About this file
This is a Good Faith Effort (GFE) Documentation Form (BEH-FRM-607) issued by the City of Dallas, Department of Aviation, for use in connection with the Architectural and Engineering Services RFQ to establish a two-year short-list of pre-qualified consultants supporting aviation facility projects. The form is required when respondents fail to meet contract-specific Small Business Enterprise (SBE) goals or when specifically requested by the City for compliance review. Respondents must complete attestations confirming they evaluated commercially reasonable subcontracting opportunities without artificially consolidating scopes, verified SBE certifications in relevant NAICS codes, and ensured all SBE work involves a Commercially Useful Function (CUF).
The form requires respondents to document their identification and segmentation of subcontracting opportunities into economically feasible packages, demonstrate adequate outreach to SBEs with a minimum seven-business-day response window, and provide proof of contact through emails, call logs, bid solicitation requests, and other documentation. Respondents must list all SBEs contacted, include quote amounts and outcomes, explain rejections with justification including cost differential percentages, and describe assistance provided to SBEs such as bonding guidance, specification clarification, or work segmentation. A certification section requires the officer's attestation that information is accurate, SBEs were not artificially excluded, all participants meet certification requirements, and documentation is complete. GFE participation and commitments are subject to monitoring and compliance review throughout the contract term.
