About this file

This is a tailored Buy American Act (BAA) Certificate form for commercially available off-the-shelf (COTS) items. The certificate requires offerors to identify and list any foreign end products included in their proposal, specifying the line item number and country of origin for each. For foreign end products that do not consist wholly or predominantly of iron or steel, offerors must indicate whether the items exceed 55 percent domestic content; if the domestic content percentage is unknown, "no" should be selected. Alternatively, if all requested items are domestically manufactured, offerors may certify this by checking the provided box and submitting a signed domestic product certification statement with the certifying individual's printed name, title, and company affiliation.

This certificate form is mandatory for the Virtual Police Training Simulator solicitation (36C26326Q0877) for Fargo VAMC, a Total Small Business Set-Aside being procured by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 23. Completion and signature of the BAA certificate is a requirement for responsiveness; failure to sign will render the quote unresponsive. All offerors must be registered in the System for Award Management (SAM) and are required to demonstrate their equipment on-site to be considered for award.
