About this file
This document is a detailed Request for Proposal (RFP) for a Firm-Fixed Price contract with one base year and four option years. The government intends to award a single-source contract, with offerors required to submit four volumes: Executed Contract Documents, Technical Submission, Past Performance Questionnaires, and Price Submission.
Key submission requirements include: a 6-page limit for Immediate Maintenance and Repair Completion Timeframe, 3-page limit for Contractor Experience, and documentation of key personnel qualifications. Technical evaluation will focus on project scheduling, contractor experience (minimum three similar projects in past five years), and key personnel credentials. Past performance will be assessed through questionnaires from up to three relevant federal or commercial contract references. Pricing will be evaluated using a provided Pricing Workbook, with the Contracting Officer determining the likelihood of adequate price competition. Significant subcontractors performing 25% or more of the contract must provide consent letters, and joint ventures must follow specific documentation requirements.

Each exception shall be specifically related to each paragraph and/or specific part of the solicitation to which the exception is taken. Offerors shall provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, and specific requirements of the solicitation. As award without discussions is contemplated, Offerors are cautioned that if the Government does not agree with the Offeror’s exception to the solicitation, the Offeror could be deemed non-responsive and therefore unawardable. This information shall be provided in the format and content of the following table:
Reference Page/Paragraph Requirement/Portion Rationale
PWS, SPEC,
Model Contract, etc.
Applicable Page and Paragraph Numbers

Supplier Performance Risk System: DFARS 252.204-7024 Notice on the Use of the Supplier Performance Risk System (SPRS). The Supplier Risk tool scores vendors on DoD-wide contract performance and assesses the probability that an award made to a supplier is going to result in unsuccessful performance or will result in supply chain risk. The Supplier
Risk assessment will only be pulled for the Awardee before award and used to determine the Awardee responsible under FAR 9.104-1. Offerors do not need to provide anything for this assessment.
NIST SP 800-171 Compliance: There is not any Controlled Unclassified Information involved with this effort therefore NIST SP 800-171 DoD Assessments will not be pulled and the provision and clause are not included.
Joint Venture (JV): A Joint Venture (JV) will be considered a sole business entity. Offerors who propose as a Joint Venture (JV) are not permitted to submit a separate offer independent of the JV. The written JV agreement must be provided in the offer and must follow SBA requirements. The JV must be separately identified with its own name, Unique Entity ID, and Commercial and Government Entity (CAGE) number in the System for Award Management (SAM).
Mentor-Protégé (M-P) is a relationship; M-Ps require formal JV, Teaming Agreements (TAs), or Letters of Intent (LOIs) to qualify for consideration as a team.
Volume II – Technical Submission
FACTOR 1 -TECHNICAL:
Subfactor 1 Immediate Maintenance and Repair Completion Timeframe Offeror to provide a detailed and realistic project schedule, including key milestones, task durations, and resource allocations. Offerors should identify potential risks to timely completion and propose mitigation strategies.
Subfactor 2 Contractor Experience: Submit documentation for a minimum of three (3) successful projects of similar size and scope that have been accomplished for the Contractor within the past five (5) years.
Subfactor 3 Key Personnel List and Qualifications: Provide a list of key personnel and their job title who are performing on the contract, and indicate that all key persons are currently employed by the Contractor or have a commitment to the Contractor to work on this effort. Only the key personnel proposed shall be utilized on the awarded contract. Any substitutions for the Contractor's key personnel requires approval from the Contracting Officer.
Provide Qualifications for each key person that includes at least one (1) successful project provided that it is equivalent in size and in complexity to the facility maintenance and repair requirement proposed for this solicitation. There are no timeframe requirements for the successful project.
Volume III – Past Performance Questionnaires
FACTOR 2 – PAST PERFORMANCE:
Provide the Past Performance Questionnaire (PPQ) (Attachment #4) of not more than three (3) references of the most relevant contracts performed for federal agencies and commercial customers within the last five (5) years. These three (3) references do not have to correlate to the three (3) experience submissions in Factor 1 Subfactor 2. The Offeror can submit another Past Performance Questionnaire that has already been completed IF it adequately details the scope, period of performance, dollar value, the Offeror’s role in the effort, and discusses the Offeror’s performance in means of Management, Schedule, and Quality. The questionnaires should be sent directly to kristina.brannon.1@us.af.mil. The Government will not be responsible for ensuring the references submit the completed past performance questionnaires.
Teaming Arrangements/Joint Ventures: Past Performance Information of Team Members or Joint Venture Partners shall be consistent with the proposed roles and responsibilities that each member or partner will perform for this requirement.
Identify each member's share of the prospective contract, 50/50, 51/49, etc. Clearly establish roles of each party (who is Prime and who is Subcontractor, who is responsible for what tasks, contract administration, proposals, work management, etc.). For protection of competition-sensitive proprietary information, Subcontractor past performance cannot be disclosed to the Prime Offeror without the Subcontractor's consent.
Provide a letter from all Significant Subcontractors (Attachment #6) that will perform major or critical aspects of the requirement, consenting to the release of their Past Performance Information to the Prime Contractor, only if it is being used as part of the past performance evaluation. A significant subcontractor is defined as a contractor who performs 25% or more of the aggregate of the entire contract. Include a Statement of Acknowledgement that the Prime Contractor is responsible for adhering to contract terms and conditions and daily management. The Prime Contractor is obligated to negotiate in good faith and is responsible for conveying mandatory Government terms and conditions to Subcontractors.
The Prime Contractor shall remain fully responsible for contract performance, regardless of any Teaming Agreement between the Prime Contractor and its Subcontractors.
Volume IV – Price Submission
FACTOR 3 - PRICE:
This volume shall include the Pricing Workbook (Attachment #3) and any supporting documentation. The Offeror shall price each element required by the Workbook in order to be eligible for award.
The Contracting Officer has determined that there is a high probability of adequate price competition for this acquisition.
Upon examination of the initial offers, the Contracting Officer will review this determination and if, in the Contracting Officer’s opinion, adequate price competition exists no additional price information will be requested. However, if at any time during the evaluation the Contracting Officer determines that adequate price competition no longer exists, Offerors may be required to submit information to the extent necessary to determine the price fair and reasonable.
