About this file
This is a Limited Asbestos and Lead-Containing Paint Survey Report for a federal renovation project at the Iowa City VA Health Care System.
The survey was conducted on November 16-17, 2022, by Terracon Consultants, Inc. (licensed asbestos inspectors Ian Bootsmiller and Alex Davis) on the 6th floor of Building 1 at 604 Highway 6 West, Iowa City, Iowa 52246, in anticipation of approximately 22,500 square feet of renovation work. The survey identified asbestos-containing materials (ACM) in multiple locations requiring abatement prior to renovation. Confirmed ACMs include: thermal systems insulation (TSI) with white and grey mudded joints (10-15% Amosite, 30-75% Chrysotile); cloth-wrapped corrugated pipe insulation (75% Chrysotile); 12"x12" off-white floor tiles with grey streaks and black adhesive (4-7% Chrysotile); 12"x12" white floor tiles with tan streaks and black adhesive (4-5% Chrysotile); black adhesive associated with 12"x12" tan floor tiles (4-6% Chrysotile); and TSI white magnesia pre-formed pipe insulation (35-40% Amosite, 5-10% Chrysotile). Total estimated quantities include approximately 3,150 square feet of floor tile, 85 linear feet of pipe insulation, and 25 square feet of mudded joint insulation. The lead-containing paint survey identified one sample at 0.5% by weight (meeting EPA definition of lead-based paint) in room 6E230 on plaster walls, with three additional samples containing lead below the LBP threshold. The report recommends hiring a qualified asbestos abatement contractor compliant with NESHAP and OSHA regulations, notifying the Iowa Department of Natural Resources 10 business days prior to abatement, and completing a supplemental inspection once spaces are vacated to assess assumed but unsampled ACMs in inaccessible areas.

1.0 PROJECT OVERVIEW
1.1 Project Objective
Terracon Consultants, Inc. (Terracon) conducted a limited asbestos and lead-containing paint survey at the Iowa City VA Hospital (VA) Building 1 located at 604 Highway West, Iowa City, Iowa. Terracon understands that these services were requested in anticipation of the renovation of approximately
22,500 square feet of the 6th floor of Building 1 (the project area). This survey was conducted in general accordance with Terracon’s Proposal (P06227130) dated August 26, 2022 and the AIA
Document C401-2017 Standard form of Agreement Between Architect and Consultant dated
September 20, 2022. We understand that Bancroft has entered into a prime contract with the
Department of Veterans Affairs to provide architect/engineering services for the project.
Terracon utilized the Shive-Hattery Architecture + Engineering (Shive) Building 1, Floors Sub-basement – 10 – Asbestos Survey document dated May 13, 2016, associated tabulated survey data, and the ICVAHCS Asbestos Survey (DVA No 263-P-1225) Drawings dated June 10, 2016 (collectively, the Shive Survey) to aid in our investigation. As part of our Survey, Terracon identified additional homogeneous areas of suspect asbestos-containing materials (ACM) from the project area, as generally defined above. The additional suspect ACMs were documented, and the results are presented herein.
Although reasonable effort was made to survey accessible suspect materials, additional suspect but unsampled materials could be located in walls, in voids, or in other concealed areas or areas where investigation was not feasible without causing significant damage to finishes. Suspect ACM samples were collected in accordance with the sampling protocols outlined in United States Environmental
Protection Agency (USEPA) 40 Code of Federal Regulations (CFR) Part 763-Asbestos, Subpart E-
Asbestos-Containing Materials in Schools (known as the Asbestos Hazard Emergency Response Act
[AHERA]). Asbestos samples were delivered to an accredited laboratory for analysis by polarized light microscopy (PLM).
At the time of the Survey, the project area was occupied and/or still in operation. Terracon was provided access to the available spaces by VA Project Manager, Mr. Benjamin Mitchell.

3.1 Asbestos Survey
3.1.1 Asbestos Visual Inspection
Survey activities were initiated with visual observation of the interior of the Project Area to identify and document homogeneous areas of suspect ACM. The Shive Survey was utilized as deemed appropriate by the inspector. A homogeneous area (HA) consists of building materials that appear similar throughout in terms of color and texture with consideration given to the date of application.
The assessment was conducted in visually accessible areas of the Project Area. Although reasonable effort was made to survey accessible suspect materials, additional suspect but unsampled materials could be located in walls, in voids, or in other concealed areas.
Due to the potential for significant damage to certain building components and the need for the spaces to remain operational, some materials could not be accessed/removed or sampled, and therefore, were assumed to be ACM; please refer to Table 2 in Appendix A for a listing of these materials. Our survey activities also excluded elevator shafts, elevator lobbies, elevator cabs, stairwells, and roof(s) on 6th floor.
3.1.2 Asbestos Physical Assessment
A physical assessment of each HA of suspect ACM was conducted to assess the friability and condition of the materials. A friable material is defined by the USEPA as a material that can be crumbled, pulverized, or reduced to powder by hand pressure when dry. Friability was assessed by physically touching suspect materials.
