X X X June 14, 2019 @ 11am Central X one (01) RE: Expand Emergency Room Department

See clarifications and pre-proposal site visit sign-in sheet attached.

Tabitha Contreras Contracting Officer Clarifications Project: Expand Emergency Department Project #: 580-17-101

1. Question: Are funds available for the award of this project?

Answer: Yes

2. Question: Drawing TK102 shows a Card Reader on the door of the room immediately adjacent to the ER elevator. Next to the symbol (CR), is a question mark. Please confirm is this door is to receive a card reader.

Answer: Yes, as shown on the drawings.

3. Question: Is there a proprietary contractor required to use for the Public Address/Mass Notification System? Since Offerors are to extend the existing system, we will need to know if only one contractor is allowed to work/service the existing equipment. Also, please provide information on the type/equipment/manufacturer of the existing PA/MNS.

Answer: The contractor must be certified by the manufacturer, VA currently uses Philstyn Audio. POC Bruce Tappe 713-747-7000

4. Question: Is there a proprietary contractor required to use for the Nurse Call/Life Safety Code Blue Communication system and/or equipment? Since Offerors are to extend and tie-in to the existing system, we will need to know if only one contractor is allowed to work/service the existing equipment. Also, please provide information on the type/equipment/manufacturer of the existing Nurse Call/Life Safety Code Blue Communication system and/or equipment.

Answer: The contractor must be certified by the manufacturer, VA currently has Rauland Borg Responder 5. The facility currently uses Lone Star Communication for work related to nurse call.

5. Question: Is this particular ER tied into a backup emergency generator? If so, would the VA please confirm that battery backups are not needed then if the overall area is backed up by an emergency generator?

Answer: Light fixtures equipped with batteries are not called for in the construction drawings for the project. Not all fixtures in the “ER” space will have generator backup. Only those fixtures shown connected to existing Panel “111SHA” will have generator backup. Per the Luminaire Schedule, Fixtures with names that include “EM” shall be equipped with Generator Transfer Device. Generator Transfer Device is shown on Detail 1, Sheet E-501. Generator Transfer Devices shall be incorporated in the subject fixtures by the manufacturer or the devices shall be concealed in accessible ceiling space near the subject fixtures. Typically, the Generator Transfer device would switch from a normal power source to an emergency power source when the normal power source suffers an outage. The transfer device also bypasses the room switching to turn the subject fixture on during a power outage even if local switching has the fixture turned off. Fixtures shown on the drawing as being fed from existing Panel “111SHA” will not require sources from both normal and emergency panels but shall be fed as shown on the drawings. However, the Generator Transfer Devices for these fixtures shall be connected to bypass the room switching as described and as shown in Detail 1, Sheet E-501.

6. Question: With phased construction required, as well as TAB and commissioning, will the VA allow Offerors to TAB and commission systems and equipment within each phase? Continuing this question, will the VA consider completion and turnover of each phase as final acceptance of that area and all new systems, equipment and material installed, and therefore warranty periods will commence?

Answer: Per Section 23 05 93, Paragraph 3.12.A – Phased Projects; TAB shall be completed per the phasing and then upon project completion, all areas shall have been test and balanced. 1A and 1B would have some over lap on the equipment making it difficult to split the warranty. It could be separated into area 1 and area 2

7. Question: Section 01 91 00, paragraph 1.7(B), includes “systems to be commissioned” as part of this project. However, the lists appears to be extremely generic and is not project-specific. For example, it includes conveying equipment (dumbwaiters, elevators, etc.). In an effort to ensure the VA is clear and direct about what systems/equipment Offerors are to commission, please revise the list provided in this section and ensure it project-specific. Clearing up this generality will ensure offerors don’t make bid assumptions that result in disagreements post-award between VA and Contractor.

Answer: Each discipline has an associated specification indicating which items shall be commissioned.

8. Question: Sheet GC102 deduct note #6 says that the contractor is to provide a unit price for sliders vs curtain, door and frame however the Bid CLIN does not call for unit pricing. Please confirm that we will be bidding per the Bid Schedule CLIN and provide lump sum pricing for the deductive alternate.

Answer: See attached revised bid/price sheet and submit pricing proposals in accordance with those deduct items.

9. Question: Sheet QH101 lists M7405 exam lighting as contractor provided, however specifications have not been provided for this item. Please provide specifications for the exam lighting.

