About this file
This is an Asbestos Survey Report prepared for the proposed VA Hospital Garland project located at three addresses in Garland, Texas: 2300 Marie Curie Boulevard (main five-story hospital building, 167,236 sq. ft.), 700 Walter Reed Boulevard (four-story medical plaza), and 900 North Shiloh Road (single-story storage building, 19,881 sq. ft.). The survey was conducted June 10-19, 2019 by Terracon Consultants, Inc., a firm of Texas Department of State Health Services (TDSHS) licensed asbestos consultants and inspectors, in accordance with EPA regulation 40 CFR 763 (AHERA) and Texas Asbestos Health Protection Rules (TAHPR).
The survey identified numerous asbestos-containing materials (ACM) across all three buildings, with 298 homogeneous areas and 1,004 samples collected from the main hospital, 322 homogeneous areas and 1,073 samples from the plaza building, and 49 homogeneous areas and 169 samples from the storage building. Confirmed ACM includes friable fireproofing in the main building stairwells (gray, tan) and nonfriable materials such as floor tile and mastic, duct sealant, texture on concrete and CMU blocks, caulking, gaskets, wall coverings, and weatherproofing materials throughout all three structures. The report notes significant survey limitations due to locked rooms, occupied medical clinics, and inaccessible areas above suspended ceilings and beneath floor coverings, indicating that additional ACM may be present in these concealed spaces. The document recommends that identified interior ACM be abated prior to any renovation or demolition activities that would disturb these materials, and specifies that abatement must be performed by a State of Texas licensed contractor with third-party air monitoring conducted during removal activities.

ASBESTOS SURVEY REPORT
Proposed VA Hospital Garland – Three Buildings
2300 Marie Curie Boulevard
INTRODUCTION
Terracon Consultants, Inc. (Terracon) conducted an asbestos survey at the site of the proposed VA Hospital Garland located at 2300 Marie Curie Boulevard, 700 Walter Reed Boulevard, and 900 North Shiloh Road in Garland, Texas. The survey was conducted June 10-19, 2019 by a team of Texas Department of State Health Services (TDSHS) licensed asbestos inspectors and asbestos consultants in general accordance with Terracon Proposal No. P94197779 dated June 6, 2019.
Accessible interior and exterior building components were surveyed and homogeneous areas of suspect asbestos-containing materials (ACM) were visually identified and documented. Although reasonable effort was made to survey accessible suspect materials, additional suspect but unsampled materials could be located in walls, in voids or in other concealed areas. Suspect ACM samples were collected in general accordance with the sampling protocols outlined in Environmental Protection Agency (EPA) regulation 40 CFR 763, the Asbestos Hazard Emergency Response Act (AHERA).
1.1 Project Objective
The objective of the asbestos survey was to identify the presence and location of accessible friable and nonfriable ACM present on the interior and exterior of the five-story main hospital structure (2300 Marie Curie Boulevard), a four-story medical plaza (700 Walter Reed Boulevard), and a single-story storage building (900 North Shiloh Road) in Garland, Texas.
The Texas Asbestos Health Protection Rules (TAHPR) and EPA regulation 40 CFR 61, Subpart M, the National Emission Standards for Hazardous Air Pollutants (NESHAP) require that an asbestos survey be performed prior to renovation or demolition activities.

