About this file
Attachment H is a qualification requirements document for the Carlsbad State Veterans Cemetery project in New Mexico, specifying insurance documentation requirements for general contractors and subcontractors bidding on this construction opportunity. The project involves construction and site work for a new state veteran's cemetery on a 15-acre tract in Carlsbad, including an approximately 2,810 GSF administration and maintenance building, a committal shelter with columbarium units, paved asphalt roads, concrete infrastructure, ornamental and chain-link fencing, in-ground crypts and cremains placement, xeriscaping, and irrigation systems. The project includes full mechanical, plumbing, fire suppression, and electrical systems with Dark Sky-compliant site lighting, with Alternate #1 encompassing interior improvements to the administration and maintenance building.
Bidders must submit two insurance-related documents: a letter from their insurance carrier on company letterhead listing the firm's Workers' Compensation Experience Modification Rate (EMR) for the most recent five-year policy period average, and either an ACORD insurance form or a carrier letter confirming the firm's ability to obtain insurance coverage in the limits specified in the General Contractor Qualification Statement Section 7.iii and, if applicable, the Subcontractor Qualification Statement Section 7.vi. These documentation requirements establish the minimum insurance qualifications necessary for contractor eligibility on this public works project.
