About this file

The document is a Performance Relevancy Survey template used to evaluate past performance for federal contract proposals. This comprehensive form requires offerors to provide detailed information about previous contracts, including project specifics such as contract value, completion dates, work description, and relevancy to the current solicitation. The survey is designed to help the government assess an offeror's capabilities by categorizing past performance as Very Relevant, Relevant, or Somewhat Relevant based on the similarity of scope, magnitude, and complexity to the current project.

The survey requires contractors to disclose contract details, subcontractor involvement, technical areas of expertise, and contact information for previous project references. Additionally, offerors must provide supplementary information about liquidated damages, problem resolution, professional awards, and unique skills within the past three years. The form emphasizes transparency and encourages offerors to proactively address any challenges encountered in previous contracts, allowing the government to comprehensively evaluate a contractor's performance history and potential for success on future projects.