Responsive ■ Resourceful ■ Reliable 4
3.1 Visual Assessment
Our survey activities began with visual observation of the interior and exterior of each building to identify homogeneous areas (HA) of suspect ACM. A HA consists of building materials that appear similar throughout in terms of color, texture and date of application. The assessment was conducted throughout visually accessible areas of each building. Building materials identified as concrete, glass, wood, masonry, metal or rubber were not considered suspect ACM.
Terracon made reasonable efforts to assess suspect materials on the interior and exterior of each building; however, the following limitations were encountered at the time of the survey:
◼ Locked rooms throughout the interior of each building/availability of keys from CBRE, property management, CBRE did not have access to several key-pad doors, ◼ Expedited schedule/access limited to building security’s schedule, ◼ Vacant areas of each building used for storage of or littered with remnant furnishings, ◼ Operational mental health clinic on the fourth and fifth floors of the main building, and ◼ Multiple occupied medical clinics in the plaza building.
Due to these limitations, access to multiple areas in each building was limited and the potential exists for additional suspect ACM or quantities of identified ACM to be present in these inaccessible areas.
Terracon assessed multiple areas above the drywall and lay-in ceiling systems in each building, where possible, but did not observe additional suspect ACM except where noted in this report;
however; due to occupancy, building condition, and building usage at the time of the survey, access to areas above the ceiling was limited and the potential exists for additional suspect ACM or quantities of identified ACM to be present in these inaccessible areas.
Terracon lifted floor coverings in several areas in each building, where possible, but did not observe additional floor coverings/layers except where noted in this report; however, as Terracon could not assess beneath all floor coverings in all areas, there may be isolated areas of additional suspect ACM or quantities of identified ACM present beneath existing covering.
Terracon observed mirrors in the restrooms of each building that appeared to be mechanically fastened; however, the potential exists for suspect ACM mirror mastic to be located behind the mechanically fastened mirrors. Terracon attempted to assess the area behind the installed mirrors for suspect ACM; however, the presence or absence of suspect materials could not be verified without forcibly removing the installed mirrors, potentially causing excessive damage to the mirrors
Responsive ■ Resourceful ■ Reliable 5 and creating a safety hazard. If encountered, suspect ACM mirror mastic should be sampled prior to disturbance if it will be impacted by the renovation.
Terracon assessed multiple areas of the crawlspace, where possible, but did not observe additional suspect ACM materials except where noted in this report; however, due to the condition of the crawlspace, access was limited to the standing areas and the potential exists for additional suspect ACM materials or quantities of identified ACM to be present in these inaccessible areas.
3.2 Physical Assessment
A physical assessment of each homogeneous area of suspect ACM was conducted to assess the friability and condition of the materials. A friable material is defined by the EPA as a material which can be crumbled, pulverized or reduced to powder by hand pressure when dry. Friability was assessed by physically touching suspect materials.
3.3 Sample Collection
Based on results of the visual observation, bulk samples of suspect ACM were collected in general accordance with AHERA sampling protocols. Random samples of suspect materials were collected from each HA observed. Sample team members collected bulk samples using wet methods as applicable to reduce the potential for fiber release. Samples were placed in sealable containers and labeled with unique sample numbers using an indelible marker.
The following Table presents the number of HAs of suspect ACM and the number of samples collect from each building:
Building Homogenous Areas of
Suspect ACM

Main Hospital – 2300 Marie Curie Boulevard 298 1,004
Plaza Building – 700 Walter Reed Boulevard 322 1,073
Storage Building – 900 North Shiloh Road 49 169
A summary of suspect ACM samples collected during the survey is included in Appendix B.
Drawings depicting sample locations and ACM locations are provided in Appendix E.

