The Great Lakes Acquisition Center (GLAC), on behalf of the Edward Hines, Jr. VA Hospital (Hines VA), 5000 South 5th Avenue, Hines, IL 60141-3030 and Jesse Brown VA Medical Center (JB VAMC), 820 South Damon Avenue, Chicago, IL 60612-3728 (Hines VA and JB VAMC collectively herein called VA) intends to award a five year Blanket Purchase Agreement (BPA) for Therapeutic Apheresis Treatment Services (Apheresis Treatments) under authority of Title 38 U.S.C. 8153, Health Care Resources (HCR) and pursuant to FAR 13.5 Certain Commercial Items. BPA effective date will be July 1, 2020.
Contractor shall provide on-site Apheresis Treatments on-demand 24 hours per day, 7 days per week, including all Federal holidays. Specified VA Laboratory personnel will call contractor to schedule routine or emergency service. VA shall give Contractor a minimum of 12 hours notice when scheduling routine Apheresis Treatments. Contractor shall arrive on-site two hours prior to beginning routine services. Contractor shall arrive on-site within 4 hours of VA Laboratory phone request to begin emergency services.
Apheresis Treatments shall be provided by Registered Nurses or allied health professional(s) who possess the following competencies:
Operational knowledge with specific instruments used by the Contractor through documented training and competency records.
Knowledge of the basic principle of separation.
Knowledge of transfusion of blood components and the requirements of fluid replacement after removal or exchange.
Expertise in the management of adverse effects of therapeutic apheresis and transfusions.
Contractor shall provide all materials and supplies (except replacement fluids, albumin or plasma), equipment, personnel and qualified supervision that are required to provide routine and Urgent On-Site Apheresis Treatment at the Hines VA and at the JB VAMC. Contractor shall always maintain equipment in good-working order in accordance with manufacturer specifications as well as remain compliant with applicable safety regulations. Contractor shall have a contingency plan that keeps VA requirements met in the event of any equipment failure.
Historically, the two sites have received contracted services as follows:
Edward Hines, Jr. VA Hospital, 10 Apheresis Treatments per year
Jesse Brown VA Medical Center, 60 Apheresis Treatments per year
All qualified Contractors who believe they can provide the required services are encouraged to respond to the follow:
Contractor s background and experience in providing Apheresis Treatment services.
Plans regarding equipment placement, and if not leaving equipment on-site, plans to get equipment to VA in cases of off hour and emergency treatment requirements.
Contractor has a sufficient amount of licensed Registered Nurses who are trained in Apheresis Treatment procedures and are available to commence working on or about July 1, 2020.
Contractors that are Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) or Veteran-Owned Small Businesses (VOSBs) shall include proof of verification in the VIP database at www.vetbiz.gov.
Contractors shall include proof of registration in the System for Award Management (SAM) system (www.SAM.gov).
Pricing is requested and will be used as part of market research.

The Department of Veterans Affairs Veterans Integrated Service Network (VISN) 9 Network Contracting Office (NCO) 9 in the interest of Memphis VA Medical Center located at 1030 Jefferson Avenue Memphis, TN 38104 intends to issue and award a Sole Source Firm-Fixed Price award, Cost-Per-Test, to Ortho Clinical Diagnostics Inc. for commercial off the shelf (COTS) immunohematology analyzer, reagents, and consumables. The award will consist of a one-year base period and four yearly options. The period of performance for the base year will be 10/01/2022-09/30/2023. Ortho Clinical Diagnostics Inc is the only source available to provide an immunohematology analyzer that automates in vitro Immunohematology testing of human blood utilizing ID-MTS Gel Cards and is capable of pre-transfusion and compatibility testing for patients in throughout the hospital. The gel technology analyzer is capable of ABO/Rh Grouping (forward and reverse), ABO/Rh confirmation, Antibody Screens, Antibody Identification, Selected Cell Panel, Donor Confirmation, Crossmatch (AHG), Antigen Typing, Serial Dilutions for Titration Studies, DAT (Polyspecific), DAT (IGG, and Cord Blood Testing. The equipment will be used to optimize laboratory efficiencies. Memphis VA Medical Center currently utilizes 2 systems that are from Ortho Clinical Diagnostics and changing equipment would require the lab to revamp their entire blood bank processes.
Market research has determined that Ortho Clinical Diagnostics has a strategically sourced Federal Supply Schedule contract (V797D-60637) for Clinical Analyzers, Laboratory, Cost-Per-Test under FSC Group 66 III which is valid through 7/14/2026. Responses to this notice must demonstrate clear and convincing evidence that competition would be advantageous to the Government, would not be cost or time prohibitive, and would not hinder the mission objective of Memphis VA Medical Center. The determination not to compete this proposed contract action is solely within the discretion of the Government. Information received will normally be considered for the purposes of determining whether to conduct a competitive procurement.
The Government will not be responsible for any costs incurred by responding to this notice. This is not a request for quote or proposals. The Department of Veterans Affairs, Network Contracting Office 9, is seeking qualified sources to provide onsite Board Certified/Board Eligible Hematology/Oncology Physician services for beneficiaries at the Robley Rex Veterans Affairs Medical Center in Louisville, Kentucky. The anticipated period of performance is one year, and the contractor will be required to provide outpatient consultation services for the diagnosis and treatment of hematologic and medical oncology diseases, with a minimum of 3 six-hour clinic sessions per week. The contractor must provide a minimum of 4 key personnel, and the Government will pay a firm fixed percentage of the Medicare rate for the services. Responses to the Sources Sought notice are due by September 6, 2024 at 5:00 PM Eastern Time, and the Government will determine the appropriate acquisition method based on the responses received.
This procurement is not set aside for any specific business size. The current estimated demand covers a range of hematology and oncology diagnoses, with the highest percentages in blood disorders (44.4%) and lung cancer (9.7%). The Government has not provided any information on potential incumbent contractors or the anticipated award value or budget range for this requirement.

