About this file

This is Amendment 0004 to Solicitation 36C25227R0001, issued November 5, 2026, by the Department of Veterans Affairs Great Lakes Acquisition Center for a Multiple Award Task Order Contract (MATOC) with a seed project. The amendment extends the proposal deadline from October 21, 2026, to November 23, 2026, at 10:00 a.m. CST and incorporates responses to 26 Request for Information (RFI) submissions. The solicitation is for general construction services (NAICS 236220) at VA Illiana Health Care System in Danville, Illinois, specifically for the seed project (Project 550-25-108) to renovate bathrooms in Building 103 PRRTP. The base MATOC scope includes general construction and incidental work such as carpentry, roofing, mechanical, electrical, plumbing, HVAC, demolition, masonry, fire protection, and telecommunications. The seed project involves phased bathroom renovations across four phases, with an anticipated start 10-30 calendar days after MATOC award and a project period of performance of 365 calendar days after Notice to Proceed. Construction hours are Monday-Friday, 7:30 a.m. to 4:00 p.m. CST, with no after-hours work permitted. The contract is tax-exempt and must comply with Buy American Act requirements.

Key personnel requirements include a full-time Superintendent and Site Safety and Health Officer (SSHO) directly employed by the prime contractor and present on-site at all times during work, with allowance for one qualified individual to serve dual or triple roles if meeting all qualifications. Past performance questionnaires from clients must be submitted directly to the contracting officer for projects completed within the last three years; CPARS ratings will be independently retrieved by the government. Proposals are limited to 25 pages for past performance and must include Volume 1 (technical information, past performance, resumes, safety documentation, financial information, and surety information) and other required attachments. The amendment clarifies that financial disclosure information is no longer required, provides the Contractor Surety Limitations form, addresses hazardous material abatement scope (lead-containing materials and asbestos-containing mud fittings identified on drawings HA101), and confirms that the VA will provide asbestos surveys and will be responsible for salvaging existing washers and dryers. Awards are intended within 120 calendar days of submission. The proposal shall be submitted as a lump-sum cost proposal with no divisional breakdown required.

I. Vendor questions:

RFI # 1.0 Not applicable.

RFI # 2.0 Not applicable.

RFI # 3.0

3.1Q (9/22/26)s: In lieu of new PPQs, would CPARS and/or previously filled out PPQs from other Solicitations be sufficient for our past performance, with key project information accompanied in the proposal? The challenge we run into is clients (Especially prior VA clients) have already been asked to provide a PPQ or CPARS for a given project, and they don't want to be asked to continue to write a review for a project 3 or 4 times. There are also occasions a COR for a completed job is no longer working at the facility and are unreachable. Contractors should not have to obtain further proof of client satisfaction if we already have it documented using authorized, VA-specific methods (CPARS and old PPQs).

3.1A: Contractors may use whatever methods they deem appropriate and best related to the overall MATOC scope of work. As noted on the Attachment 4 instructions, the evaluations must be sent directly from the customer to the VA rather than through correspondence from the prime contractor. Any customer evaluations included in contractor proposals will NOT be evaluated. Contractors shall NOT include CPARS as the government will independently retrieve them from the system.

3.2Q: How many Past Performance examples are needed? The Proposal instructions do not indicate a quantity.

3.2A: Contractors are responsible to determine how many examples they provide; this information is included in the by page count restrictions.

3.3Q: Is Relevant Past Performance limited to interior renovations in medical environments, or does the VA want to see a variety of medical environment construction across multiple disciplines and trades? The primary purpose/work to be ordered under this contract is general construction (NAICS 236220), but may include work incidental to the overall construction work, such as carpentry; asphalt and concrete paving; roofing; excavation; interior renovation; carpet, window, door installation; mechanical; electrical; plumbing; painting and stucco; demolition; masonry; fire protection; telecommunications; HVAC; and other construction-related work.

