About this file
This is a reference questionnaire issued by the Nevada State Purchasing Division, Department of Administration, for solicitation number 40DHHS-S3547. The document serves as an evaluation tool for assessing vendor performance and qualifications as part of a formal state procurement process. Vendors and subcontractors are required to provide client references from current or former clients for projects of similar size and scope. The completed questionnaire must be submitted to Annette Morfin at amorfin@admin.nv.gov by February 10, 2026, with the solicitation number included in the subject line. The State requests comprehensive responses to all evaluation questions, with the option to answer "U/K" if information is unknown or "N/A" if not applicable.
The reference questionnaire requires clients to rate vendor performance across thirteen evaluation criteria using a scale of 0-10, where 0 indicates poor or inadequate performance, 1-3 represents below average performance, 4-6 reflects average performance, 7-9 denotes above average performance, and 10 signifies exceptional performance. Evaluation areas include vendor knowledge and expertise, flexibility regarding scope and timeline changes, quality of delivered materials and products, staff dynamics and interaction, schedule adherence and timeliness, customer service responsiveness, billing accuracy, problem resolution capabilities, business requirement flexibility, and likelihood of future recommendations. Completed questionnaires are designated as confidential business information under Nevada Revised Statutes and are not subject to public disclosure. The State reserves the right to contact references directly for further clarification beyond the written responses provided.
