About this file

This is a Past Performance Questionnaire (PPQ) template developed by the Veterans Health Administration (VHA) Program Contracting Activity Central for evaluating contractor performance on past projects relevant to federal contract opportunities. The form is completed by the contractor in Section 1, providing detailed information about the offeror, contract specifics, project description, pricing history, and scope of work performed. Section 2 is completed by the client or project evaluator, who assesses the contractor's performance across seven key criteria: quality of design and construction standards, schedule and timeliness of performance, communication and cooperation, management and personnel effectiveness, cost and financial management, subcontract management, and an overall summary rating on a four-tier adjectival scale (Outstanding, Above Average, Satisfactory, or Unacceptable).

The PPQ is designed to be incorporated into proposal submissions and counts toward the stated page limitations in the Instructions to Offerors. Evaluators must verify contractor information accuracy, provide detailed comments on performance against contractual requirements, and indicate whether they would recommend working with the firm again. The completed questionnaire must be returned to the offeror for inclusion in their proposal by the solicitation deadline; late submissions will not be accepted. This template is applicable to the Garland VA Medical Center elevator replacement project (Project No. 549-24-425, Solicitation No. 36C77626R0061) and other VHA construction and service contracting opportunities.