3.3A The Base MATOC Scope of Work includes a multitude of tradework that may not be included in the Seed Project. Contractors should demonstrate qualifications and experience to provide for the overall MATOC Base contract requirements.

3.4Q: The Financial Disclosure attachment and Surety Limitations attachment are not present on SAM but are asked to be included in our proposal. Please provide these attachments.

3.4A: The Financial Disclosure is no longer required. The Surety Limitations attachment is attached to this amendment.

3.5Q: We understand the VA has 120 day acceptance period - that being said, does the VA have an anticipated timeline awards will be made after the closing date?

3.5A: Awards are intended to be awarded no later than 120 calendar days after submission.

3.6Q: Is this MATOC contract & Seed Project Tax Exempt?

3.6A: Contractors shall comply with solicitation provisions and clauses. Tax exemption details are provided in the solicitation.

3.7Q: Seed Project:

i. Does the Superintendent need to be directly employed by the Prime Contractor?

ii. Must the Superintendent be on site at all times while work is commencing?

iii. Does the SSHO (Site Safety Health Officer) need to be directly employed by the Prime Contractor?

iv. Must the SSHO be on site at all times while work is commencing?

v. Can the Superintendent & SSHO be a dual-hatted individual assuming that individual is qualified for both roles?

vi. Are there any other mandatory on-site Prime-held roles required in this contract?

vii. When does the VA expect the seed project to start following the awards?

3.7A: Seed Project:

i. Yes

ii. Yes

iii. Yes

iv. Yes

v. Yes

vi. None other than the Superintendent & SSHO.

vii. VA expects kickoff/NTP for seed project 10 – 30 calendar days after MATOC awards.

4.1Q: I am reviewing the above opportunity on Sam.gov and just based on what I see will be very interested in pursuing. However, the only attachment/document is the Combined Synopsis/Solicitation Notice and it does not define what we are supposed to bid.

4.1A: What you referred to is the pre-solicitation notice. The full solicitation with all details was posted to https://sam.gov/workspace/contract/opp/d5d9fe0c6d9c4d5ea29bc6f28f14de11/view on 9/21/2026 and included the following items:

5.1Q: Apologies if I am missing something, but there seems to be attachments missing on SAM. The final page references the list of attachments that I do not see on SAM.

5.1A: Refer to RFI #4 above. Click the following link and scroll down to bottom until you see all the attachment links https://sam.gov/workspace/contract/opp/d5d9fe0c6d9c4d5ea29bc6f28f14de11/view

RFI # 6.0

6.1Q: We have a question regarding the technical evaluation requirements. For purposes of the MATOC technical evaluation, may an SDVOSB prime offeror satisfy relevant corporate experience, past performance, and key personnel requirements through the demonstrated experience, past performance, and personnel of a proposed first-tier construction subcontractor/team member? If so, please clarify which evaluation factors may be satisfied through the proposed subcontractor/team member.

6.1A: The government will evaluate the proposals as a whole. Per contract requirements, per task order, the SDVOSB prime contractor will not pay more than 85% of the amount paid by the government to the prime for contract performance, excluding the cost of materials, to firms that are not certified SDVOSBs listed in the SBA certification database

RFI # 7.0

7.1Q: Scranton Products submitted an equal product substitution request in accordance with FAR 52.211-6 Brand Name or Equal. Scranton Products states that 1. Proposed substitution has been fully investigated and determined to be equal or superior in all respects to the specified product, 2. Same warranty will be furnished for proposed substitution as for specified product, 3. Same maintenance service and source of replacement parts, as applicable, is available, 4. Proposed substitution will have no adverse effect on other trades and will not affect or delay progress schedule, 5. Proposed substitution does not affect dimensions and functional clearances.

7.1A: The substitution request for this product is approved.

RFI # 8.0

8.1Q: are we to acknowledge this on our sf1442 as amendment 1? (please clarify since there was no SF30 form posted with it)

8.1A: Yes

8.2Q: Please verify that the bid date has changed from October 21st to October 20th per the new sf1442 solicitation document.

