• Grade D - Poor Condition. Past assigned useful life. Failure is not critical.
“Poor” or “Problematic”
• Grade F - Critical Condition. Needs immediate attention. “Failing” or “Critical”
VAMC Staff Assistance
• Site access and badging
• Small conference room or work area with internet access, telephone, and copier
• VAMC personnel knowledgeable of the electrical system
• Obtaining special permissions (pictures, roof access, etc.)
• Existing data (drawings, maintenance records, testing procedures, etc.). It is critical this information is current.
• Facility Ground Resistance Test Reports (required every 36 months per VHA Directive 1028)
• Outages
Outages
• Outages may be required to obtain specific data – especially on the medium voltage system
• Lock-out / Tag-out procedures
• We can advise early in the survey if an outage is required
• We can plan accordingly to minimize outage duration
• Can be performed during off hours
• VAMC personnel must operate equipment and perform any power transfer
Safety
Electricians will wear appropriate PPE to open panels
Personnel with minimal PPE will stay outside of limited approach boundary until panels are open. After panels are open, they may enter the limited approach boundary but cannot encroach on the restricted approach boundary per NFPA 70E.
• NFPA 70E Table 130.5C
• Allows for examination of insulated cable without manipulation
• Allows for non-contact inspections outside the restricted approach boundary
Safety
• Local or VAMC safety procedures
• Please advise on site-specific safety procedures
• Example: Two-man rule required to enter substation building
• General safety procedures
• Personal Protective Equipment (PPE)
• Lock-Out / Tag-Out
• Urgent safety issues (signs of arcing in a panel) will be conveyed to the VAMC staff immediately (will not wait for the out-brief)
• Non-urgent safety issues (a box stored in front of a panel) will be discussed in the out-brief.
The Final Report
• The VAMC Chief Engineer will receive:
• A completed Final Report, including all corrected data from the 60% and 90% submittals and the coordination curves shall be printed in color.
• Responses to the 60% and 90% submission comments from CFM, VHA and VAMC, including annotations and response.
• Electronic copy (CD) of the entire report including all SKM data, AutoCad, Excel files and PDF.
• Color photos that show devices listed on the deficiency report.
• Arc Flash Boundary Labels reflecting the Electrical Hazard information.
NOTE: Labels need to only be provided to the VAMC.
• Final report will have a stamp and signature from a Licensed Professional Electrical Engineer.
• Copies also provided to the VISN, VA OCAMES, and VA CFM

Bowman Study 2.8.1 10 1
Protective relay for E-Wing transformer is noted as "still pending." Since this is the Final submission, how will this lack of information be corrected? This affects both the report narrative and TCC sheet 59.
The relay information was obtained after submission for peer review. The relay has been included in the current submission.
Removed sentence in 2.8.1 stating that the relay information was not available. Updated model and analyses to include the effects of the relay.
Bowman Study 4.4 365 2
The recommended reconfiguration described for the feeder originating in Panel BEH1 should be listed in the FCA table in Section 7.6 so that this cost is captured and this recommendation is not missed.
The estimated costs for reconfiguration of the feeder have been added to Section 7.6 and cost estimate detail
Updated costs summary table, FCA table, and cost estimate details in Appendix J.
Bowman Study 4.7 634-637 3
In regard to 90% comment #6 (suggesting consideration for a legend for these sheets), your response indicated that a legend is provided on the "appendix title sheet", presumably indicating Appendix E. When reading through Section 4.7 of the report, it seems unlikely that a reader would know to skip to Appendix E to find the legend describing the meaning of the yellow highlights.
This response was in error as a different comment had been made concerning the highlighting used in Appendix E. The cover sheet for the breaker settings table in Section 4.7 has been updated to state that the highlighting has been used to bring attention to the settings that are recommended to be changed. The highlighting in the tables in Appendix E has been used to bring attention to items that are graded "D".
Added statement to cover sheet of recommended circuit breaker settings table in Section 4.7 to state that the highlighting is used to draw attention to the settings that are recommended to be changed.
Bowman Study 6.4 726 4
Related to comment #1, the transformer for Building 1 should not be labeled as 152 calories/cm2 which is certainly inaccurate. This value is a function of not having protective relay information modeled rather than the true operation of the system.
The 152 calories/cm2 was evaluated before the relay information was obtained.
However, even with the relay in the analysis, the incident energy is at 146 cal/cm2. This is primarily due to the fact that the trip time is close to 2 seconds, and that the transformer secondary is being evaluated with horizontal conductors in metallic box (HCB) correction factor.
Model and arc flash analysis/table had been updated once the relay information had been obtained.