3.4 Sample Analysis
Suspect ACM samples were delivered under proper chain of custody to Cates Laboratories of Dallas, Texas, a National Voluntary Laboratory Accreditation Program accredited (Lab No.
200569-0) and TDSHS licensed (License No. 30-0287) laboratory for analysis by Polarized Light Microscopy (PLM) with dispersion staining techniques per EPA Method 600/R-93/116. The percentage of asbestos, where applicable, was determined by microscopic visual estimation.
REGULATORY OVERVIEW
The State of Texas has established the TAHPR which requires any asbestos-related activity to be performed by an individual licensed by the State of Texas, through the TDSHS. An asbestos related activity consists of the disturbance (whether intentional or unintentional), removal, encapsulation, or enclosure of asbestos, including preparations or final clearance, the performance of asbestos surveys, the development of management plans and response actions, asbestos project design, the collection or analysis of asbestos samples, monitoring for airborne asbestos, bidding for a contract for any of these activities, or any other activity required to be licensed under TAHPR.
Abatement must be performed by a State of Texas licensed asbestos abatement contractor in accordance with a project design prepared by a State of Texas licensed asbestos consultant. In addition, third party air monitoring must be conducted during the abatement activities.
The asbestos NESHAP (40 CFR Part 61 Subpart M) regulates asbestos fiber emission and asbestos waste disposal practices. It also requires the identification and classification of existing building materials prior to demolition or renovation activity. Under NESHAP, asbestos containing building materials are classified as either friable, Category I nonfriable or Category II nonfriable ACM. Friable materials are those that, when dry, may be crumbled, pulverized or reduced to powder by hand pressure. Category I nonfriable ACM includes packing, gaskets, resilient floor coverings and asphalt roofing products containing more than 1% asbestos. Category II nonfriable ACM are any materials other than Category I materials that contain more than 1% asbestos.
Friable ACM, Category I and II nonfriable ACM in poor condition and has become friable or which will be subject to drilling, sanding, grinding, cutting, or abrading and which could be crushed or pulverized during anticipated renovation or demolition activities are considered regulated ACM (RACM). The TAHPR and NESHAP require that written notification be submitted before beginning renovation projects which include the disturbance of any asbestos-containing material (ACM) in a building or facility, or before the demolition of a building or facility, even when no asbestos is present. This written notification must be provided to the TDSHS at least 10 working days prior to the commencement of asbestos abatement or demolition activities. Removal of
Responsive ■ Resourceful ■ Reliable 7
RACM must be conducted by a State of Texas licensed asbestos contractor. In addition, third party air monitoring must be performed during the abatement.
The Occupational Safety and Health Administration (OSHA) Asbestos standard for the construction industry (29 CFR 1926.1101) regulates workplace exposure to asbestos. The OSHA standard requires employee exposure to airborne asbestos fibers be maintained below 0.1 asbestos fibers per cubic centimeter of air (0.1 f/cc).
The OSHA standard classifies construction and maintenance activities which could disturb ACM and specifies work practices and precautions which employers must follow when engaging in each class of regulated work.

The identified ACM gray fireproofing observed in the Main Building Stairwells was assessed to be friable but in good condition on the day of the survey.
Responsive ■ Resourceful ■ Reliable 9
The remaining identified ACM collected from the three structures were assessed to be nonfriable and in good condition on the day of the survey. Terracon understands that the building surveyed is scheduled for future renovation or demolition. Therefore, the identified interior ACM should be abated prior to activities that will disturb the identified ACM.
The following samples were determined by PLM visual estimation to contain <1% Chrysotile asbestos:
◼ Main Building-Basement, Samples 81-83 Texture on concrete ◼ Main Building-First Floor, Samples 171-173 Residual thinset ◼ Main Building-First Floor, Samples 260-262 Ceramic floor tile thinset ◼ Main Building-First Floor, Samples 263-265 Ceramic wall tile thinset
USEPA NESHAP regulations and the TAHPR require that materials determined by PLM analysis with results greater than zero but less than 10% asbestos be reanalyzed using the more quantitative point counting technique or that the material be considered ACM and subject to NESHAP regulations and the TAHPR. Terracon recommends Samples 81-83, 171-173, 260-262, and 263-265 be re-submitted for point count analysis.
The asbestos abatement must be performed by a TDSHS licensed asbestos contractor in accordance with a project design prepared by a TDSHS licensed asbestos consultant. In addition, third party air monitoring must be performed during the abatement.
It is important to note the TAHPR and NESHAP require that written notification be submitted before beginning renovation projects which include the disturbance of any ACM in a building or facility, or before the demolition of a building or facility, even when no asbestos is present. This written notification must be provided to the TDSHS at least 10 working days prior to the commencement of asbestos abatement or demolition activities. These activities must be performed in accordance with the current TDSHS, EPA, and OSHA guidelines.