3.1.3 Asbestos Sample Collection
Based on results of the visual observation, bulk samples of suspect ACM were collected in accordance with the sampling protocols outlined in 40 CFR 763.86 – Sampling. Samples of suspect materials were collected from randomly selected locations in each HA. Samples were placed in sealable containers and labeled with unique sample numbers using an indelible marker. The selection of sample locations and frequency of sampling were based on Terracon’s observations and the assumption that like materials in the same area are homogenous in content.
It should be emphasized that ACMs could be present in inaccessible areas such as walls, voids, or other concealed areas. In the event that materials are discovered during demolition that have not been characterized, they should be sampled prior to disturbance to determine if they contain asbestos or they can be assumed to contain asbestos and appropriately abated.
Terracon collected 48 bulk samples from 16 HAs of suspect ACM in the survey area. A summary of suspect ACM samples collected during the survey is provided in Table 3 in Appendix A and is included as part of the chain of custody (COC) presented in Appendix C. Exhibit 1 in Appendix B depicts the approximate sample locations. Representative photographs are presented in Appendix D.
Facilities | Environmental | Geotechnical | Materials 7
3.1.4 Asbestos Sample Analysis
Bulk samples were submitted under COC to EMSL Analytical, Inc. (EMSL) in Pineville, North Carolina for analysis by polarized light microscopy (PLM) with dispersion staining techniques in accordance with
USEPA’s Method for the Determination of Asbestos in Bulk Building Materials (600/R-93-116). The percentage of asbestos, if present, was determined by microscopic visual estimation. EMSL is NVLAP accredited (lab code 200841-0).
3.2 Lead-Containing Paint Survey
The LCP portion of the survey was completed by Mr. Bootsmiller and Mr. Davis concurrently with the asbestos survey. The purposes of the LCP survey were to assess for the presence of lead in coatings that might be present in materials that might be impacted by project.
Terracon collected six paint samples from painted substrates likely to be impacted by the project.
Terracon utilized a wood chisel and write-on, sealable sample bags to collect paint chips from the different surface coatings. The sample was delivered under COC to EMSL Cinnaminson, New Jersey for analysis of lead by USEPA SW-846 Methods 3050B Acid Digestion of Sediments, Sludges, and Soils and 7000B Flame Atomic Absorption Spectrophotometry. However, due to the lab running overcapacity, these samples were forwarded to EMSL’s Pineville, North Carolina lab. EMSL is ELLAP accredited (lab code 200841-0).
4.0 FINDINGS AND RECOMMENDATIONS
4.1 Asbestos Findings and Recommendations
4.1.1 Asbestos Findings
The following materials were identified to greater than 1% asbestos during Terracon’s survey:
▪ Thermal systems insulation (TSI) - white insulation associated with mudded joints
▪ TSI - grey insulation associated with mudded joints
▪ TSI- cloth wrapped corrugated pipe insulation
▪ 12-inch (”)x12” off-white floor tile with grey streaks and associated black adhesive
▪ 12”x12” white floor tile with tan streaks and associated black adhesive
▪ Black adhesive associated with 12”x12” tan floor tile
Facilities | Environmental | Geotechnical | Materials 8
▪ TSI - white magnesia (mag) pre-formed pipe insulation and associated cloth wrap
The Shive Report identified the following ACMs in the project area:
▪ Off-white floor tile under carpet
Table 1 included in Appendix A identifies the confirmed ACMs, sample locations, and approximate quantities, along with additional information. Table 3 included in Appendix A summarize the bulk asbestos samples collected as part of the project. Exhibits showing the approximate sampling locations and the approximate locations and extents of confirmed ACMs are provided in Appendix B.
Photographs of the select ACMs are presented in Appendix C. EMSL’s laboratory analytical reports and the COCs are included in Appendix D.
Without significant demolition installed flooring materials, built-ins and due to the spaces being occupied, we cannot determine the full extent of these materials at this time.
Due to the limitations placed on this Survey, Terracon was not able to investigate for the presence of select materials due to the potential for significant damage to the building materials and due to the fact that the spaces were generally occupied at the time of the Survey efforts. Table 2 in Appendix A includes a list of inaccessible areas/spaces.
4.1.2 Asbestos Recommendations
As the National Standards for Hazardous Air Pollutants (NESHAP) and Occupational Safety and Health
Administration’s (OSHA) regulations govern the removal of ACMs, Terracon recommends that a qualified contractor be hired to properly abate and dispose of ACMs identified within this report in accordance with an asbestos abatement specification; local, state, and federal regulations; and the demolition plans set forth by the design team.
The owner and/or operator are responsible for USEPA NESHAP regulatory compliance regarding the proper removal, handling, and disposal of ACMs containing greater than 1% asbestos prior to renovation or demolition. Also, per state regulations, please be aware that the owner and/or operator must notify the Iowa Department of Natural Resources (IDNR) 10-business days prior to asbestos abatement at certain quantity thresholds and prior to demolition activities.
It is likely that asbestos containing adhesives extend under walls, base cabinets and other built-ins.
Due to the location of the ACMs, the abatement contractor shall be prepared to complete significant demolition to gain access to the materials. Additionally, we recommend carpeting and other floor coverings directly adhered to ACM, be treated as ACM waste.