Answer: Exam Light fixture shall be as described for Type “G” Luminaire in the Luminaire Schedule on Sheet E-603.

10. Question: Please provide a detail for at type F-3 HM door frame, as referenced on note 8 on AE601. The detail is not shown.

Answer: The type F-3 frames will not be used the opening will remain sized for the sliding units and the openings will get curtains as indicated on the drawings. No door or frame installed.

11. Question: Please specify the dimensions of the F-3 type door frames that would replace the aluminum sliding doors as part of deduct alternate 3. Note 8/AE601 says to frame the wall to the new door size but the new door size is not specified.

Answer: The type F-3 frames will not be used the opening will remain sized for the sliding units and the openings will get curtains as indicated on the drawings. No door or frame installed.

12. Question: Please provide specifications of FRP-X panels listed per sheet note N/AE701.

Answer: This is to be a Fiberglass reinforce wall panel for the wet areas. Not a type X panel.

13. Question: Please provide material selection and color selection info for ITEMS RB-1, CT-1, C2-2, PT-1, PT-2, PT-3, ACT-1, PL-SC noted on sheet AE701 color and finish schedule.

Answer: These materials are specified, and the finishes and colors will be selected by the COR from the manufacturer’ standard colors and finishes.

14. Question: The division 09 06 00 schedule for finishes does not contain any finish details. All of the tables are blank. Please provide a complete finish schedule including colors, models and material types.

Answer: No colors or finishes were provided final color selections and will be made by the COR from manufacturer standard colors during the submittal process.

15. Question: Specifications 10 14 00 2.3A references that signage must conform to the VA design guide. Please provide the VA signage design guide.

Answer: See attached signage information for phase 1A and 1B

16. Question: Sheet AE801 general sheet note states that the contractor is to provide a unit cost for entire signage package. The provided Bid CLIN does not have lines for unit cost, please clarify.

Answer: See attached signage information for phase 1A and 1B

17. Question: Please provide a signage plan layout so that we can determine the locations and qty of each sign type.

Answer: See attached signage information for phase 1A and 1B

18. Question: Equipment types M1620 and M7435 are shown on sheet QH101 but it does not list if it is the responsibility of the VA or the Contractor to provide/ install. Please clarify who is responsible to provide and install this equipment.

Answer: Both pieces of equipment are Contractor Furnished Contractor Installed.

19. Question: The Equipment Schedule on sheet QH101 lists A1200 lift system, freestanding overhead rail, floor supported as VA provided and installed. Please confirm that Item A1200 is VA provided.

Answer: This equipment is provided and installed by the facility.

20. Question: The Equipment Schedule on sheet QH101 does not list overhead patient lifts, however a specification has been provided for 11 73 00 Ceiling Mounted Patient Lift System. Please clarify if the overhead lifts are required to be provided and installed by the contractor and provide the correct equipment listing for this item on the Equipment schedule.

Answer: A1200 List system note in RFI question 19 is the patient lift system. There is no section for a floor mounted lift so this section was modified and does note the floor mounted unit called out in the drawings.

21. Question: Please provide specifications for Item “A1107 Rail System, Utility, Gas and Electric” shown on the QH101 Equipment Schedule.

Answer: Specification Section 10 25 13 “PATIENT BED SERVICE WALL” is this system. Design based on Amico surface mounted double tier system.

22. Question: Please provide architectural drawings and details of the new Patient Headwalls.

Answer: The item is specified, and the drawings and details will be provided by the supplier in the submittal phase. Design based on Amico surface mounted double tier system. Equal is acceptable.

23. Question: What are the dimensions of the Headwall units?

Answer: The item is specified and the drawings and details will be provided by the supplier in the submittal phase. Design based on Amico surface mounted double tier system. Equal is acceptable.

24. Question: Are the patient bed service wall units required to have a television remote control unit included? If so, please provide specification and details of what is required.

Answer: There are NO TV’s in the rooms, see QH101, QH102 and AE401 for info (These are Telemetry monitors)

25. Question: What model/type of Patient Bed Service Wall units were used as the basis of design?

Answer: See Specification section 10 25 13 for unit requirements. Design based on Amico surface mounted double tier system. Equal is acceptable.

26. Question: Sheet QH102 Exam room 1 which is shown as the TYP example for exam rooms 1-3, does not show any callouts for any of the equipment within these rooms. Please provide a revised plan with the callouts for this room.

Answer: Exam rooms 1 – 3 are the same equipment configuration as Exam room 6.

