About this file

This is a Past Performance Client Questionnaire template for a Multiple Award Task Order Contract (MATOC) 36C25227R0001 issued by the Department of Veterans Affairs Network Contracting Office 12 (NCO 12), located in Milwaukee, Wisconsin. The questionnaire is designed to be completed by past clients of contractors submitting proposals under this solicitation and should be returned via email to Ricky Bond (ricky.bond@va.gov) and Contracting Officer Kristi Kluck (kristi.kluck3@va.gov), rather than directly to the contractor. Respondent names will be kept confidential in accordance with Federal Acquisition Regulation 15.306(e)(4).

The questionnaire requires clients to evaluate contractor performance across seven major categories: Quality (technical data preparation, quality standards, problem resolution, and quality control), Schedule/Timeliness of Performance (delivery compliance and resource utilization), Customer Satisfaction (end-user satisfaction, contractor cooperation, and overall satisfaction), Management/Personnel/Labor (on-site management, workforce qualification, small business utilization, and change management), Cost/Financial Management (price compliance, cost-saving innovations, and invoice accuracy for cost-type contracts), Safety/Security (safety plan adherence and security compliance), and General (emergency response capabilities and contractual compliance). Evaluators must assign ratings of Exceptional, Good, Satisfactory, Marginal, Unsatisfactory, or Neutral/Not Applicable for each criterion and provide narrative comments addressing contractor strengths, weaknesses, and performance risk factors. Only one response per client office is required, though separate forms should be completed if the contractor holds multiple contracts with the client agency within the past three years.
