About this file
This document contains questions and responses related to a request for information (RFI) for a construction project to renovate and expand laboratory space at a Veterans Affairs medical center in Lebanon, Pennsylvania. The project includes renovating 4,750 square feet of an existing main laboratory and constructing a new 6,900 square foot addition, with interior work such as construction of floors, walls, ceilings, mechanical and electrical systems, and exterior work to build a single-story addition. Questions clarify responsibilities for furnishing and installing various laboratory casework, benches, and equipment. Responses indicate the VA will provide mobile lab benches but other fixed casework and sinks will be contractor responsibilities. Clarification is also provided on finishes, drawings, site utilities, and safety roles.

Phase 1 has this states that “Phase 1 has been completed as an inhouse project in the past.”
However, Drawing GI001 identifies Phase 1 as “New Mechanical Basement – including areaway” and “New Lab Addition.” This work has not been completed. Has Phase 1 been completed or not? Which Phases are complete, requiring no additional work, and which are not complete?
All phases are the responsibility of the contractor. The Phase I area is available immediately for construction. The Phase II and Phase III areas will be turned over to the construction contractor after the VA has occupied and activated the Phase I area. The only area of Phase II on sheet GI-001 that the construction contractor is responsible for are those areas shown on sheet GI-003
This states that Phase 1 has been completed, and work on Phases 2 and 3 can begin immediately.
However, this does not match the phasing in the drawings. Can you confirm that Phase 2 in the drawing phasing plan is complete and requires no additional work? Also, please confirm that Phase 1 and Phase 3 are not completed and are the responsibility of the Contractor.
All phases are the responsibility of the contractor. The Phase I area is available immediately for construction. The only area of Phase II on sheet GI-001 that the construction contractor is responsible for are those areas shown on sheet GI-003
This indicates Phase 2 is the phase that was recently abated. Can you confirm that there is no additional work here?
The only asbestos abatement required in the Phase 2 area is that work associated with penetrations of the existing wall between the Phase I and Phase 2 areas

May the SSHO be the site Superintendent? The VA responded “The Government will not dictate how the contractor manages or assigns their personnel to this project.” However, Specification section 01 35 26 1.7C states “the SSHO has to be a separate qualified individual from the Prime Contractor’s Superintendent and/or Quality Control Manager with duties only as the SSHO.”
Must the SSHO be a separate qualified individual in addition to the Prime Contractor’s Superintendent?
The SSHO may not be the Superintendent. However, in accordance with 01 35 26 1.7D, the SSHO is not required to be on site at all times, as long as another individual, such as the superintendent, is equally qualified as an OSHA competent person

I see in the addenda’s - RFI’s ( number 74. ) that the 50 each mobile lab benches are by the VA Hospital. Is this correct ?
Can you send me the RFP for these tables? Have they been purchased yet ?
Mobile lab benches will be government supplied and government installed.
This would be a supply order issued by Acquisitions and that we have FMS has no way of knowing when then these will be purchased at this time. Acquisitions have many different ways of obtaining supplies and a SDVOSB Contractor would provide these supplies.
If he is an SDVOSB Contractor and has his information is SAMS and certified, he can follow up with a supervisor is Acquisitions for more details.
In Amendment 3, RFI Questions & Answers, Responses #74 & #75: These responses state that the VA will furnish and install (50) mobile lab benches. The response does not address other equipment for the laboratory, which would appear to complement the benches. These items are cited in General Lab Note #3 and are to be provided by the Prime contractor.
Do the items in Note #3 -- such as integral sinks, epoxy resin counters, matching plumbing faucets – need to be acquired from the same mobile lab bench vendor? If yes, who is the vendor and its contact info?
Counter, matching plumbing faucets, and sinks are typically installed by the contractor. Mobile lab benches will be government supplied and government installed.
The vendor should be, that might be up to the contractor to deiced and when they submit their submittals for approval, COR will need to determine if they are appropriate.