8.2A: The solicitation closing date has changed in accordance with this amendment.

8.3Q: per the instructions listed for volume 1, part 1 – it lists financial disclosure information & contractor surety limitations as attachments 3 & 4 but those seem to be missing?

8.3A: The Financial Disclosure is no longer required. The Surety Limitations attachment is attached to this amendment.

RFI # 9.0

9.1Q: Does the VA have access to the relevant data through CPARS?

9.1A: Depending on the size of a Project it may not be in CPARS. And yes, we also check out CPARS. We regularly have been using the form and have been comments from federal contacts.

RFI # 10.0

10.1Q: Can the applicable CPARS evaluation be used in lieu of the past performance client questionnaire, and if so, can contractor include a copy of the CPARS evaluation in either volume 1, part 2 – past performance; or should contractor identify the contract/project and allow the va to retrieve the CPARS evaluation directly? Please also confirm that no separate attachment 6 questionnaire would be required for that contract if CPARS is used.

10.1A: projects may not be listed in CPARS depending on the size of a project. it is the contractor’s responsibility to demonstrate capabilities. if the contractor chooses to demonstrate past performance outside of the federal government, or not listed in CPARS, they should have clients return questionnaires directly to the VA.

RFI # 11.0

We are requesting clarification regarding the required documentation for Tabs 6 and 7 of the Technical Proposal Index:

11.1Q: Tab 6 – Financial Disclosure Information: Please clarify and identify the applicable solicitation attachment/document required for Tab 6, Financial Disclosure Information.

11.1A: The Financial Disclosure Information form is no longer required.

11.2Q: Tab 7 – Contractor Surety Limitations: The Technical Proposal Index identifies the Tab 7 Contractor Surety Limitations document as Solicitation Attachment 4; however, the posted Attachment 4 is the Past Performance Client Questionnaire. Please clarify and provide or identify the correct attachment/document required for Tab 7.

11.2A: The Contractor Surety Limitations form is attached to this amendment under attachments.

RFI # 12.0

12.1Q: Hazardous-material abatement quantities Drawing HA101 quantifies approximately 9,680 square feet of lead-containing ceramic wall tile under General Note E and identifies 130 asbestos-containing mud fittings under General Note D. General Note G also directs the contractor to assume lead paint is present on exposed plaster walls and painted surfaces. The specifications further require a destructive NESHAP inspection and address potentially concealed asbestos-containing materials.

Please provide bid quantities for the following work so all offerors can price the same abatement scope:

a. Lead-containing paint or coatings requiring stabilization, removal, or disposal, stated in square feet or another measurable unit.

Guidon response: Lead-containing paint shall be assumed present on existing exposed plaster walls and painted surfaces affected by demolition, as noted on HA101, General Note G. Include required lead-related controls, handling, and disposal for materials disturbed by the demolition work indicated in the Contract Documents. Bidders shall determine affected quantities from the indicated demolition limits. Full removal of lead-containing paint from surfaces remaining undisturbed is not required. Section 02 83 33.13, paragraph 3.4.A, shall not be interpreted as requiring blanket paint removal or stripping of components scheduled for demolition to expose the substrate before disposal.

b. Asbestos-containing pipe insulation, fittings, or other materials requiring abatement beyond the 130 mud fittings identified on HA101.

Guidon response: The contractor is to provide removal of the quantities listed on the drawings. See response below about variations.

c. Any concealed asbestos-containing materials that the Government intends bidders to include in the base proposal.

Guidon Response: See response below.

d. Please also confirm that hazardous material quantities exceeding the stated bid quantities, or previously unidentified materials encountered after demolition begins, will be addressed as a contract change.

Guidon response: The contractor is to provide removal of the quantities listed on the drawings. Specification sections 02 82 11.1.2.A and 02 82 13.13.1.2.A also notes a 10% variation from the drawings shall not change the contract price. Also, refer to specifications section 02 82 11.1.1.B.1 and 02 82 13.13.1.1.B.

