About this file

This is a Drug Free Workplace Certification form for the Town of Palm Beach, Florida, required as part of the Marine Resource Assessment & Monitoring Services contract opportunity. The certification mandates that the grantee establish and maintain a drug-free workplace by publishing a statement prohibiting controlled substance use, establishing a drug-free awareness program, requiring employee acknowledgment of the policy, and implementing procedures for addressing violations. The form requires the authorized agent to certify compliance with all specified requirements, including notifying employees of penalties, maintaining records of drug-related convictions, and taking appropriate personnel action within 30 days of receiving notice of employee convictions related to controlled substances.

The certification serves as a compliance document that must be completed and signed by an authorized representative of the responding firm prior to contract award. The form captures the firm name, authorized agent information, and contact details, and requires both a wet signature and electronic signature. This is a mandatory requirement for all proposers responding to the Town of Palm Beach's request for qualifications for marine resource assessment and monitoring services, ensuring that any selected contractor maintains drug-free workplace standards throughout the contract period.
