About this file
This is an Engagement Score and Past Commitment Form (BEH-FRM-606) for the City of Dallas, Department of Aviation, designed to evaluate vendor qualifications for an Architectural and Engineering Services Request for Qualifications to establish a two-year short-list of pre-qualified consultants supporting aviation facility projects. The form functions as an evaluation document rather than a procurement request with specific deliverables or timelines. Respondents must demonstrate historical performance through documentation of three relevant projects completed within the last five years, detailing contract values, subcontracting program types, goals, and final utilization percentages achieved. The evaluation criteria encompass past subcontractor utilization (3 points), documented engagement efforts (2 points), and past timely payments to subcontractors (1 point), with point allocations intended to assess vendor commitment to small and local business development in the Dallas region.
The form requires respondents to provide verified documentation supporting their claims across multiple evaluation categories, including mentor-protégé program participation, active involvement with Dallas-based industry organizations supporting small business development, and evidence of consistent on-time payment practices through system-generated reports, subcontractor letters, compliance audits, or internal payment logs. Respondents must submit a certification statement signed by an authorized officer attesting to the accuracy and completeness of provided information, with acknowledgment that the City of Dallas may verify past performance claims and that submission of false or misleading information may result in response rejection or other enforcement actions. The form establishes a comprehensive evaluation framework prioritizing vendor engagement with certified subcontracting programs and demonstrated commitment to equitable contracting practices within the regional business community.