12.2Q: Concealed metal-stud replacement Demolition Note 8 on AD102 requires removal of the wall finish and backer board to expose the existing steel studs and states that the COR will direct the extent of removal of damaged or rusted studs. Architectural Plan Note 2 on AE102 similarly requires installation of new steel studs as directed following the COR’s inspection. Because the condition and quantity of concealed studs cannot be established before demolition, please provide a unit-price quantity for removal and replacement of damaged or rusted steel studs. Alternatively, please confirm that bidders should exclude concealed stud replacement from the base proposal and that COR-directed replacement will be handled as a contract change. Please identify the required stud size, gauge, coating, and unit of measurement to be used for pricing.

Guidon response: Refer to sheet AE002. Sizing and general stud makeup to match existing.

12.3Q: Existing systems and owner-furnished items Sheets ED100 and EP100 show removal, relocation/reinstallation, and replacement of nurse-call devices within the bathroom renovation areas. EP100 requires new devices to be connected to the existing nurse-call system. Sheet E002 identifies Rauland as the basis of design but directs the contractor to coordinate the desired manufacturer with the Owner.

Sheet FA100 shows new wall-mounted strobes within the renovated bathrooms and requires the devices to be compatible and synchronized with the existing fire-alarm system.

For bidding purposes, please provide:

a. The manufacturer and model or series of the existing nurse-call system and fire-alarm system.

VA Response: Fire Alarm system Manufacturer is Simplex, however there are no existing fire alarm devices in the restrooms. The building's existing main fire alarm control panel is a Simplex 4100. The manufacturer and model/system of the existing nurse-call system are as follows: B103 Ward 2 is Dukane (Non-Operational). B103 Ward 3 is Rauland Responder 3. B103 Ward 4 is Rauland Responder 4.

b. The manufacturer and model or series required for the new bathroom nurse-call devices and fire-alarm strobes.

Charlotte Engineering Response (10/16/2026): The design intent is to match the existing systems.

VA Response: The manufacturer and model/system of the existing nurse-call system are as follows: B103 Ward 2 is Dukane (Non-Operational). B103 Ward 3 is Rauland Responder 3. B103 Ward 4 is Rauland Responder 4. Plan to reuse all existing nurse-call devices. Our nurse call in B103 is super antiquated and does not have vendor support. Therefore, removal, storage, and reinstallation of existing nurse call devices will have to be done extremely carefully. If anything goes amiss and things are damaged, there are no parts or vendor support, and we would not have nurse call in the RRTP restrooms.

There is a current project underway that is upgrading the fire alarm system in B103 and adding new fire alarm devices in the specific bathrooms for this project. Therefore, the new devices and installation planned in the scope of project 550-25-108 – Renovate B103 PRRTP Bathrooms can be removed. Please plan to reuse all existing fire alarm devices and wiring to be installed as a part of the new fire alarm project.

Manufacturer and Model of the fire alarm devices are as follows:

WHEELOCK ELSPSTWC - HIGH FIDELITY LED SPEAKER/STROBE, CONVENTIONAL, CEILING MOUNT, MULTI CANDELA, CLEAR LENS, WHITE, FIRE LETTERING

WHEELOCK ELSPKWC - HIGH FIDELITY SPEAKER, CONVENTIONAL, CEILING MOUNT, WHITE, FIRE LETTERING

c. Whether connection, programming, testing, and certification must be performed by a specific VA-approved or factory-authorized vendor.

VA Response: There is not a specific VA-approved and factory-authorized vendor. However, our fire alarm system programming, testing, and certification is typically performed by Johnson Controls. Regarding the nurse-call system, Primary Systems is the only authorized vendor to perform maintenance and programming for the Rauland system in our area.

d. Contact information for any required vendor so all bidders can obtain comparable quotations.