27. Question: Please provide a ceiling plan that shows the locations of the ceiling mounted exam lighting, the J box locations that serve the exam lights are shown on the electrical plans, but the mounting locations of the exam fixtures are not clearly shown on the architectural or equipment plans.

Answer: Sheet AE421 shows a typical location of the exam room light and Detail 11/AE502 shows the mounting detail for the light through the ceiling to the interstitial floor slab. See electrical drawings for J-box locations.

28. Question: In review of the deductive alternates, the language states for deduct #1 to perform all base bid work, except replace the switchable glass for the exam room sliding doors and replace (in lieu of) with standard cubicle curtain system. The price provided should be the base bid, minus the switchable glass for the exam room sliding doors removed for standard cubicle curtain systems. However, for deductive #2, the similar language continues and states, “Contractor shall perform all work except Deduct #1 and eliminate Phase 2B work from the project.” The price added on the CLIN schedule is Base Bid minus Deduct #1 plus Deduct #2, equals total for CLIN No. 3. Similarly, deductive #3 states, “Contractor shall perform all work except Deduct #1, Deduct #2 and eliminate room sliding doors and add cubical curtains in L-shape.” The price added on the CLIN schedule is Base Bid minus Deduct #1 plus Deduct #2 plus Deduct #3, equals total price for CLIN No. 4. Based on this language and pricing schedule, it appears that if CLIN No. 4 is awarded, all three deducts and base bid will be awarded. Is that correct?

Answer: See attached revised bid/price schedule.

29. Question: Please confirm that only one (1) hardcopy of the Price Proposal is required, and one (1) original and two (2) hardcopies (i.e., a total of three hardcopies for the technical) of the Technical Proposal are required.

Answer: Confirmed.

30. Question: Technical submission requirements ask for a list of sub-contractors for this project to be included in the proposal. However, since pricing often comes down to the last minute, especially for mechanical and electrical subcontractors, can Offerors simply list known, major Teaming Partners only? Identifying subcontractors often come within ten minutes of bid time, and that’s too late to list in the proposal.

Answer: Offerors are required to list any possible subcontractor for the trades specified in the evaluation factor they intend to use if one is not decided upon at the time the proposal is due. Any and all proposed subcontractors must have the required minimum number of years’ experience to be considered technically acceptable.

31. Question: The second page of the CLIN schedule asks for a Divisional Breakdown of material, labor, equipment and total costs per division. Since pricing typically is finalized minutes before the bid is submitted, knowing these numbers prior is very difficult. Would the VA please consider allowing Offerors to submit this form, via email, within one hour of bid time? This would provide Offeror time to complete the form based on the final bid, and would ensure the VA that realistic and actual costs are included in this form.

Answer: Offerors, to the best of their ability, shall submit the price breakdown with their hard copy proposal submission.

32. Question: Is there funding for this project?

Answer: Yes

33. Question: What is the order of precedence for contract documents? For example, do the contract specifications take precedence over the contract drawings, scope of work narrative and solicitation? Please provide an order of precedence for these bidding documents.

Answer: Please refer to FAR 52.236-21(a).

34. Question: It is difficult to understand the potential impact (cost and time) associated with complying with FAR Clause 52.236-8 without the VA providing known information. Please provide information on adjacent work, hospital operations or any other Clause 52.236-8-related issues that might impact this project’s progress or scheduling.

Answer: There are no projects scheduled at this time.

35. Question: What is the timeline for obtaining a VA badge? Will any VA-related delay by the VA office that issues the badge be cause for an appropriate time extension to the Contractor?

Answer: The process will start once the contract is in place. The timeline line depends on the type of badge needed. We do not expect any delays that would impact the project.

36. Question: Can the VA advise as to where the Contractor may store materials?

Answer: Limited storage may be available near the parking garage.

37. Question: Is there an identified location for contractor parking, laydown yard, etc.?

Answer: Contractor parking would be in Lot W near the parking garage. Some storage would be available in the same area. A dumpster and possible job trailer could be located on the west end of the hospital in the parking lot.

38. Question: There are a total of four (4) phases of construction: Phase 1A, 1B, 2A and 2B. Can Phases 1A and 1B be performed concurrently, as well as Phase 2A and 2B? Or do the phases need to be performed sequentially?

Answer: Phasing is provided so certain functions of the Emergency Department will remain operational at all times so the work must be accomplished per each individual phase prior to moving to the next phase. Coordinate with COR.

