Insurance Requirements Summary
This is an Insurance Requirements attachment document establishing mandatory coverage standards for subcontractors working with McKinstry Co., LLC and the Salt Lake City School District on the Whittier Elementary HVAC Electrification project in Utah. The document outlines comprehensive insurance requirements that all subcontractors must maintain prior to commencing work, with coverage spanning the duration of the subcontract plus applicable warranty periods, statutes of repose, or statutes of limitation, whichever is longest. All required insurance documentation must be submitted to subinsurance@mckinstry.com before work begins, with certificates of insurance submitted on the industry-standard ACORD form. The requirements emphasize that all coverage must be provided as "All Operations" policies without job-specific restrictions and must name McKinstry Essention, LLC, McKinstry Co., LLC, and McKinstry as certificate holders with appropriate endorsements.
Minimum insurance coverages required include Commercial General Liability with a combined single limit of $2,000,000 per occurrence and $2,000,000 general aggregate per project; Automobile Liability with a $2,000,000 combined single limit for owned, non-owned, and hired vehicles; Worker's Compensation at statutory amounts with $2,000,000 employers liability limits; Professional Liability at $5,000,000 per claim for architectural or engineering design work or $2,000,000 for other professional services; and Contractor's Pollution Liability ranging from $2,000,000 for mechanical work to $5,000,000 for hazardous materials remediation. All required endorsements must include Additional Insured status for McKinstry and customers, Primary and Non-Contributory coverage, and Waiver of Subrogation provisions. Deductibles and self-insured retentions cannot exceed $25,000 without prior written approval, insurers must maintain a minimum A-VIII Best's Rating, and subcontractors are responsible for ensuring lower-tier subcontractors comply with these same requirements.

Subcontract - Rev Aug-16
You are being notified of a compliance issue with a certificate of insurance. Your certificate of insurance is either not in compliance with
McKinstry’s requirements, is expired or is missing. Please supply a compliant certificate of insurance ASAP to avoid a delay in the release of payments for services performed.
All vendors must comply with the insurance requirements as stated in the Subcontract Agreement, Subcontract Master Order or a
Subcontract Change Order or with these instructions. See the sample certificate of insurance starting on page 4.
NOTE: A frequent causes of non-compliance of a certificate of insurance are the following:
1. The presence of the job name, customer name, subcontract number, or other specific identifications of a single job in the Description of
Operation field. We require All Operations coverage. We prefer you to use the description in our sample certificate attached.
2. Failure to list the correct certificate holder. We require all our company entities to be named: McKinstry Essention, LLC, McKinstry
Co., LLC and McKinstry. Our company names can be included in the Description of Operation field if you prefer.
3. Failure to provide endorsements.
If you have sent in your insurance certificate thank you for complying.
If not, please do so immediately so there will not be a hold up with releasing payment for the services performed.
Please send your insurance certificate along with the endorsement to subinsurance@McKinstry.com.
Following with a mailed copy is not necessary and only causes added work.
