About this file
The document is a Financial Document Review (FDR) Request Form issued by the California State Department of Transportation's Independent Office of Audits and Investigations (IOAI). This form is specifically designed for Local Public Agencies (LPAs) to submit financial documentation for new Architectural & Engineering (A&E) consultant contracts valued at $1 million or more, or for amendments involving additional subconsultants or changes in Indirect Cost Rate (ICR). The form requires detailed financial information for prime consultants and subconsultants, with a focus on contracts where subconsultants have estimated participation amounts of $500,000 or more.
The document outlines comprehensive requirements for financial document submission, categorizing documentation needs into four distinct categories based on the consultant's status (e.g., having a Cognizant Approval Letter, requesting Safe Harbor Rate, having a CPA Audited ICR Report, or participating with a contract amount of $500K or greater). For the period of July 1, 2024 through June 30, 2025, consultants must submit 2023 or newer Indirect Cost Rate (ICR) documents. The form includes a detailed checklist specifying required documents such as Certification of Indirect Costs, CPA Audited ICR Reports, ICR Schedules, Cognizant Approval Letters, and AASHTO Internal Control Questionnaire. Additional supporting documents may be requested, including prior year ICR schedules, related party rent analysis, income statements, and various policy documents.