39. Question: Is there any timeline between each phase that the end-user will need to relocate? If so, what is that time period that the Contractor will be not working?

Answer: It is intended that the contractor can continue work in other areas while VA end-users relocate.

40. Question: Will temporary utility needed for construction services (i.e., water, electric, etc.) be provided to the Contractor? If so, will there be a cost? If there’s a cost, what is the rate so all Offerors can include that appropriate cost in our estimate?

Answer: Yes, at no cost to the contractor.

41. Question: Section 01 00 00, paragraph 1.23 calls for a commercial, professional photographer to photo-document this project throughout construction. In an effort to minimize cost, would the VA please consider removing this requirement and allow project Superintendents to document daily photos via their smart phones?

Answer: Photos from the project superintendent are acceptable.

42. Question: Section 01 35 26, paragraph 1.7(C), states, “However, the SSHO has be a separate qualified individual from the Prime Contractor’s Superintendent and/or Quality Control Manager with duties only as the SSHO.” In an effort to minimize cost, would the VA consider allowing the SSHO to serve as the Superintendent, as long as that individual meets the qualifications of both positions?

Answer: Yes, that will be permitted.

43. Question: What Infection Control Class classification is required for this project? The specifications don’t have a class listed, but the drawings indicate Class III. Please clarify.

Answer: The information was provided by the VA to put on the drawings and Class 3 is required ICRA class. Follow PCRA Sheet G-102.

44. Question: Will the Contractor be allowed use of an elevator during both demolition and construction activities? If so, which elevator?

Answer: Yes, Green Service Elevators are available and the closest to the work site.

45. Question: The Section 9 finish schedule is completely blank. Please provide a completed finish schedule in Section 9 so Offerors know what the finish level requirements.

Answer: No colors or finishes were provided final color selections will be made by the COR from manufacturer standard colors. Reference specifications for level of finish required on walls for painting.

46. Question: Is there a sole source or proprietary subcontractor to be utilized for the Fire Sprinkler/Fire Alarm work?

Answer: No, new items need to be compatible with the existing system.

47. Question: Is there a sole source or proprietary subcontractor to be utilized for the DDC controls work?

Answer: No, new items need to be compatible with the existing system.

48. Question: Is there a sole source or proprietary subcontractor to be utilized for the access control work?

Answer: No, new items need to be compatible with the existing system.

49. Question: Is there a sole source or proprietary subcontractor to be utilized for the video surveillance and/or security work?

Answer: No, new items need to be compatible to the existing system.

50. Question: There is medical air, oxygen, gas or vacuum work on this project. Once the work is performed, do the new lines need to be certified? If so, is that system isolated in a manner that will allow the newly altered/renovated lines to be recertified within the space being renovated? If not, is it the VA’s intent to have the entire floor(s) or system(s) be recertified? If yes, this could be a significant cost.

Answer: Per NFPA 99-2018 paragraph 5.1.12.1.5: “Breached portions of the systems subject to inspection and testing shall be confined to only the specific altered zone and components in the immediate zone or area that is located upstream for vacuum systems and downstream for pressure gases at the point or area of intrusion.” The system is isolated to the area served by the mechanical room and controlled by the Zone Valve Boxes. This is the only area impacted not the entire floor or system.

51. Question: Section 07 81 00, paragraph 3.3(A) states, “the applied fireproofing to be tested by a COR approved independent testing laboratory and paid for by the Contractor.” Please provide a list of COR-approved independent testing laboratories so Offeror’s can get pricing from approved companies.

Answer: The VA will perform these inspections; Independent Lab will not be required.

52. Question: The specifications require a third party firestopping inspector to perform inspections and final reports. In order to provide cost savings, would the VA please consider removing this requirement?

Answer: The VA will perform these inspections. Independent Lab will not be required.

53. Question: During the site visit, the VA instructed Offerors to not take pictures. Therefore, can the VA please provide detailed photos of the four phases to be renovated? Specifically, the Emergency Room areas (2A and 2B), but also 1A and 1B.

Answer: Pictures provided for the work in 2A and 2B.

54. Question: Can the VA please provide direction to Offerors for the required haul route for demolition material and to bring in new equipment and material? It appears the most efficient path would be to take it out the front door of the ER department.

Answer: The contractor may use the green elevators. A dumpster can be located in the parking lot at the west end of the building. There is an exit door close to the location on the back of the building. No material will be permitted through the main entry doors into the ER from outside.

