About this file
The document is a Debarment and Suspension Certification form for the City of Pasadena, conforming to Title 2 CFR, Part 180 "OMB Guidelines to Agencies on Government wide Debarment and Suspension (nonprocurement)". The two-page form is designed for both primary contractors and lower tier covered transactions (subcontractors) to certify their legal and ethical standing in government contracting. Each section requires the bidding entity to affirm that they are not currently suspended, debarred, or ineligible by any federal agency, have not been suspended or debarred within the past three years, do not have a proposed debarment pending, and have not been indicted or convicted of fraud or official misconduct.
The certification requires detailed disclosures about the bidding entity's legal status, with provisions for explaining any exceptions. While no specific project details are provided, the form emphasizes that exceptions will not automatically disqualify a bidder but will be considered in determining the bidder's responsibility. The form must be signed by an authorized representative of the primary contractor or subcontractor, with space for name, title, signature, and date. Exceptions must be disclosed to the City of Pasadena, and final determinations regarding the Federal Government Excluded Parties List System will be made by the relevant Federal Agency.
