About this opportunity

The Department of Veterans Affairs Veterans Integrated Service Network 21 seeks to establish and operate a Community Based Outpatient Clinic in Yreka, California, through an IDIQ contract for primary care services, with proposals due May 15, 2026. This is an SDVOSB sole source set-aside, restricting competition to service-disabled veteran-owned small businesses registered in the SBA certification database. Questions are due April 12, 2026 at 10:00 AM PST.

Place of performance is Yreka, California, within city limits. The clinic will serve an estimated 953 veterans (maximum capacity 1,350) from the Sacramento VA Medical Center under the Patient Aligned Care Team (PACT) model and must commence operations within 120 calendar days of award.

The procurement requires comprehensive primary care staffing including a Physician Director, primary care providers (1,200 active patients per full-time physician or 900 per midlevel), registered nurse care managers, clinical associates, administrative associates, clinical pharmacist practitioners, licensed social workers, registered dietitians, and specialty providers (podiatrists, physical therapists). The contractor must provide all primary care personnel, clinic operations, initial mental health screening, care coordination, and facility equipment and furnishings; the VA provides telehealth services with VA-supplied equipment, specialty care, medications, vaccines, clinical pharmacy consultation, and mental health clinical services. Clinic hours are 7:30 AM to 5:30 PM Monday through Friday, with minimum eight hours daily operation.

Contract duration is ten years (September 1, 2026, through August 31, 2036) with a guaranteed minimum of $1,000 and ceiling of $18 million. Reimbursement is per member per month capitation based on qualifying encounters with billable roster of assigned veterans; the contractor must accept VA payment as payment in full and cannot bill beneficiaries or their insurance.

All contractor personnel must complete VA mandatory training, ACLS/BLS certification, information security and privacy training, VA EHR training, and Rules of Behavior sign-off prior to system access. Providers must obtain state licensure, register with VetPro credentialing system, and complete clinical reminders at 90% monthly standard. The facility must meet PACT Space Module Design Guide standards, automatic fire sprinkler and alarm systems, panic alarm coverage, intrusion detection at external doors, and CCTV monitoring of building entrances and restricted areas. The contractor must obtain Certificate of Occupancy or fire protection engineer certification before clinic opening.

Performance standards require new primary care appointments within 20 calendar days at 76% acceptable quality level; established patients within 20 calendar days of patient-indicated date at 94% AQL; behavioral health screening at 80% AQL; cardiovascular/diabetes risk management at 65% AQL; prevention measures at 75% AQL; and post-discharge contact within two business days at 48% AQL. Environment of Care deficiencies must be closed within 14 business days or have documented plan for action.

The contractor must prohibit abortions and abortion counseling, except life-saving treatment for pregnant women in life-threatening circumstances, ectopic pregnancies, spontaneous abortions, and other treatments necessary to preserve a pregnant veteran's life. The contractor is prohibited from racially discriminatory diversity, equity, and inclusion activities in recruitment, employment, contracting, and program participation per clause 52.222-90. The NAICS code is 621498, All Other Outpatient Care Centers; PSC is Q201, Medical-Managed Healthcare. Contracting Officer is Maricela Berrones-Gauger. Amendment 0006 (issued May 13, 2026) clarifies that registered dietitian and social worker services are provided by VA through consultation or telehealth unless the offeror proposes on-site staffing, and that core teamlet staffing ratio assessment considers reasonable transitions, recruitment efforts, credentialing timelines, and temporary vacancies.