Statement of Work (SOW)
A. GENERAL GUIDANCE
Title of Project- Establish a Services Contract for Preoperative Blood Cell Saver Autologous Transfusion.
Scope of Work: Vendor shall provide Preoperative Blood Cell Saver Autologous Transfusion Services to support the veteran beneficiary patients in the Operating Room working in conjunction with the blood bank in the laboratory and the operating room in the collection and processing of shed blood from patients for reinfusion into the same patient at surgical or postoperative bedside; and sequestration of platelet rich plasma from patient s whole blood for topical application by a surgeon; provide all equipment and supplies necessary to recover and process patient s shed blood; and provide services in accordance with American Association of Blood Bank (AABB) standards.
Background: WJBD Medical Center is a 216-bed facility, encompassing acute medical, surgical, psychiatric, and long-term care. The hospital is located in Columbia, South Carolina and provides primary, secondary, and some tertiary care. Satellite outpatient clinics located in Anderson, Greenville, Florence, Orangeburg, Rock Hill, Sumter and Spartanburg, South Carolina.
Performance Period: The contractor shall provide services required under this SOW during the one (1) calendar year period beginning on the award date, with four option years, unless otherwise directed by the Contracting officer (CO).

B. GENERAL REQUIREMENTS:
A Contracting Officer Representative (COR) and/or Alternate Contracting Officer Representative (ACOR) shall be assigned to this procurement and written notification shall be provided to the Contractor.
Scheduling of work performance shall be arranged through the designated COR/ACOR assigned.
The Contractor shall provide preoperative Blood Cell Saver Autologous transfusion services to the Veteran beneficiary patients identified by the Operating Room requiring this service. The Contractor shall work in conjunction with the blood bank in the laboratory and the operating room in the collection and processing of shed blood from patients for reinfusion into the same patient at surgical or postoperative bedside; and sequestration of platelet rich plasma from patient s whole blood for topical application by a surgeon.
The Contractor shall provide all equipment and supplies necessary to recover and process patient s shed blood.
The Contractor shall provide services in accordance with American Association of Blood Bank (AABB) standards.
The Contractor shall ensure a Contractor Medical Director is always available by cell phone and/or pager during the procedure for physician conferences.
The Contractor shall perform semi-annual education for hospital staff regarding autologous
transfusion.
ORDERING PROCEDURES:
Operating Room (OR) Physician places an order through a note on the surgical request sheet.
The OR Medical Support Assistant prepares an e-mail in VISTA to the Cell Saver Group (G.BLOOD, #1 = Blood Bank).
The email requests that a Cell Saver Case be scheduled and shall include the following information and upon receipt of the email, the Authorized Staff listed shall log the information on the form called REQUEST FOR INTRAOPERATIVE AUTOLOGOUS TRANSFUSION .

Date of Birth/Age
Date of Surgery/Name of Procedure
Time of Surgery

Any changes referencing Authorized Staff as stated shall be communicated to the Contracting Specialist/Officer and the Contractor in writing throughout the life of this service requirement as submitted by the COR.
QUALIFICATIONS:
The Contractor shall provide competency assessments, evaluations, credentials, licensure and/or any other documents on its employees required to comply with the accreditation policies and standards of the Joint Commission.
Proof Credentials (ex. RN CCA, LPN CCA, CST CA, CA, EMT CA, LPN CST CA, as are now on file in the Blood Bank department, per CAP guidelines).
Be a citizen of the United States.
Be proficient in spoken and written English.
Contractor shall be a legally registered business in the state of South Carolina.
In accordance with VHA Directive 6500, Appendix G, Department of Veteran s Affair (VA) National Rules of Behavior, each Contractor must read and sign the VA National Rules of Behavior prior to gaining any access to VA information and/or information systems. Contractors must initial and date each page of the copy of the page of the VA National Rules of Behavior; they must also provide the information requested on the last page, sign, and date it.
These requirements shall be maintained in a Contractor employee file by the CO/COR for each Contractor employee working on this service.
CONTRACTOR RESPONSIBILITIES:
The Contractor shall bear the expense of obtaining background investigations.

The Office of Security and Law Enforcement adds an administrative fee. Patricia Moore, 202-461-5240 can be contacted to obtain information of the current amount of the fee.
The Contractor shall prescreen all personnel requiring access to the computer systems to
ensure they maintain a U.S. citizenship and can read, write, speak, and understand the English language. The Contractor shall provide the screening results of the VA Contracting officer prior to award and anytime new employees are hired.
The Contractor shall submit or have their employees submit the following required forms to the VA Office of Security and Law Enforcement within thirty (30) calendar days of receipt:
FD 258, U.S> Department of Justice Fingerprint Application Chart.
VA Form 0710, Authority of Release of Information form.
The Contractor, when notified of an unfavorable determination by the government, shall
withdraw the employee from consideration from working under this service.
