About this file

This file is a Benefit Program Comparison Variances document for Franklin County Cooperative's 2025-06-09 RFP, outlining differences between current and proposed employee health benefit plans across multiple employee categories. The document structures plan comparisons for six employee groups: Hourly HMO, New Hire Hourly HMO, Salaried HMO, Salaried PPO HDHP, Salaried Student PPO, and Student HMO. The RFP encompasses medical benefits, disability and voluntary programs including Short-Term Disability (STD), Long-Term Disability (LTD), legal services, pet insurance, and ID Theft insurance (newly added). The document serves as a template for bidders to identify and document all plan deviations from the County's current plan design, with the requirement that awarded contractors provide all programs, services, and administration currently offered by the incumbent unless specifically identified as deviations.

Key identified variances between current and proposed plans include restrictions on Preferred Specialty Drugs—reducing supply limits from 34-90 days to a maximum of 30 days with 15-day limits on select drugs—and significant changes to Mental Health and Substance Abuse coverage. The proposed plan cannot accommodate the current outpatient mental health benefit structure (Visits 1-20 at 100%, Visits 21+ with $20 copay) and cannot maintain the current inpatient deductible waiver for visits 1-45, creating potential Mental Health Parity Compliance concerns. Additionally, Legal Services benefits are reduced from a 4-session limit to 3 sessions. The document emphasizes that medical policy variations are excluded from comparison scope as proprietary information and notes that additional variances may emerge during implementation review if complete Summary Plan Descriptions were not provided during the proposal process.
