installation of final minor hardware has been deferred until after the documentation has been completed, the system is fully operational as is.

026-14, Update the MH-60T CPT and OFT Embedded Global Positioning (EGI), Inertial Navigation System (INS) Model. Based upon research and Subject Matter Expert (SME) input, ASI conducted tuning of malfunction effects related to Receiver Autonomous Integrity Monitoring (RAIM) and Actual Navigation Performance (ANP) calculation and validities. As part of CFI phase, ASI noted issues with performing Stored Heading alignments. ASI engineers captured Stored Heading alignment 1553 data off of an aircraft and is in the process deciphering and evaluating the 1553 data.

014-14, HC-144A CPT Vibrations. Changes have been incorporated into the CPT Block Configuration Upgrade (BCU) software and hardware. ASI is in the process of documenting the hardware changes made by this upgrade.

ASI will attempt to schedule formal testing of this ECP once the documentation is complete.

013-14, HC-144B OFT BCU and 012-14, HC-144B RFTD

BCU.

• The Integrated Processing Computer (IPC) has been installed and in use since early November 2016 with no operational issues noted.

• Currently, three DRs remain open. The three open DRs are written up against the Enhanced Ground Proximity Warning System (EGPWS), and the same issues are present on the aircraft. These items were written up as DRs for tracking purposes and cannot be addressed until the issues are corrected on the aircraft. As of the end of this reporting period, these same discrepancies are still present on the aircraft.

• A list of open HC-144B OFT BCU DRs is included in Appendix A for tracking purposes.

• Engineering Change Notice (ECN) o Thirteen new ECNs were generated this reporting period.

o Ten ECNs were completed and closed this reporting period.

o Additional information on specific ECNs can be found in Appendix

C.

• ASI engineers have begun evaluating options to replace the executive and possibly the operating system on the MH-65Dx devices. This effort would also replace the existing Initial Conditions (IC) scheme on the devices as well. ASI will generate an ECP based on our evaluation to the USCG for consideration.

• The HC-144 training building, Wilks Hall, has experienced numerous issues with the air conditioning for the OFT and CPT computer rooms. On one occurrence, a failed belt caused the fire suppression system to deploy. Additionally, the fire suppression system triggered an alarm for an unknown reason, and would have deployed again had the suppressant tank been refilled. ASI will monitor the areas closer and suggests that ASI site personnel be added to call lists for any future issues with the AC or fire suppression systems during non-working hours.

• ASI installed new Government Furnished Equipment (GFE) monitors at the MH-60T Instructor Operator Station (IOS). This was done to evaluate methods of mounting the monitors. ASI will generate an ECP for this effort next reporting period.

• ASI continues to monitor the MH-65D1 and MH-65D2 due to what is described by instructors as “aerodynamic lockups”, “pitch bumps” and “pitch sensitivity” issues. There was only one occurrence of the aerodynamic lockup this reporting period. ASI will continue to collect data and provide this information to the USCG.

• ASI engineers and technicians continued to support the USCG, and other contractors, with their efforts on programs contracted to other companies.

• ASI has continued to purge the obsolete records, parts, and equipment from the Flight Training System (FTS) facility. The obsolete parts will be removed from the annual inventory utilization report.

2) There was a PMCQ of 10.04 for this reporting period. The PMCQ was primarily attributed to the sim crashing for no apparent reason, failure and or sticking of miscellaneous switches and hardware and Intercommunications System (ICS) cord issues. The sim crashing was written up as “Flying straight & level sim seemed to hit something (invisible) pitch

20deg nose low & airspeed fell to zero. Sim Froze in space”, the IP froze the device while maintenance personnel looked at various things. Maintenance personnel then asked the Instructor Pilot (IP) to take the sim out of freeze and the crew was able to fly out of the condition without a host reset. The headset cables were replaced or repaired. Miscellaneous hardware/bulbs were cleaned, repaired or replaced.

g) MH-65D2:

1) There was no CDT during the reporting period.

2) There was a PMCQ of 8.65 for this reporting period. The

PMCQ was primarily attributed to issues with the copilots Attitude Director Indicator (ADI), loss of motion, loss of video one of the pilots displays and miscellaneous burnt out lights and loose hardware was removed and replaced.
