Amendment No. 5 Department of Veterans Affairs (DVA)

Grand Junction, CO – Primary Care PACT Clinic VA Request for Lease Proposals (RLP)

No.36C24W25R0055

The purpose of this Amendment is to provide responses to the following requests for information:

Question 1: Section 1.02. A: States min and max ABOA. In a single-tenant building where the VA is the sole occupant utilizing 100% of the space, industry standards (e.g., ANSI/BOMA Z65.1-2017) typically indicate that Rentable Square Footage (RSF) is equal to ANSI/BOMA Office Area (ABOA, or Usable Square Footage) since there are no shared common areas. Please confirm the VA aligns with this interpretation, where RSF and ABOA are equivalent in such a scenario. If the VA’s standards differ, please specify any room categories or areas that must be excluded from RSF to calculate ABOA according to VA measurement standards.

Answer 1: It’s not unusual for a single tenant building to have the same square footage for both the ABOA and RSF, but the final measurements will be determined concurrent with the acceptance of the Space. Para 4.11 of the Lease template (Exhibit A of the RLP) in part states, “At acceptance, the Space shall be measured in accordance with the standards set forth in this Lease to determine the total ABOA SF in the Space. The rent for the Space will be adjusted based upon the measured ABOA square footage as outlined under the Payment clause of the General Clauses.”

Question 2: Section 1.06 A: Provides a list of exhibits and their respective page numbers. Exhibit B.3 is shown to include 47 pages however the exhibit B.3 provided in the RLP package only contains 46 pages.

Please confirm exhibit B.3 is intended to only be 46 pages.

Answer 2: 46 pages is correct.

Question 3: How many PPQ’s should be submitted to achieve the top technical score for an offeror?

Answer 3: The contents of the Past Performance Questionnaires is more important than the number of Questionnaires received.

Question 4: Would be it possible to be more specific with the projected Lease Award Date? We saw Fiscal Year 2026 in Amendment 3, but would it be possible to provide a projected month or maybe a projected quarter for the Lease Award?

Answer 4: The VA desires to make the award in the earlier part of FY2026, but is also cognizant of the uncertainty of issues which may result in an award in the latter part of FY2026.

Question 5: We would like to respectfully request a 3-week extension to submit the initial offer for this procurement.

Answer 5: The VA will extend the time for the submission of offers from 7/31/2025 to 12:01pm Mountain Time on 8/14/2025. The deadline for submitting questions is 7/31/2025.

Question 6: RLP 36C24W25R0055 – Section 2.03 and 2.04 regarding Seismic Safety have been intentionally deleted. Lease Section 1.07 shows “Seismic Form C, Building Retrofit Or new Construction Preaward Commitment” as a document incorporated into the Lease, but this document has not been provided and/or incorporated into Section 1.06 List of Documents in RLP 36C24W25R0055. Lease Section 3.46 states requirements for, “Seismic Safety for New Construction”. Can the Agency clarify if there are Seismic Requirements for RLP 36C24W25R0055.

Answer 6: Grand Junction is in a low seismic area. Therefore, the requirements for seismic in the RLP have been deleted. However, for leases requiring new construction, the space will not be considered substantially complete until the LCO receives the Seismic Form F, Certificate of Seismic Compliance – New Building. This form must be completed by the civil or structural engineer and certify that the building was designed and constructed in accordance with the appropriate local code. Seismic Form F is attached to this Amendment.

Question 7: Exhibit B.3 ASR – Design Specs and Cleaning Section 10.4 states, “Before working hours 6:30 AM to 5:00 PM remove snow and ice…”. Lease Section 6.01 – A states, “The Government’s normal hours of operation are established as 7:30 AM to 5 PM, Monday through Friday…”. Can the Agency clarify the correct hours of operation for the facility?

Answer 7: The hours of operation will be 7:30am to 5pm.

Question 8: Lease Section 4.01 has some amount of days filled in and others to be established for a working schedule. Page 1 of the Lease states, “ INSTRUCTIONS TO OFFEROR: Do not attempt to complete this lease (GSA Lease Template L100, hereinafter Lease Template). Upon selection for award, GSA will transcribe the successful Offeror's final offered rent and other price data included on Offeror’s submitted GSA Lease Proposal Form 1364, (hereinafter Lease Proposal Form) into a Lease Template, and transmit the completed Lease Template, together with appropriate attachments, to the successful Offeror for execution.” Can the agency clarify that expectations that Offerors should fill these dates out on GSA 1364 or is it the Agency’s expectation to perform within these timeframes?

Answer 8: The Offeror should fill out GSA Form 1364. The dates that have been pre-filled in para 4.01 of the Lease template (Exhibit A of the RLP) are indeed the Agency’s expectations to perform within these timeframes.

Attached – Seismic Form F

All other requirements remain unchanged.
