About this file
This is a federal contracting form—specifically the Buy American Act Certificate (Tailored) for Commercially Available Off-the-Shelf (COTS) items. It is a template used by offerors to certify compliance with Buy American Act requirements when submitting bids on federal solicitations.
The form requires offerors to identify and list any foreign end products being offered, including the line item number, country of origin, and whether non-iron/steel products exceed 55 percent domestic content. For non-American made COTS items, only the line item number and country of origin need completion. Alternatively, if all requested items are domestically produced, the offeror may check a certification box confirming that all items listed in the solicitation are domestically produced. The form must be completed and signed by an authorized representative, who must print their name, company, and title. Additional pages may be added as necessary to accommodate multiple line items or products.
