A.2 STATEMENT OF WORK

STATEMENT OF WORK

Expand First Floor B-1 for PACT VA Medical Center – Ft. Wayne, IN Project: 610-311

Project Background:

The project is the first-floor addition to Building 1 at the VA Northern Indiana Healthcare System in Fort Wayne, IN. There will be some renovation in the existing part of the hospital to add restrooms, a corridor leading to the new addition and adding doors to existing exam rooms on the south wall located in the south wing that open into the new addition.

Statement of Work:

The Contractor shall provide all labor, materials, and equipment required to Expand First Floor B-1 for PACT at the Ft. Wayne-VAMC to include, but not limited to the following:

1. Site Work to include: Demolition of existing vegetation, asphaltic parking and roadway including curbs, sidewalks, site lighting, and various underground utilities. The creation of a new patient parking lot supported with modular retaining wall blocks, to support new fill for the new paved parking lot. Installation of new under pavement storm water storage chamber. New and relocated site lighting, numerous lighted bollards, and landscaping. New patient covered vehicle ‘drop-off’ area along with covered exterior sidewalks and ramp.

2. Building Work to include: Demolition of existing building components (parapet, roof membrane, roof insulation) to allow the construction of a new poured in place pan and joist-floor (a new roof structure for the First Floor). Shoring of the new poured in-place forms and concrete will not be allowed to be installed within the Basement Floor below and must be contained within this new First Floor addition. The new façade is metal stud infill between the new poured concrete structural frame and has brick and metal exterior finishes and a larger number of aluminum store front windows. The construction of new partitions, doors, frames, hardware, new ceilings, electrical, HVAC and plumbing. Installation of Physical Access Control System (PACS). A new data/communication server room and installation of fiber optic cables within the building. Installation and integration of new electrical, plumbing and HVAC systems with existing systems and the relocation of a(n) air cooled chiller, heat recovery fan and smaller exhaust fans is also a part of this project. Some work will be required in the Basement Floor below the First Floor to incorporate new plumbing, electrical and HVAC

Page of 2 and careful coordination will be required since the Basement Floor contains a new Sterilization Department and Clean Supplies.

3. To connect the new first floor expansion to the existing Hospital, there is also a substantial amount of work to all of the adjacent Existing Hospital Spaces. As a working Hospital the adjacent spaces and functions will necessarily need to be maintained and a high degree of coordination, scheduling and phasing will be required to allow the continuation of the existing Hospital Services during the entire Construction Project.

4. The Contractor will only be afforded access to the VAMC from the exterior ‘work side’ and no access will be afforded the Contractor in or through VAMC spaces.

Justification for this Project:

This Project is required in order to provide to the VAMC the space and facilities to provide updated Primary Care Services to the Veteran Population that is serves in this community. The additional Square Footage is needed to provide the space needed to provide the enhanced VA Model of PACT and to the growing Veteran Population.

REVIEW REQUIRED REGISTRATION WITH CONTRACTOR PERFORMANCE ASSESSMENT SYSTEM (CPARS)

1. As prescribed in Federal Acquisition Regulation (FAR) Part 42.15, the Department of Veterans Affairs (VA) evaluates contractor past performance on all construction contracts that exceed $700,000 and shares those evaluations with other Federal Government contract specialists and procurement officials. The FAR requires that the contractor be provided an opportunity to comment on past performance evaluations prior to each report closing. To fulfill this requirement VA uses an online database, CPARS, which is maintained by the Naval Seal Logistics Center in Portsmouth, Virginia. CPARS has connectivity with the Past Performance Information Retrieval System (PPIRS) database, which is available to all Federal agencies. PPIRS is the system used to collect and retrieve performance assessment reports used in source selection determinations and completed CPARS report cards transferred to PPIRS. CPARS also includes access to the federal awardee performance and integrity information system (FAPIIS). FAPIIS is a web-enabled application accessed via CPARS for contractor responsibility determination information.

1. Each contractor whose contract award is estimated to exceed $150,000 is required to register with CPARS database at the following web address: www.cpars.csd.disa.mil. Help in registering can be obtained by contacting Customer Support Desk at DSN: 684-1690 or COMM: 207-438-1690. Registration should occur no later than thirty days after contract award and must be kept current should there be any change to the contractor's registered representative.

1. For contracts with a period of one year or less, the contracting officer will perform a single evaluation when the contract is complete. For contracts exceeding one year, the contracting officer will evaluate the contractor's performance annually. Interim reports will be filed each year until the last year of the contract, when the final report will be completed. The report shall be assigned in CPARS to the contractor's designated representative for comment. The contractor representative will have thirty days to submit any comments and re-assign the report to the VA contracting officer.

Failure to have a current registration in the CPARS database, or to re-assign the report to the VA contracting officer within those thirty days, will result in the Government's evaluation being placed on file in the database with a statement that the contractor failed to respond.”