Bowman Study 7.5 744 5
In relation to 90% comment #2, you responded that (along with the replacement of 3 pole ATS with 4 pole) no modifications to the generator at Building 110 are required since the generator is already a separately derived system. This seems unlikely since a generator currently equipped with 3 pole ATS should not have been designed or constructed that way. Also, your response that the neutrals from the utility, the generator, and the load are bonded does not mean that the neutral and ground at the generator location are bonded, or there is adequate grounding at the generator location, as is required to create a separately derived system. Suggest you revisit this issue.
The grounding plan for the CFA generator building shows a ground rod at each of the 4 building corners, and a ground loop that connects each of these ground rods. The generator is bonded to this ground loop. The service transformer has its own ground rod, located outside the building, that is bonded to both the service transformer and the building ground loop. Grounding at the generator appears to be adequate. Ground resistance and presence of neutral-ground bonds would be verified and corrected as part of the ATS replacement.

For assemblies/systems, such as Elevators, with transformers/disconnect/panel (E701 Zone D8) and fire pump systems (E702 Zone L9), which have equipment co-located with each other, please dotted line around to denote them
We will put a dotted line box around the elevator motors, fire pumps, and the equipment that is located in the same room
Drawings updated with bounding boxes
Drawing E701 2
For Circuit 1-BUS-1EH2, could you put a note next to the BUS or dotted line around that whole branch - Veterans Benefits
Administration (VBA). A text box has been added to the One-Line Drawing updated with text note
Watanabe Drawing E702 3
Based on our ACC drawings sheet E.8.3 Zone B7, branch circuits with 30-CT-1 and 30-CT-2, with VFDs connected to them are not shown on the one-line. Could you verify to why?
Out of scope?
These are 10 HP motors, which are typically excluded from the one-line. We have all of the breaker and conductor information, so we've added them to the model.
SKM model updated and SKM outputs (reports, one-lines, analyses) that include these feeds have also been updated
Watanabe Drawing E702 4
Zone I8, 30-VFD-CHLR-1 and 30-VFD-CHLR-2, could you verify that the items are identified correctly, in reference to the ACC As-builts E8.2, E8.3 provided. I could not identify or locate in reference to as-builts.
Each Chiller has a retrofit kit with a Trane TR200 VFD installed. None
Watanabe Drawing E702 5
Some of the pump equipment on roof is not captured, is that because they were are out of scope of the analysis? ACC drawing E8.3. B.9
Smaller motors are not captured in the One-line per the SOW. The larger AHU's have been added.
SKM model updated and SKM outputs (reports, one-lines, analyses) that include these feeds have also been updated
Watanabe Drawing E702 6
Zone G14 - 30-EF1 and 30-Jockey-Pump, can you add the room locations. Also, 30-EF2 is missing, it is on the ACC drawings but not located within this study.
Room numbers for EF1 and the Jockey Pump have been added.
The drawing you referenced in comment 4 (E8.2) shows EF2 being fed from Panel E. There are only 6 breakers on Panel E with conductors landed on them, none of which are labeled EF2. Are you sure this Exhaust Fan is still connected? If so, can you confirm where it is fed from?
SKM model and one-lines updated with room labels
Watanabe Drawing E702 7
Zone H9 - 30 Bus-DL 1 DISC, 30 Bus-SL-1 DISC, 30 BUS-DL DISC, could you denote what equipment these were for? I don't believe the disconnects on the loading dock were labelled. We can label them correctly to match the one-line
These disconnects are located on the loading dock, just outside of the main electrical room. We're assuming that DL stands for Dock Leveler, and SL stands for Scissor Lift. From our photos these disconnects do have labels, SL 1, DL 1, and DL 2. The name for DL 2 has been updated to match.
SKM model updated and SKM outputs (reports, one-lines, analyses) that include these feeds have also been updated

Please note, actual cost may differ in comparison with RS MEANS COSTWORK due to the RSMEAN being an average of all cost within the area. Area cost factor for Honolulu are typically higher than what is published by RSMeans, typically actual contractor bids for award are reflective of the industry economics and extra administrative cost of federal contracts.
We will add a note to Section 7.6 to bring attention to the possible cost differences

