TECHNICAL PROPOSAL SUBMISSION AID

Solicitation No.: 1282AK20R0003

Project Title: Rio Grande Supervisor’s Office Construction

NOTE: This form is provided as an aid, but is not required. Proposals may be submitted in another format addressing all criteria in Section L.

1. Technical Approach and Capability: This section of the technical proposal must describe the offeror's proposed approach to performing the requirement as detailed in the solicitation and its attachments. The contractor must clearly describe their approach to meeting all requirements stated in the scope. The Offeror’s Technical Approach and Capability proposal shall include the following:

A. Resource Availability and Allocation.

B. Subcontracting Plans:

i. Work proposed to be completed by your company versus work proposed to be sub-contracted.

ii. Past Experience of Proposed Subcontractors.

C. Quality Control Plan. Provide a quality control plan for how quality control will be implemented on this project.

D. State Certification for Asbestos Abatement.

2. Management Approach and Capability: This section of the technical proposal must describe the offeror's proposed approach to managing the offeror’s employees, as well as any subcontractors and subcontractor employees, to promote timely performance, quality of performance, and customer satisfaction throughout contract performance. In addition, proposals must identify key personnel who will serve in management positions throughout the project. At a minimum, the proposal shall address the following:

A. Plan to Identify, Address, and Resolve Subcontractor Issues proactively.

B. Key Personnel:

i. Identify the Site Safety and Health Officer, Quality Control Manager, Project Manager, Superintendent, and Scheduler.

ii. Provide resumes, experience, certifications, trainings, and organizational charts showing hierarchical relationships of key personnel.

C. Schedule: Offerors are asked to provide a schedule of completion of work. This schedule should identify major milestones for the project and provide a completion period between 18 and 24 months in accordance with Section C, C.4. Schedule. Upon award, contract time will be identified on the award document to provide the period of performance based on the offeror’s proposed schedule of completion.

3. Experience and Past Performance: Specifically the USDA Forest Service is looking for past Federal contract experience and past similar contract experience working on hazardous materials abatement. At a minimum, experience and past performance shall include:

A. Experience: Summary of Previous Projects with similar scope, references/contractor evaluations from customers for previous projects. The offeror may use the form entitled "Experience Questionnaire" in the Proposal Submission Package or a different format that includes the information identified in the "Experience Questionnaire" to provide information regarding completed projects similar in scope and complexity to this requirement. The offeror shall limit submissions to no more than six (6) projects that were completed in the last six (6) years.

NOTE: Experience Questionairre is below to assist in response. This form is not required, but provided as a tool.

B. Past Performance (which is separate than past experience):

i. Have you received a marginal or unsatisfactory performance evaluation in last 6 years? If yes, explain.

ii. Have Liquidated Damages been assessed on any projects in the last 6 years? How many days per project? If yes, explain.

iii. References Shall Comment on:

1. Quality of Work

3. Schedule and Performance Management

4. Documentation/Administration

5. Cost Control

6. Major Challenges and/or Successes *** Offerors may use the attached past performance rating form to submit reference information.

The offeror may also provide finalized performance evaluations received from former contracts completed in the past six (6) years and similar in scope and complexity to this requirement.

Past performance information should be generated by previous customers and should not be a self-declaration of capabilities.

Past performance information regarding contracts with the Federal Government, other public agencies, or private organizations will be considered. The past performance information submitted may be that of the offeror's company or its proposed subcontractors. Subcontractor past performance will only be considered if the past performance relates to the work the subcontractor will be performing on the contemplated contract.

The offeror must disclose any instances in which its past performance on a particular contract may be considered by others to be less than fully satisfactory by attaching a narrative to the applicable Past Performance Questionnaire. The offeror should relate pertinent facts and circumstances and describe any remedial action taken or to be taken to correct the deficiency. Failure to disclose such instances may result in a determination that the offeror has been less than candid with the Government, which could result in an unfavorable assessment of the offeror's past performance record.

The Government reserves the right to limit the number of references it ultimately contacts and to use sources other than those provided by the offeror to obtain information related to past performance. Other sources include, but are not limited to: the Past Performance Information Retrieval System (PPIRS), now part of the Contract Performance Assessment Reporting System (CPARS) and/or Federal Awardee Performance and Integrity Information System (FAPIIS) also a module in CPARS. Limited exchanges between the Government and an offeror may occur to clarify the relevance of an offeror's past performance information and adverse past performance information to which the offeror has not previously had an opportunity to respond.

☐ d. Safety Plan.

The contractor shall provide an overview of their company Construction Safety Program and a narrative discussion of its assessment of the safety risks associated with the work, including any changes it would make to its company Construction Safety Program to address its assessment of the project risk on the this contract. The offeror shall include the names, resumes, and specific safety training (i.e., OSHA, etc.) for the proposed Project Site Safety Manager for this project.

Provide a brief summary of the contract team’s financial capacity, including bonding capacity, working lines of credit, limits of insurance, credit rating (Dunn-Bradstreet) and other relevant information. Must also provide Tax Identification Number (TIN).

Biobased Certification

1. Offerors shall identify the Biobased products to be purchased and used under this contract. For each Biobased product. the offeror shall specify the percentage of Biobased content, and for the USDA-designated Biobased content products, the offeror shall demonstrate that the products to be used under this contract will contain the percentage specified in the USDA recommendations or the highest level of Biobased material practicable, consistent with USDA's recommended percentages of Biobased content Product Description % Biobased Content

2. The offeror shall document prior experience in specifying, purchasing, using, and installing Biobased products. The offeror shall provide a list for all relevant contracts over the past two years involving the specification, purchase, and/or use of Biobased products. The offeror shall include a list of the Biobased products specified, purchased, used, and installed.

If so, then I feel like we may want to tweak some language in performance level section because it reads specific to the past performance, but I think we are combining the past performance and other evaluation criteria at this point to communicate our assessment of their ability to perform this project in consideration of past performance and other technical proposal requirements.

If this is for the past experience and previous customer is reviewer, then I think language is fine but we need to clarify that expectation with some prose.