VA Response: Contant information for Johnson Controls and Primary Systems listed below:

Johnson Controls, Phone: 317-826-2130

Primary Systems, Paul Cross, RN Healthcare Account Executive Office 314-880-9977, Mobile 563-271-8757 Email: pcross@primary-systems.com

e. Confirmation that the nurse-call and fire-alarm scope is limited to the devices and associated work shown within the bathroom renovation areas, including required connections and programming to the existing systems.

Charlotte Engineering Response (10/16/2026): Confirmed.

RFI 13.

We are requesting clarification regarding the door requirements for Project 550-25-108 – Renovate B103 PRRTP Bathrooms at the VA Illiana Health Care System.

The door schedule appears to identify the applicable doors as hollow metal; however, the scheduled finish is noted as stained. The specifications also include Section 08 14 00, Interior Wood Doors, which provides requirements for transparent-finished wood doors, including the use of stain where required to achieve the specified finish. Section 08 11 13, Hollow Metal Doors and Frames, also references hollow-metal frames for wood doors at interior locations.

13.1Q: Please clarify whether the doors shown on the door schedule are intended to be hollow metal doors or stained wood doors.

Guidon Response (10/20/2026) - Doors to be HM and painted. Omit Wood door spec.

RFI 14. Not applicable.

RFI 15.

We request written clarification of the controlling proposal deadline and amendment-acknowledgment requirements for Solicitation 36C25227R0001.

15.1Q: The notice labeled 0001 lists October 22, 2026, at 4:00 p.m. Central Time as the response deadline. The SF 1442 in the solicitation document provided to us lists October 21, 2026, at 4:00 p.m. CDT in Block 13. Please confirm which date and time controls and, if the deadline has changed, identify the revised solicitation document or amendment that establishes it?

VA Response: 15.1A: The deadline has been revised in accordance with this amendment.

15.2Q: Please confirm which amendments have been formally issued, including their respective numbers and dates, as no SF 30 forms have been provided. Additionally, please clarify the required method for acknowledging Amendments 0001 and 0002. Should these be acknowledged in Block 19 of the SF 1442, through separate amendment acknowledgment forms, or by another method?

VA Response: 15.2A: All amendments such as 0001, 0002, 0003, and 0004 should be acknowledged in Block 19 of the SF 1442.

RFI 16.

16.1Q: Ref: SOW

a. The project/SOW title includes “B103 PRRTP & B64/72,” but the supplied drawings and specifications indicate Building 103 work only. Please confirm whether any work in Buildings 64 and/or 72 is part of this contract and provide the applicable drawings/specifications.

VA Response: Scope of work is Building 103 only. No work is required in Building 64/72.

16.2Q: Ref: 09 91 00 Paint.

a. Section 09 91 00 PAINTING is repeatedly referenced by the drawings and specifications, but Section 09 91 00 is not included in the supplied specification package. Please provide the governing painting specification and confirm products, surface preparation, primers, finish coats, and sheens for P-1, P-2, and P-3.

Guidon Response (10/20/2026) – Specification provided.

16.3Q: Ref: AE102 Note 2; AD102 Note 8

a. AE102 Note 2 directs installation of new steel studs “as directed by COR inspection” after finishes are removed, but no bid quantity is defined. Please provide a bidding allowance, unit-price basis, or required assumed quantity for damaged/rusted existing stud replacement.

Guidon Response: For bidding, assume up to 2 feet of stud replacement AFF at all existing walls, perimeter and interior of room.

16.4Q: Ref: AE600; Sections 08 11 13, 08 14 00, and 08 71 00

a. AE600 lists the two scheduled door assemblies as HM door material with “STAIN” finish, while General Note B states hollow metal doors/frames are factory primed and field painted. The specification package also includes Section 08 14 00 Interior Wood Doors although the TOC omits it. Please confirm door leaf material, finish and existing keyway.

Guidon Response (10/20/2026): Doors and frames to be HM. Omit wood door spec.