Could you add another column to the comments, if the action was rolled into the study, could you denote where possibly. I know there are a lot of "we can". For the comments for the 60%, if you can update to show which "we can" actions were implemented. I feel like I would like them added if they weren't addressed.
There were only 2 comments on the 60% where the response was a "we can" - 1. to add a scenario for the alternative feed 2. to add a column to the table of recommended adjustments to include the corresponding TCC's. In hindsight, we should have used "we will" as we were intending to incorporate changes from these comments. Both of these comments were incorporated into the 90% submission.
A column has been added to the comment sheets to briefly describe any changes resulting from the comment.

Just as a general note - ACC - should be denoted as "Acute Care Clinic". Officially, the space for occupancy purposes is by definition in NFPA101/99, the spaces are considered business occupancy, NOT ambulatory care or heatlhcare, since the clinic or areas DO NOT provides anesthesia or other treatments that leaves the patient unable to care for himself without assistance in an emergency situation. Consequently, could you update/denote to ensure that if Joint Commission reviews our documentation that they do not make the same mistake due to compliance requirements being vastly different.
We will update Section 2 with a note stating the desire to refer to the Ambulatory Care Clinic as the Acute Care Clinic. The subsequent references in Section 2 will also be updated. Please note that this building is referred to as the Ambulatory Care Clinic on documents provided by the VA CFM, the previous set of one-lines, and also the VAPIHCS "About" website.
Updated section 2 with note concerning reference name change and subsequent uses

Bowman Study 2.5 9 1
Based on the extreme oversizing of three of the existing generators, was any consideration given to replacing them with more appropriately sized units?
The risk of wet stacking is currently being mitigated by annual load banking. The VA could investigate replacing their generators with smaller units, but would have to investigate the projected load that might be added by any expansion for the next 10- 20 years to ensure the new, smaller generator is not undersized. None
Bowman Study 2.5 9 2
You indicate a deficiency related to the 3-pole ATS in the Generator 110 system and reference it to the feeding of non-motor loads. The related recommendation in Section 7 (page 728 of the pdf) is to replace the ATS with 4-pole switches in order to meet the VA design guide. Two comments here: 1) On re-reading the current Electrical Design Manual, I think there are two possible interpretations related to the need for 4-pole ATS, and the feeding of non-motor loads does not require the use of 4-pole switches. I would suggest reviewing the Design Manual again. 2) If your interpretation remains that 4-pole switches are required, your recommendation needs to take into account the modifications that must be made to convert the generator installation to a separately derived system, to ensure proper operation of the overall system once the VA replaces the 3-pole switches with 4-pole switches.
The generator for building 110 is already a separately derived system. It is a wye connected generator and the neutrals from the utility, generator, and load are all bonded to a bus bar in the ATS's. None
Bowman Study 2.8.1 10 3
OCFM typically expects outages to be taken to definitively determine transformer fusing. Assumptions taken on protective devices can result in flawed results, both for protective device coordination and for incident energy calculations. We recommend taking outages to obtain this data.
Understood. We opted to obtain the fuse information from manufacturer/site documentation.
None
Bowman Study 2.8.2 10 4
Using similar protective devices as substitutes for unknown devices carries some risk as even similar products can have quite different time-current characteristics and produce dissimilar results, especially as it relates to incident energy. A better approach may be to simply label the device unknown and use the SKM feature where you can exclude a device from consideration and force the upstream device to act.
Understood. We will look into if certain devices need to be excluded to allow an upstream device to act.
None. No unknown devices were found where the upstream device should be allowed to act.

Bowman Study TCC sheet 59 267 5
In Section 2.8.1, you stated that service transformer primary fuses were assumed. An upstream breaker and relay is now shown, but there are no upstream fuses in the model one line diagram or in the TCC graph. On the one line diagram, there is a note indicating that relay information is still to be added to the model. Does the Section 2.8.1 statement need to be revised?
Are there no transformer primary fuses? Or are they just missing from the model? Likewise, it should be stated earlier in the report that the relay information is not included.
Section 2.8.1 needs to be revised.
The one-lines and TCCs were updated with the current fuse information for the transformers that have fused primaries. Only the relay protection for the one transformer is still pending. We will add a statement concerning the relay information if it is not to be included.
