About this file

This is a Past Performance Questionnaire (PPQ) template used by federal agencies to evaluate contractor performance on previous government contracts. The form is divided into two primary sections: contractor-completed blocks (1-4) and client-completed blocks (5-8).

The contractor section requires submission of firm identification details (name, CAGE code, UEI number, contact information), work role specification (prime contractor, subcontractor, joint venture, or other), and comprehensive contract information including contract number, type (firm fixed price, cost reimbursement, or other), award and completion dates, original and final contract prices, and project descriptions with complexity ratings. The client section collects evaluator information and their assessment of the contractor's performance across seven detailed categories: quality (technical data preparation, quality standards compliance, problem resolution timeliness, and quality control program effectiveness); schedule/timeliness of performance (delivery schedule compliance and resource utilization); customer satisfaction (end-user satisfaction, contractor reasonableness and cooperativeness, and overall satisfaction); management/personnel/labor (on-site management effectiveness, workforce qualification and retention, government property control, personnel expertise, small business utilization, multi-project capability, requirement changes assimilation, and overall management); cost/financial management (price compliance, innovative alternatives, invoice accuracy for cost-type contracts, accounting system adequacy, and termination history); safety/security (safety environment maintenance and security compliance); and general performance (emergency response capability, contractual compliance, rehire likelihood, and overall performance rating). Performance is rated using a six-point scale: Exceptional (E), Very Good (VG), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Not Applicable (N), with detailed definitions provided for each rating level. The form includes provisions for narrative commentary addressing strengths, weaknesses, deficiencies, and additional remarks, with instructions noting that Virginia requests clients submit questionnaires directly to the offeror, who may duplicate them for future solicitations, though the government reserves verification rights.
