About this file

This is a tailored Buy American Act Certificate form for commercially available off-the-shelf (COTS) items. The form is a template designed to allow offerors to certify the domestic or foreign origin of products being offered in response to a federal solicitation.

The certificate requires offerors to complete three identification fields (Solicitation, Procurement Name, and Location) and then declare the status of their products. Section (a) requires listing of any foreign end products with their line item numbers and countries of origin. For foreign products that do not consist wholly or predominantly of iron or steel, offerors must indicate whether the items exceed 55 percent domestic content, with the instruction to select "no" if the domestic content percentage is unknown. Section (a)(i) specifies that for non-American made COTS items, only the Line Item Number and Country of Origin columns require completion. Section (b) provides a checkbox for offerors to certify that all requested items are domestically produced. The form concludes with signature and identification blocks requiring the completing person's printed name, company affiliation, and title. Additional pages may be added as necessary to accommodate multiple line items.