55. Question: In our experience, JCI is the proprietary contractor for DDC controls. However, during the site visit, it was discovered that another controls equipment was being utilized. Please clarify and confirm the scope of work with regard to the controls. Are Offerors to provide JCI DDC controls that tie back into the existing non-JCI equipment? Does the VA know if the two systems are compatible?

Answer: It is the contractor’s responsibility for the newly installed controls to be fully compatible with the existing system. The existing is Metasys and all new controls shall be compatible.

56. Question: Drawing GC103, Sheet Key Note #6 states, “Deductive Alternate 3 – Eliminate exam sliding doors. Add cubicle curtains in L-shape. Contractor to provide a unit price for sliders vs. curtain/door and frame.” However, the CLIN schedule, page 3 of 42, does not ask for, nor does it include a space to input, a unit price for sliders vs. curtain/door and frame. Please clarify direction to appropriate price and bid Line Item No. 4, Deduct #3.

Answer: See attached revised bid/price schedule.

57. Question: Does the end-user intend to occupy the completed Phase 1A while the contractor is completing Phase 1B? If so, it appears that Contractors will be coming and going through Phase 1A space, specifically new Corridor C1A-715 and C1A-711, to access Phase 1B space. Will this be acceptable?

Answer: Yes, each phase must be completed before moving to next phase.

58. Question: Will Phase 1B be occupied by the end-user during Phase 1A construction?

Answer: Yes, 1B must remain fully operational during Phase 1A construction.

59. Question: Is there any existing equipment within any of the phases that the Contractor is responsible for relocating for the end-user? If so, please provide a detailed list and the location item are to be relocated.

Answer: No.

60. Question: It was observed during the site visit that some existing rooms, which are scheduled to be demolished, have a WiFi receiver just in front of the door, mounted to the ceiling tiles. Are Offerors to remove and return these items to the VA, or will the VA ensure all these items will be removed prior to Offeror’s commencing with demolition?

Answer: Wireless access points WiFi shall be protected during construction, bag them and secure above ceiling, reinstall when done. Coordinate with COR.

61. Question: Is there any equipment, furniture or fixtures that Offerors are to remove and return to the VA prior to demolition? If so, please direct Offerors to a complete list of items to save.

Answer: This information is noted on the demolition plan. Coordinate with COR for items to be turned over to VA. Typical items would be doors, frames, hand rails.

62. Question: Drawing E-501 state under 1A, Wall framing may consist of either wood studs or steel channel studs. Please confirm that wood studs are acceptable.

Answer: Wood is NOT acceptable. The architectural drawings dictate partition construction.

63. Question: Spec Section 08 34 53 is included in the bid documents, but there is no indication of location or quantity of forced-entry resistant and/or ballistic resistant security doors and frames, including no details listed on the Door Schedule on Drawing AE-601. Please indicate on the drawings which doors are to comply with Section 08 34 53.

Answer: This is not required on this phase of the project.

64. Question: Drawing AE701, Room Finish Schedule, lists PT. Please clarify if the Sherwin-Williams-manufactured PT standards for paint or painted texture.

Answer: No colors or finishes have been provided, all finishes are specified, and the Contractor is to submit the manufacturers colors and finishes in the submittal phase for selection by the COR. PT is the paint.

65. Question: Drawing AE701, Room Finish Schedule, lists PL-SC. Please identify what PL-SC stands. And, identify what Wilsonart model number/type/color, etc., Offerors are to price.

Answer: No colors or finishes have been provided, all finishes are specified and the Contractor is to submit the manufacturers colors and finishes in the submittal phase for selection by the COR. The PL is Wilsonart Amber Cherry.

66. Question: The specifications includes Section 09 84 33, Sound-Absorbing Wall Units. However, we can’t find where on the drawings this product is required. In section 09 84 33, paragraph 1.2(B) states, “Color and location for hanging: Section 09 06 00, Schedule for Finishes.” But, there is nothing stated in that section for this product. Again, further in section 09 84 33, paragraph 2.1(B)(2), states, “As indicated on construction documents.”, but nothing is shown on the drawings. The closest reference is in Section 09 06 00, page 4, Nurses Station Staff Side, which calls for Tackable Wall Covering. Is this the fabric covered acoustical wall panel product called out in section 09 84 33? It doesn’t appear to be, as there is a Section 10 11 23, Tackboards, which would be more applicable.
