About this file

This file is a Past Performance Client Questionnaire template designed for the Department of Veterans Affairs Network Contracting Office 12 (NCO 12) to evaluate contractor performance under Multiple Award Task Order Contract (MATOC) 36C25227R0001. The questionnaire serves as a standardized evaluation instrument that clients and references complete to assess contractors' past performance across multiple dimensions. Completed questionnaires must be submitted via email to ricky.bond@va.gov and kristi.kluck3@va.gov, with responses kept confidential per FAR 15.306(e)(4), ensuring respondent names are not disclosed to offerors.

The questionnaire evaluates contractor performance across seven major categories: Quality (technical data preparation, quality standards compliance, problem resolution, and quality control); Schedule/Timeliness (delivery schedules and resource utilization); Customer Satisfaction (end-user satisfaction, contractor cooperation, and overall satisfaction); Management/Personnel/Labor (on-site management, workforce qualification, government property control, personnel expertise, small business utilization, multi-project management, and requirement changes); Cost/Financial Management (price compliance, innovative cost-reduction methods, invoice accuracy for cost-type contracts, and financial stability); Safety/Security (safety plan adherence and security compliance); and General (emergency response capability, contractual compliance, and overall hiring willingness). For each criterion, respondents assign adjectival ratings—Exceptional, Good, Satisfactory, Marginal, Unsatisfactory, or Neutral/Not Applicable—with detailed definitions provided for each rating level. The template requires respondent and contractor identification information and concludes with a narrative section requesting specific details on strengths, weaknesses, and performance risk factors relevant to contract evaluation.
