About this file

This is a Contract Modification #2 and Extension #1 document between the Franklin County Board of Commissioners and Metropolitan Life Insurance Company for the continuation of voluntary group disability insurance and related employee benefits. The extension modifies and extends the original contract dated January 18, 2022 (RFP #2020-06-35), which was previously amended on December 18, 2023 to add group legal services plan benefits. The extension term runs from January 1, 2025 through December 31, 2026, with no additional extension periods remaining. The services include voluntary long-term disability insurance, voluntary short-term disability insurance, group legal services plans, pet insurance, and optional identity theft insurance for benefits-eligible county employees.

The total not-to-exceed amount for the extension is $185,000.00. Voluntary long-term disability rates are guaranteed at reductions ranging from 1.1% below current rates across all age bands, with annual premiums ranging from $29,483 to $259,327 depending on age cohort. Voluntary short-term disability rates are guaranteed at 4.2% reductions, with annual premiums ranging from $60,605 to $276,662. Employee-paid group legal services participation fees remain at $16.50 per month, while a new employer-paid group legal services option is added at $8.25 per month. Pet insurance will be entirely employee-paid with rates determined by selected plan. Identity theft insurance is available if the county elects coverage, with rates ranging from $3.00 to $7.85 per employee per month for employer-paid options and $5.55 to $12.95 for voluntary employee-paid options, guaranteed for three years. The contractor's notice contact has been updated to Charlie Cross, Senior Account Executive, at MetLife's Southfield, Michigan office.