Any assumed (as indicated in Table 2 in Appendix A) and unsampled suspect ACMs needs to be further investigated once the spaces are vacated and significant demolition can occur to assess for the presence of the suspected ACMs. Terracon’s contract includes a return inspection once all spaces have been vacated; the return inspections needs to be completed well in advance of any demolition activity.
Facilities | Environmental | Geotechnical | Materials 9
Should project design plans change / additional work areas be added to the project scope, additional investigation will be warranted. If additional but unsampled suspect ACMs are revealed during renovation or demolition activities, the material(s) must be assumed to contain asbestos and treated as such unless sampled by an accredited inspector and laboratory analysis determines otherwise.
Our contract includes services for developing asbestos abatement specifications (project design) and for performing abatement oversight and air monitoring. We recommend that Bancroft confirms with the VA contact that these services are requested.
A summary of applicable asbestos regulations is provided in Appendix F.
4.2 Lead-Containing Paint Findings and Recommendations
4.2.1 LCP Findings
One paint chip sample was reported by the laboratory at a concentration of 0.5% by weight (the
USEPA definition of lead-based paint (LBP). The LBP identified in room 6E230 appears to extend through the floor and is ls located on plaster walls; the paint may also extend into other areas that are not included as part of this scope of services. The analytical laboratory also reported three other paint chips with concentrations of lead but below the definition of LBP. The result of the limited lead containing paint sampling is included in Table 4 in Appendix A. The lead laboratory analytical report is included in Appendix C. Select photos are provided in Appendix B. A summary of lead regulations is provided in Appendix E.
4.2.2 LCP Recommendations
Terracon recommends that the contractor(s) involved in the demolition/renovation be notified of the presence of LBP and LCP on various surface coatings. The OSHA lead standards for construction (29
CFR 1926.62) apply regardless of the lead concentrations and contractor(s) will need to provide appropriate personal protective equipment and conduct personal exposure monitoring, at a minimum.
The levels indicated could create conditions above published occupational exposure limits. No other substances were analyzed.
If waste materials containing LCP are generated, they may be regulated as hazardous waste. Lead containing paint waste from renovation activities, such as debris, paint chips, dust, and sludges, that exhibit the toxicity characteristic must be managed and disposed of as a hazardous waste under the
Resource Conservation and Recovery Act (RCRA), except whole-building demolition debris. The hazardous waste criterion for lead wastes is established under 40 CFR 261, as 5.0 milligrams per liter
(mg/L) measured by the Toxicity Characteristic Leaching Procedure (TCLP). A summary of lead regulations is provided in Appendix E.

5.0 LIMITATIONS/GENERAL COMMENTS
Terracon did not perform sampling that required demolition or destructive activities such as knocking holes in walls, dismantling of equipment, or removal of protective coverings. Reasonable efforts to access suspect materials within known areas of restricted access (e.g., above ceiling grids, in pipe chases) were made. Please note that final demolition and/or renovation plans were not provided to
Terracon at the onset of these services. Terracon relied on documentation provided in the 2016 Shive
Survey of the Iowa City VA to conduct the limited asbestos portion of the survey. Limitations associated with the 2016 Shive Survey were not provided to Terracon.
Terracon’s proposal P06227130 dated August 26, 2022, included costs for a supplemental inspection that will be conducted once the floor has been vacated by all VA operations and prior to construction/demolition activities. During this supplemental inspection, Terracon’s Iowa licensed asbestos inspectors will access suspect materials that have been assumed during this initial survey.
This task will include destructive investigation of materials that will be removed from the project areas.
This Survey was conducted in a manner consistent with the level of care and skill ordinarily exercised by members of the profession currently practicing under similar conditions in the same locale. The results, findings, conclusions, and recommendations expressed in this report are based on conditions observed during our survey of the project area as generally defined herein. The information contained in this report is relevant to the date on which this survey was performed and should not be relied upon to represent conditions at a later date. This report has been prepared on behalf of and exclusively for use by Bancroft for specific application to their project as discussed. This report is not a bidding document. Contractors or consultants reviewing this report must draw their own conclusions regarding further investigation or remediation deemed necessary. Terracon does not warrant the work of regulatory agencies, laboratories, or other third parties supplying information which may have been used in the preparation of this report. No warranty, express or implied is made.

HA16: Thermal Systems Insulation (TSI), white magnesia (mag) pre-formed pipe insulation and associated cloth wrap tlrowley Text Box Notes 1.These locations are approximate and should be utilized for informational purposes. The abatement contractor should confirm all locations and quantities.
2. Due to the complexity of the piping system, not all Asbestos Containing Material (ACM) TSI runs may be shown.
3.Mudded joint packings associated with all layered paper pipe insulation (and fiberglass runs) are ACM and are not depicted. If there is a hard fitting; it is considered ACM. The pipe hangars also have mudded packings that are ACM.
4. Due to the limited nature of this survey, Terracon was not able to evaluate the extent of flooring adhesives without causing significant damage to building materials. This ACM is likely to extend under demising walls, built-ins, and other furnishings. Additional investigation may be warranted to determine the full extent of the materials.
