James Gooden, AIA, LEED AP BD+C, NCARB, CSI Sr. Project Manager + Architectural Team Leader Guidon Design

Mr. Gooden:

Enclosed, please find the calculations for the amount of shielding required in the Redesign of the Nuclear Medicine Suite at the Indianapolis Roudebush Veterans Administration Medical Center in Indianapolis, Indiana. This report covers both the SPECT/CT as well as PET/CT rooms in the department. Installing the specified required shielding will reduce the exposure to less than the most restrictive recommended levels, i.e. 2 mrem per week (100 mrem per year) to the general public, and 10 mrem per week (500 mrem per year) for radiation monitored employees.

When practical, shielding has been selected to further reduce the radiation exposures to individuals to a level that is As Low As Reasonably Achievable (ALARA). A copy of the architectural drawings of the department and a copy of this report with calculations should be kept on file for review by regulatory auditors.

Occupancy factors for all areas were determined in accordance with National Council on Radiation Protection and Measurements (NCRP) Report Number 147, Table 4.1 Suggested Occupancy Factors.

The following values were used in the shielding calculations and were supplied by your facility. If you do not agree with these factors and assumptions and find them insufficient, please contact me at once, as my calculations may not be valid. A narrative description of the shielding requirements and recommendations follows.

PET Patient Estimates:

➢ Number of PET patients per day: 20

➢ Number of imaging days per week: 5

➢ Injected dose: 12 millicuries F-18 FDG

➢ Uptake time: 60 minutes

➢ Imaging time: 30 minutes

➢ Imaging takes place in rooms A-B004 and A-B006

➢ Uptakes take place in rooms A-B002, A-B003, A-B012, A-B010, A-B009, and A-

B008 o Each uptake room holds 25 injected patients per week

Roudebush VAMC September 17, 2020

CT Workload of PET Scanner:

➢ 40 head/neck scans per week with a DLP of 1200 mGy*cm

➢ 120 body scans per week with a DLP of 550 mGy*cm

➢ 40% of patients receive contrast and are scanned twice

➢ The CT scanner will be used for attenuation correction of PET scans as well as overflow of diagnostic CT scans.

SPECT/CT Workload:

➢ Number of cardiac scans per week: 40 (20 stress and 20 rest)

➢ Cardiac dosages: 36 mCi for stress, 12 mCi for rest

➢ Number of bone scans per week: 10

➢ Bone dosage: 35 mCi

➢ Cardiac DLP of 1200 mGy*cm

➢ Bone DLP of 1200 mGy*cm

➢ Imaging takes place in rooms A-B029a and A-B029b

General Comments:

➢ Walls are to be constructed with leaded (Pb) drywall of specified thickness with the lead (Pb) extending from the floor to a height of at least seven feet. Shielding must be lapped along corners and any pull-out sections. The screws/nails do NOT need to be capped with lead (Pb).

➢ The patient viewing window and windowsill, door and jamb are to be lined with the same thickness of lead (Pb) as the wall that they penetrate, unless specified otherwise.

Be sure that the leaded frames overlap the lead (Pb) in the gypsum drywall.

➢ All electrical outlets, switches, and other penetrations of all shielded walls are to be backed with the same thickness of lead (Pb) as the wall that they penetrate.

➢ Shielding in the Hot Lab is to be accomplished through localized source shielding, rather than general room shielding.

➢ The following calculations are based on NCRP Report 147 and the report of AAPM Task Group 108.

Architectural Information

1. Diagrams of the scan and injection/uptake rooms, along with the adjacent areas, are enclosed.

2. The Nuclear Medicine department is located on the basement floor. There is not an occupied floor below the department. There is an occupied floor above the department. Occupancy of the adjacent areas as assumed in my calculations is stated in the following section.

3. Any change in the occupancy of surrounding areas will require the shielding requirements to be re-evaluated.

4. This is a redesign of the department. There is currently installed shielding in several of the walls. If the currently installed shielding meets or exceeds what I am recommending, no additional shielding need be installed in the specified wall.

5. The floor to ceiling height is 13 feet, 8 inches.

6. There is 9 inches of standard density concrete in the ceiling.
