About this file

This document is a Local Assistance Procedures Manual Exhibit 15-H form for Proposer/Contractor Good Faith Efforts, specifically related to Disadvantaged Business Enterprise (DBE) participation in a federal-aid project. The form is designed for proposers or bidders to document their efforts to meet or exceed a predetermined DBE contract goal, which requires submission within five calendar days of the cost proposal due date or bid opening. The form provides a comprehensive template for contractors to demonstrate their good faith efforts in soliciting and including DBE firms in the project, with sections covering advertisement placements, written notices to certified DBEs, breakdown of work items, reasons for rejecting DBE firms, assistance efforts, and agency contacts.

The form requires detailed documentation of DBE participation strategies, including the specific percentage goal for DBE involvement (which is left blank in this template), publication of solicitations, direct outreach to certified DBE firms, breaking down contract work into economically feasible units, and providing assistance to DBEs in obtaining necessary resources like bonding, credit lines, insurance, and equipment. The purpose of this document is to protect the proposer's eligibility for contract award by showing proactive attempts to include DBE firms, even if the initial DBE commitment form indicates goal achievement. The form serves as a critical compliance mechanism to ensure fair and inclusive contracting practices in federally-aided projects.
