About this file
This is a Past Performance Questionnaire (PPQ) template for the Library of Congress Transportation Services IDIQ solicitation (030ADV26R0011). The questionnaire is designed to collect past performance information from current or previous customers of contractors bidding on the contract to assess their suitability for providing packaging, storage, and transportation services for Library materials across four performance areas: Non-Fragile, Fragile, Fine Arts, and Film/Media. Respondents must rate contractor performance across ten evaluation criteria using a five-point scale (Outstanding, Good, Satisfactory, Marginal, or Unsatisfactory) covering overall management, product and service quality, project phase management, schedule compliance, reporting timeliness, subcontractor effectiveness, personnel competency and communication, cost performance, and contract requirement fulfillment. Narrative explanations are required for any Marginal, Unsatisfactory, or N/A ratings.
Completed questionnaires must be submitted via email to Contract Specialist Nikolas Wellstead (nwellstead@loc.gov) with a copy to Contracting Officer Jennifer Zwahlen (jzwa@loc.gov) by 12:00 PM ET on February 20, 2026. The questionnaire requests general company information including contractor name, point of contact, telephone number, contract number, contract type, period of performance, dollar value, and contract description. This template allows both handwritten and typed responses and emphasizes that honest assessments from Government or company personnel are critical to the proposal evaluation process for this firm-fixed-price IDIQ contract with a June 1, 2026 start date.
