About this file
The document is a Disqualification Disclosure form required by California Public Contract Code (PCC) §10162, designed to be completed by prospective bidders under penalty of perjury. The form requires bidders to disclose whether the bidder, any officer, or employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.
The form provides a binary response option (Yes or No) with an instruction to explain and attach supporting documentation if the answer is affirmative. This disclosure serves as a mechanism for government agencies to assess the past performance, compliance history, and potential risks associated with bidders seeking to participate in public contracts. By mandating this self-reporting, the state aims to maintain transparency, accountability, and integrity in the procurement process.
